Study Design: Mandibular reconstruction is a challenge. Several reconstructive techniques are available, and the individualization of choice for each patient leads to better results and quality of life for the individual. Objective: The aim of this study is to evaluate the characteristics and complications of cases submitted to mandibular reconstruction with autogenous bone graft block of the iliac crest. Methods: Records of 45 patients undergoing mandibular reconstruction with autogenous bone graft block of the iliac crest were analyzed from January 2000 to December 2014. The data collected included age, gender, etiology and graft size, surgical approach, complications, comorbidities, and habits and addictions. Analysis of variance, chi-square test, and Fisher exact test were used for analysis of the variables with a significance level of P < .05. Results: The success rate of the 45 charts analyzed was 75.6%. No statistical differences were found between age and presence of complications, between defect size and presence or absence of complications, and between type of surgical approach and presence or absence of complications. Conclusion: According to our study, medical history may influence postoperative complications and require attention, though further studies should be performed to further elucidate the relationship between diseases and postoperative complications.
Introduction: Verrucous carcinoma is a non-metastatic variant of squamous cell carcinoma. It was first reported by Ackerman in 1948. It is a verrucous exophytic tumor and, although it is a rare lesion, it mainly affects the oral cavity, with slow growth and can be locally invasive. Objective: To report a case and present an observational and retrospective analysis of medical records containing biopsy data from patients diagnosed with oral verrucous carcinoma (OVC) in an oral diagnostic referral service for a period of 24 years. Material and methods: The following data were collected: age, sex, ethnicity, anatomical location and management. Result: Eight cases of OVC were found, all in Caucasian patients (n = 8, 100%), aged between 57 and 102 years. 62% of the injuries affected women. The most affected region was the jugal mucosa (n = 4, 50%). In addition, Smoking patients were not found. In all cases, incisional biopsy was performed and referral to the head and neck surgeon. Conclusion: Caucasian women in the sixth and seventh decades of life were the most affected by OVC. The absence of smoking patients corroborates the literature, which reports that cigarettes appear to be unrelated to this injury. The dentist's role is to recognize the disease early, diagnose it and refer it for medical treatment.
There are few reports in the literature concerning needle fractures during the local administration of anesthetic solution in dental procedures. However, the risk still exists. The patient’s unexpected movement during anesthesia, bending the needle prior to its use in the tissue, incorrectly performed anesthetic technique, uncooperative patient, small needle diameter or size may be considered substantial risks of fracture. At the moment the needle fracture occurs in the tissues it can move to the patient’s vital regions, and it is of utmost importance to remove it. If it is not possible to remove the needle immediately, an oral surgeon should be contacted to remove it under general anesthesia at the hospital. This paper aims to report a case of needle fracture in third molar exodontia during lower alveolar, lingual and buccal right nerve block, referred to the oral and maxillofacial surgery and traumatology department of the State University of Maringá. After clinical examination and tomography assessment, the patient underwent a surgical procedure to remove the needle in the operating room under general anesthesia and aid of trans-operative fluoroscopy. Therefore, we conclude that fluoroscopy is an extremely important resource in cases such as this one, since it allows a guided procedure, reducing the surgical trauma of an exploratory surgery.
