IntroduçãoO Brasil atualmente está entre os países que mais realiza transplantes no mundo. Entretanto, a fila de espera por um transplante ainda é grande. A falta desses órgãos é geralmente devido a insuficiência de doadores e a fatores de obtenção, conservação e transporte [1].Segundo a Associação Brasileira de Transplantes de Órgãos (ABTO), no país o aproveitamento de órgãos é em torno de 70% para rim, 55% fígado, 10% coração, 3,8% pâncreas e 2,6% pulmão [2].Os protocolos da Agência Nacional de Vigilância Sanitária (ANVISA) para transporte de órgãos são baseados em hipotermia (baixas temperaturas) [3]. O acondicionamento correto de órgãos é dentro de caixas térmicas, imersos em solução de conservação, temperatura de aproximadamente 4 o C, e o tempo limite de 4 horas no caso do coração [1].Em virtude das perdas decorrentes das atuais técnicas de conservação em hipotermia e da insuficiência de doadores. Esse estudo tem o objetivo de expor as inovações tecnológicas que podem vir a agregar métodos diferentes dos convencionais adotados no país. Materiais e MétodosNesta revisão literária, foram selecionados trabalhos que abordam os transplantes de órgãos nos últimos dez anos. Foram feitas pesquisas em várias bases de dados com os seguintes descritores (transplante cardíaco, OCS e acondicionamento de órgãos) e incluídos estudos in vitro e in vivo. Como critérios de exclusão não foram analisados textos incompletos e resumos. Toda a busca foi realizada on-line. Descrição do Organ Care SystemTecnologias como o OCS da TransMedics, mantêm órgãos (coração, pulmões e rins) em ambiente semelhante ao fisiológico. É um equipamento médico hospitalar capaz de ressuscitar, perfundir e aquecer conservando-os por mais tempo, até 12h [4].O OCS (Figura 1) é uma plataforma que preserva a perfusão do coração, que permanece batendo até a realização do transplante, não sendo submetido a hipotermia, a temperatura é mantida em torno de 34 o C, evitando assim os prejuízos de morte tecidual da isquemia fria [5].Dentre os benefícios proporcionados temos o aumento do tempo de até 4 horas para até 12 horas desse órgão ex-vivo preservado, a possível ampliação do número de pacientes aptos aos transplantes, alcançando distâncias maiores que não seriam viáveis por questão de tempo na técnica convencional. Dispondo ainda ao cirurgião implantador mais tranquilidade durante a cirurgia, em virtude do fator tempo ampliado, além da possibilidade de avaliação prévia do órgão antes do procedimento [6].
