Objetivo: revisar a literatura sobre a influência do tratamento periodontal convencional não cirúrgico (TPC) no controle glicêmico em pacientes diabéticos e diagnosticados com periodontite. Material e Métodos: utilizou-se os descritores Diabetes mellitus, Periodontitis e Therapy, em inglês, combinados, para busca na base de dados PubMed, de 2015 a 2019. Um total de 233 artigos foram encontrados, após leitura de seus resumos, selecionou-se 25, que estavam relacionados ao tema. Estes foram lidos na íntegra. Revisões de literatura, pesquisas in vivo e ensaios clínicos que não analisaram sistemicamente os pacientes, especialmente quanto aos exames glicêmicos, ou que objetivaram avaliar efeitos de fármacos foram excluídos. Resultados: a maioria dos ensaios clínicos selecionados analisou prospectivamente a presente associação. Vinte estudos indicaram que o TPC pode influenciar positivamente no controle glicêmico em pacientes com DM e periodontite. As citocinas pró-inflamatórias são reconhecidas por aumentarem a resistência celular periférica à ação da insulina e possuírem relação causal com DM. O presente resultado foi associado ao potencial do TPC, realizado através de raspagens supra e subgengivais, em reduzir a carga microbiana local e, consequentemente, a concentração maciça de mediadores inflamatórios no sulco ou bolsa periodontal, reduzindo a disseminação sérica dos mesmos. Conclusão: o TPC apresenta benefícios sistêmicos ao facilitar o controle glicêmico em pacientes diabéticos. Sugere-se que as visitas ao cirurgião-dentista sejam protocolizadas no tratamento de pacientes diabéticos
A osseointegração de implantes endósseos (OI) é um processo dinâmico, envolvendo diversos tipos celulares, buscando o íntimo contato entre osso alveolar e superfície do implante. Os anti-inflamatórios não esteroidais (AINES) são frequentemente prescritos no pós-operatório da instalação de implantes devido a seu efeito analgésico, entretanto, por interferirem com a atuação de prostaglandinas (PGs) ao inibir enzimas ciclooxigenases (COX), podem prejudicar o processo de OI. Assim, o presente artigo objetivou, mediante uma revisão na literatura científica, analisar a influência do uso de AINES no processo de OI. Para isso, realizou-se uma busca por artigos científicos na base de dados PubMed com os termos "dental implants" AND "osseointegration" AND "antiinflammatory agents, non-steroidal". A busca resultou em 20 manuscritos e, após a leitura de seus resumos, foram selecionados, sem qualquer restrição de ano, 10 artigos. Estes foram lidos na íntegra e corresponderam a pesquisas científicas pré-clínicas e clínicas diretamente relacionadas ao efeito de AINES na OI ou a atuação de COX neste processo. Foram excluídos artigos referentes a outros fármacos e outras revisões de literatura. A maioria dos estudos selecionados sugeriu que os AINES podem intervir negativamente na OI, especialmente se tratando da atividade inibitória proeminentemente sobre COX-2, interferindo no equilíbrio osteoblástico e osteoclástico característico da remodelação óssea contínua. No reparo ósseo, a ativação de COX-2 mostra-se importante na ativação primária de osteoclastos, sendo crucial para turn over ósseo e efetividade de OI. Nota-se, entretanto, escassez de estudos na literatura científica que possibilitem a afirmação concreta da influência de AINES sobre OI.
A periodontite, desordem periodontal sucessora da gengivite, é uma doença de caráter infectoinflamatório e de alta prevalência mundial, a qual pode acarretar em perda dentária, já que tem como uma das principais características a reabsorção óssea alveolar. Seu tratamento convencional consiste em raspagem e alisamento corono-radicular com finalidade de reduzir a carga microbiana local, e consequentemente seus efeitos inflamatórios destrutivos. Apesar disso, sabe-se que alguns pacientes, em decorrência de suas individualidades, podem ter uma responsividade complicada à terapia convencional da periodontite, e, portanto, considerando os atributos dessa patologia, vale ressaltar a busca por métodos de tratamentos químicos auxiliares para tal doença. Dessa forma, devido ao polifenol resveratrol (RSV), presente em uvas e casca de amendoim, apresentar propriedades antibacteriana, anti-inflamatória e antioxidante, essa substância tem sido bastante estudada quanto ao seu uso em doenças infectoinflamatórias, como a periodontite. Assim sendo, essa revisão objetiva analisar a literatura no que se refere à ação adjuvante do RSV no tratamento da doença periodontal. Para isso, foi feita uma busca no banco de dados PubMed, utilizando os descritores "Resveratrol", "Periodontitis", "Therapy", reunindo estudos que avaliaram o potencial do RSV na periodontite. Após a leitura dos resumos dos 31 artigos encontrados, 13 foram selecionados. Em síntese, a maioria deles demonstrou benefícios na utilização do RSV na terapia da periodontite, uma vez que tal polifenol apresentou redução da inflamação e da reabsorção óssea na maior parte dos estudos. Dessa forma, por essas ditas propriedades biológicas, o RSV apresenta potencial adjuvante na terapia da periodontite.
