For posterosuperior tears, even though these are not absolute contraindications, the following preoperative variables have been found to significantly correlate with poorer outcomes when performing a latissimus dorsi tendon transfer (or possibly even other tendon transfers, but very few data are available concerning these tears): previous shoulder surgical procedure, atrophy and fatty infiltration (Goutallier stage of $3) of the teres minor muscle, subscapularis insufficiency (positive lift-off test), pseudoparetic shoulders (defined as those with massive rotator cuff tears and with active elevation of ,90°), and passive forward elevation of #80°.Disclosure: There was no source of external funding for this study. ResumoNo contexto do tratamento cirúrgico dos pacientes jovens com lesões irreparáveis da porção posterossuperior do manguito rotador, a técnica mais usada é a transferência do tendão do grande dorsal para a porção superolateral do tubérculo maior, conforme descrita e preconizada por Gerber et al. Entretanto, duas características dessa técnica podem levar a resultados ruins e complicações: (i) a deiscência da origem do deltoide, que ocorre devido à sua violação durante a criação da via em golpe de sabre e (ii) a ruptura pós-operatória da inserção da transferência. Na tentativa de solucionar esses dois problemas, as seguintes modificações foram feitas à técnica cirúrgica original. Por meio de uma única via deltopeitoral, o tendão do grande dorsal é isolado e desinserido do úmero. Ele é então alongado e reforçado com um enxerto tendíneo homólogo, transferido ao redor do úmero e fixado à porção superolateral do tubérculo maior. Não foi usada imobilização pós-operatória com órtese toracobraquial rígida.

ObjectiveTo evaluate the results of arthroscopic treatment of large and extensive rotator cuff injuries (RCI) that involved the supra and infraspinatus muscles using the suture bridge (SB) technique.MethodsBetween July 2010 and November 2014, 37 patients with RCI who were treated with SB technique were evaluated. The study included all patients with a minimum follow-up of 12 months who underwent primary surgery of the shoulder. Twenty-four patients were male and 13 were female. The mean age was 60 years (45–75). The dominant side was affected in 32 cases. The most common cause of injury was trauma (18 cases). The mean preoperative motion was 123°, 58°, T11. Through magnetic resonance imaging, 36 fatty degenerations were classified according to Goutallier. Patients underwent rotator cuff repair with SB technique, which consists of using a medial row anchor with two Corkscrew® fibertape® or fiberwire® at the articular margin, associated with lateral fixation without stitch using PushLocks® or SwiveLocks®.ResultsThe mean age was 60 years and mean fatty degeneration was 2.6. The mean range of motion (following the AAOS) in the postoperative evaluation was 148° of forward elevation, 55° in lateral rotation and medial rotation in T9. Using the criteria of the University of California at Los Angeles (UCLA), 35 (94%) patients had excellent and good results; one (2.7%), fair; and one (2.7%), poor.ConclusionArthroscopic repair of a large and extensive RCI using SB technique had good and excellent results in 94% of the patients.

Resumo Objetivos Avaliar os resultados clínicos dos pacientes submetidos ao tratamento artroscópico da lesão parcial da porção articular do manguito rotador pelas técnicas de sutura transtendão e após completada a lesão e comparar o tempo de recuperação pós-operatório das duas técnicas. Método Estudo retrospectivo baseado na identificação de todos os casos com lesão parcial da porção articular do manguito rotador submetidos a tratamento artroscópico no período de outubro de 1999 a dezembro de 2016 pelo Grupo de Ombro e Cotovelo da nossa instituição. Foram incluídos 39 pacientes divididos em 2 grupos: os que foram submetidos à técnica transtendínea e os outros em que a lesão foi completada. Os dois grupos foram estatisticamente semelhantes. Foi realizada a análise dos respectivos prontuários, da identificação da técnica aplicada, dos resultados pós-operatórios e das complicações relacionadas. A avaliação funcional foi realizada através do escore da University of California at Los Angeles (UCLA). Resultados Não houve diferença estatisticamente relevante entre os grupos, com escore UCLA médio de 32, não havendo diferença também no tempo necessário para reabilitação. Conclusão Não houve diferença entre o resultado clínico dos pacientes, independente da técnica empregada para o reparo das lesões parciais da porção articular do manguito rotador com resultados satisfatórios em 93% dos casos.

An association between closed posterior elbow dislocation and traumatic brachial artery injury is rare. Absence of radial pulse on palpation is an important warning sign and arteriography is the gold-standard diagnostic test. Early diagnosis is essential for appropriate treatment to be provided. This consists of joint reduction and immobilization, along with urgent surgical restoration of arterial flow. Here, a case (novel to the Brazilian literature) of an association between these injuries (and the treatment implemented) in a 27-year-old male patient is reported. These injuries were sustained through physical assault.

Resumo Objetivo Identificar agentes bacterianos em amostras de cirurgias do ombro de pacientes sem histórico de infecção e de cirurgias prévias no ombro. Métodos Amostras de tendão, bursa e osso foram coletadas no intraoperatório, armazenadas em frascos estéreis a seco e enviadas para análise de crescimento de cultura em meios para agentes aeróbios e anaeróbios no laboratório credenciado ao hospital. Foram analisados os resultados de 141 amostras de 47 ombros. Resultados Obtivemos resultados de culturas negativas em 46 casos (97,8%) e em 140 amostras (99,2%). Apenas um paciente apresentou resultado positivo, com crescimento bacteriano do Staphylococcus hominis em uma das três amostras coletadas. Conclusões Não evidenciamos taxas de crescimento bacteriano condizentes com a literatura internacional, alertando para a baixa eficácia dos métodos laboratoriais utilizados no nosso país.

The shoulder is the most unstable joint in the human body. Traumatic anterior instability of the shoulder is a common condition, which, especially in young patients, is associated with high recurrence rates. The effectiveness of non-surgical treatments when compared to surgical ones is still controversial. The purpose of this study was to review the literature for current concepts and updates regarding the treatment of this condition.
