INTRODUÇÃO: A má oclusão de Classe II é muito comum, e, para se eleger o momento adequado para intervenção e selecionar o tipo de tratamento, diversos fatores devem ser levados em consideração. OBJETIVO: Relatar um caso de tratamento ortodôntico de uma paciente com 9 anos de idade e má oclusão de Classe II, divisão 2 de Angle, com protrusão maxilar e ausência de espaços para a erupção dos caninos superiores permanentes. RELATO DO CASO: Como a família recusou a cirurgia ortognática e devido ao perfil facial, a opção de tratamento escolhida foi a camuflagem ortodôntica, sendo realizadas extrações dos primeiros pré-molares superiores previamente ao tratamento ortodôntico, para que fossem criados espaços para a erupção dos caninos superiores. RESULTADOS: Ao fim do tratamento, pôde-se observar uma estética facial agradável e boa oclusão, que se manteve estável 10 anos após o tratamento ortodôntico. CONCLUSÃO: Entre as diversas abordagens possíveis, a realização de exodontias de dois primeiros pré-molares superiores e camuflagem ortodôntica é uma alternativa de tratamento viável para a má oclusão de Classe II, demonstrando resultados adequados, melhorando a qualidade de vida, a estética e a função dos pacientes.
Introduction: External apical root resorption (EARR) is characterized by the definitive loss of tooth root structure, with a higher incidence in lateral and central maxillary incisors. Objective: To identify, in different chronological periods, the incidence of EARR in the maxillary incisors (MI) of patients orthodontically treated with or without premolars extraction. Methods: Periapical radiographs before and after orthodontic treatment of 1,304 MIs from 326 patients (205 women and 121 men) were evaluated for EARR, divided into five groups, according to the chronological period in which treatments were started: G90) from 1990 to 1994, G95) from 1995 to 1999, G00) from 2000 to 2004, G05) from 2005 to 2009, G10) from 2010 to 2015. The evaluation was performed in each group, in patients who underwent maxillary first premolars extraction and those who did not. For statistical analysis, Fisher’s exact test was used, with a significance level of p < 0.05. The EARR was measured using the adapted Levander and Malmgren classification. Results: Incidence of EARR was higher in MIs of patients treated with maxillary premolar extraction (p < 0.05) in two chronological periods (G00 and G10), also being influenced by orthodontic treatments with longer duration, and due to possible individual genetic factors. Conclusion: Even with the limitations of a retrospective study, the lack of a defined EARR pattern in the MIs at different chronological periods was larger in the experimental group, due to the sum of factors such as premolars extraction, prolonged orthodontic treatment, possible genetic characteristics, and root shape, without the influence of the sex and age.
Genetic polymorphisms could explain the inter-individual differences in the oral health-related quality of life (OHRQoL) of children with anterior open bite (AOB). Objective: To assess the impact of AOB on OHRQoL in children and to evaluate whether MTR (rs1805087), MTRR (rs1801394), TGFβ1 (rs1800469) and TNF-α (rs1799964, rs1799724 and rs1800629) genes are potential biomarkers for OHRQoL in children with AOB. Study design: A cross-sectional study was performed with 173 children aged between 2–6 years. The Brazilian version of Early Childhood Oral Health Impact Scale (ECOHIS) was applied. Genetic polymorphisms were analyzed using real-time PCR. Mann-Whitney U-test and Chi-square were used. Results: The overall mean ECOHIS scores were 5.49 (SD= 5.72) and 3.45 (SD = 4.49) (p < 0.01) in the AOB and control groups, respectively. Children with the CC genotype of TNF-α (rs1799724) had a significantly higher psychological QoL level. The MTRR AA genotype group showed a lower QoL level in the child subscale (p = 0.006), function (p = 0.017), and psychological (p = 0.006) domains. There was no significant difference between OHRQoL and the genetic polymorphisms in MTR and TGFβ1. Conclusions: Genetic polymorphisms in TNF-α and MTRR are associated with the impact on the OHRQoL in children with AOB.
