Introdução: A Esclerose Lateral Amiotrófica (ELA) é uma doença que se manifesta pela degeneração dos neurônios motores superiores e inferiores. A degeneração dos neurônios provoca a perda de contração muscular voluntária e o controle dos movimentos, de forma importante sobre os músculos respiratórios, o que leva à uma expectativa de vida máxima de três a cinco anos após o início dos sintomas. As evidências sugerem que o percurso clínico da doença está altamente relacionado a funções e capacidades respiratórias. Objetivo: Traçar o perfil epidemiológico, função pulmonar e incidência de hipoventilação de pacientes portadores de ELA. Metodologia: Estudo transversal retrospectivo, com coletas de dados de prontuários dos pacientes que já vieram a óbito atendidos no setor Distrofias do Hospital Júlia Kubitschek de Belo Horizonte. Resultados: Foram coletados 46 prontuários de paciente com ELA com média de idade de 59,98±11,66 com 63,8% do sexo masculino e 84,8% com diagnóstico de ELA esporádica. O tempo de evolução da doença foi em média de 3,37±2,87 anos. Apresentaram em média capacidade vital de 1,19±0,57 litros e com presença de hipoventilação alveolar em 90,3% dos indivíduos. Conclusão: A grande maioria dos dados foram compatíveis com a literatura atual e com o curso clínico da doença, porém ainda são necessários mais estudos dessas variáveis para melhor caracterização desses pacientes.
Objective: To describe the clinical profile of individuals with Amyotrophic Lateral Sclerosis (ALS) from a reference hospital in the city of Belo Horizonte, Brazil. Method: This is a retrospective cross-sectional study with data collection from electronic medical records of individuals with a defined diagnosis of ALS between 2010 and 2020, in the Muscular Dystrophies reference sector of a hospital in a Brazilian capital. Results: A total of 103 individuals with ALS were included, with a mean age of 60±12 years, mean diagnostic age 56±12 years, and mean time of disease progression of 3±3 years. Furthermore, 70% were male, 88% with sporadic ALS, with a similar bulbar involvement between mild (32%), moderate (27%) and severe (28%), and with a higher rate of diagnosis from 50 to 70 years of age. Conclusion: The epidemiological data from this study are very similar to those in the literature. However, the heterogeneity of the disease, the complexity of the diagnosis and the diversity of forms that each study brings to the disease, and especially the rapid progression, make a more extensive picture difficult to be discussed. Tracing this profile is important for a more focused clinic and a more adequate management, and for that, further studies are needed.
ResumoObjetivos Avaliar o estado nutricional dos pacientes em hemodiálise (HD). Métodos Estudo transversal realizado em todos os pacientes em HD de um único centro.Resultados Foram estudados 72 pacientes, sexo masculino (57%) e 45,8% entre 45 e 64 anos. A etiologia mais frequente foi hipertensão arterial sistêmica (50,0%) e diabetes (20,8%). O teste de depuração fracional da ureia (KT/V) > 1,2 foi encontrado em 88,8% dos pacientes, e nível de fósforo > 5,5 mg/dL em 25%. Os desnutridos classificados pelo índice de massa corporal (IMC) < 25 Kg/m 2 (66,67%), índice de circunferência muscular do braço (CMB) (84,72%) e albumina ( 4,0 g/dL) (28,8%), sendo que 22,22% utilizam suplemento nutricional oral específico para paciente dialítico hiperproteico hipercalórico, fornecido pelo serviço de hemodiálise. O ganho de peso interdialítico médio (GPIDm) foi < 2,5kg em 58,3%, e o ganho de peso interdialítico relativo (GPIDr) foi < 4,5% em 75,0% dos pacientes. Houve associação significativa entre o GPIDm e a CMB, pacientes com menor GPIDm ( 2,5 Kg) tendem a ter mais desnutrição leve ou moderada (p ¼ 0,003). Não foram observadas diferenças por tempo em hemodiálise em relação à idade, ganho de peso interdialítico, medidas laboratoriais e antropométricas. Conclusão A avaliação nutricional pelo IMC esteve de acordo com a literatura brasileira. Pela CMB, a maioria foi classificada como desnutridos, o que difere da albumina, pela qual apenas $ 30% eram desnutridos. Os resultados mostram a importância da suplementação para esse público. Vimos que > 75% dos pacientes estão dentro do GPIDr ideal (< 4,5% do peso seco). O tempo de diálise não afetou os parâmetros de avaliação nutricional.
