Due to rare occurrence and similarity to botryoid odontogenic cyst and low-grade mucoepidermoid carcinoma GOC is difficult to identify. It remains without clinical signs for years and is identified in routine X-rays in most cases. We recommend cystectomy after use of Carnoy's solution for intravital fixation. No filling material is required for complete bony healing.
oregut or enterogenous cysts are the result of an abnormal budding or division of the primitive foregut and are usually found in adults. On the basis of histologic features and embryogenesis, they are classified into bronchogenic, esophageal, gastroenteric, and neuroenteric cysts and usually occur in the mediastinum or, less frequently, in the soft tissues of the chest wall. 1 Intrasternal foregut cysts lined by respiratory epithelium have never previously been described in the literature, and we present here the first case.
ZusammenfassungNach einer großen Tumorregisterstudie wird das 15 Jahre kumulative Risiko eines asynchronen bilateralen Keimzelltumors mit 3,9 % angegeben [29]. Im Vergleich mit der Allgemeinbevölkerung ist das relative Risiko (RR) 36fach erhöht [27]. Die Inzidenz eines kontralateralen Tumors korreliert eng mit dem Nachweis einer testikulären intraepithelialen Neoplasie (TIN). Hodentumorpatienten haben zusätzlich ein erhöhtes Risiko für das Auftreten eines keimzellunabhängigen Zweittumors, insbesondere Patienten, die radiotherapiert wurden. Wir berichten über einen 61 Jahre alten Mann mit einem asynchronen kontralateralen Seminom 29 Jahre nach Orchiektomie wegen eines Teratoblastoms und partieller Lungenresektion einer Lungenmetastase und Nachbestrahlung. Zwei keimzellunabhängige intestinale Zweittumoren (Sigma-und Rektumkarzinom) wurden ein Jahr vor der kontralateralen Orchiektomie durch eine Sigma-Rektumexstirpation behandelt. Der Patient verstarb zwei Jahre später postoperativ nach einer ausgedehnten intraabdominellen Debulking Operation des metastasierten kolorektalen Karzinoms. AbstractIn a large tumor registry study the 15-year cumulative risk of developing a second germ cell tumor for patients previously treated for testis cancer is reported to be 3.9 % [29]. The relative risk is 36-times higher than in the general population [27]. The incidence of a contralateral tumor is closely related to the presence of testicular intraepithelial neoplasia (TIN). Testis cancer patients are also known to be at an increased risk of developing a second non-germ malignandy, especially in patients previously treated with radiotherapy. We report a 61 year old man with an asynchronous contralateral seminoma 29 years after orchiectomy for teratoblastoma and partial lung resection and radiotherapy of lung metastasis. Two second non-germ cell intestinal carcinoma (sigma and rectum) were treated by sigma-rectum exstirpation one year before contralateral orchiectomy. The patient died two years later after intraabdominal debulking surgery of metastatic disease of the colorectal carcinoma.
| Der Urologe [B] 4•2002 336 Zystische Nephrome (CN) sind seltene, gutartige Tumoren der Nieren, die meistens als Zufallsbefund im Rahmen von sonographischen Routineuntersuchungen der Abdominalorgane entdeckt werden. Klinische Symptome können in Form von Flanken-oder Oberbauchbeschwerden bestehen. Die bildgebenden Verfahren (Sonographie, Computertomographie und Kernspintomographie) zeigen in der Regel einen polyzystischen Tumor unterschiedlicher Größe mit inhomogener Septierung. Eine Abgrenzung gegen ein zystisches Nierenzellkarzinom ist ohne histologische Klärung nicht möglich. Die Therapie besteht in einer kompletten Tumorexzision oder Nephrektomie.Wir berichten über 4 Patientinnen mit CN, die in den letzten 4 Jahren in unserer Klinik behandelt wurden. Zystische Nephrome sind grundsätzlich gutartige Tumoren. Die zur Verfügung stehenden bildgebenden Verfahren wie Sonographie, Computertomographie oder Kernspintomographie erlauben keine eindeutige Abgrenzung zu einem malignen zystischen Tumor, sodass die Diagnose nur durch eine histopathologische Untersuchung des Tumors zu stellen ist.Die Freilegung, Exzision des Tumors und Schnellschnittuntersuchung, bietet zum einen die Sicherheit einer histopathologischen Klärung der Diagnose, zum anderen die Möglichkeit, so viel wie möglich an funktionstüchtigem Nierenparenchym zu erhalten.In unserer Klinik wurden in den letzten 4 Jahren 4 Patientinnen mit einem zystischen Nephrom behandelt.Bei einer Patientin war es aufgrund der Tumorgröße möglich, den Tumor komplett zu resezieren und die Niere zu erhalten. In den drei anderen Fällen war wegen der Größe des Tumors eine Nephrektomie unumgänglich.