The upper airways (UA) are influenced by the growth and development of the craniofacial complex and any alteration in the maxillary bones during orthodontic treatment can narrow or increase the size of the pharyngeal spaces. The three-dimensional analysis of the volume and the minimum axial area of the respective sub-spaces is beneficial for case planning as it allows the identification of possible physical barriers that compromise the air passage and the respiratory flow, besides being an auxiliary diagnostic method for disorders related to this function. The request for cone-beam computed tomography (CBCT) is necessary for the correct digital evaluation with the appropriate protocols for the acquisition and reconstruction of the image exams, calibration of the methodology, and standardization of their analysis. The objective of this work is to demystify the specific step-by-step analysis in the Dolphin Imaging® software and to describe the technical and anatomical factors for delimiting the spaces corresponding to the UA. To illustrate this protocol, CBCT of a patient with Angle Class II malocclusion, first division were analyzed before the installation (T1) of a mandibular thruster (fixed cemented Herbst appliance with dental anchorage) and after 12 months of treatment (T2). From the measurement of the spaces corresponding to VAS, comparing the two treatment times, it was possible to analyze the changes in the pharyngeal space after the use of mandibular thruster and to highlight the importance of the three-dimensional assessment of the air dimension for individualized orthodontic planning.
INTRODUÇÃO: Pacientes portadores de má oclusão de Classe III geralmente manifestam impactos negativos decorrentes dessa condição, seja em relação à estética facial ou função. Quando a má oclusão não compromete a estética facial, e a discrepância maxilomandibular não é excessiva, um tratamento ortodôntico-cirúrgico pode ser preterido em detrimento à compensação ortodôntica. RELATO DE CASO: Esse relato descreve o tratamento ortodôntico de uma paciente com 17 anos de idade, portadora de má oclusão de Classe III por protrusão mandibular, associada a mordida cruzada posterior e mordida anterior em topo-a-topo. A intervenção na dimensão transversa foi realizada com um aparelho de Porter. Para resolução da Classe III dentária e da mordida em topo-a-topo, o tratamento escolhido foi a compensação ortodôntica com auxílio de ancoragem óssea, para retração de toda a arcada inferior; sem interferência na face. RESULTADOS: As características faciais foram mantidas, com melhora importante na oclusão, resolvendo os problemas nos sentidos transverso e anteroposterior, apresentando relação de molares e caninos em Classe I; foram obtidas guias excursivas bem estabelecidas, além de sobremordida e sobressaliência adequadas. CONCLUSÃO: Devido à variedade de dispositivos e alternativas de abordagens, é importante identificar os limites de correção das discrepâncias ósseas e compensações dentárias, para escolher a terapêutica a ser adotada nos casos limítrofes; atentando-se, ainda, para as queixas do paciente. E, nesse relato de caso, a compensação da Classe III com auxílio de miniplacas mostrou-se uma alternativa acertada, restabelecendo a função e alcançando os objetivos do tratamento.
INTRODUÇÃO: Muitos ortodontistas preferem tratar a Classe II em uma fase. Porém, quando o paciente apresenta características que aumentam o risco de fraturas dos incisivos e afetam sua autopercepção, podendo levar ao bullying, deve-se considerar o tratamento da Classe II em duas fases, que pode ser uma excelente alternativa, devido às características que indicam essa abordagem. OBJETIVO: Descrever os resultados do tratamento ortodôntico em duas fases de um paciente com Classe II dentária e esquelética, sobressaliência de 10 mm, sobremordida exagerada e padrão facial braquicefálico, com queixas do comprometimento da estética dentária e qualidade de vida, além de histórico de bullying. RELATO DE CASO: Primeira fase do tratamento: aparelho funcional Twin- -Block, com objetivo de promover projeção mandibular, melhorar a discrepância esquelética e o perfil, impactando positivamente na estética e autopercepção. Segunda fase: aparelho ortodôntico fixo, para alinhamento, nivelamento e estabelecer oclusão funcional. RESULTADOS: Excelente oclusão posterior, sobremordida e sobressaliência adequadas, estética dentária e função. CONCLUSÃO: O diagnóstico antes do pico do surto de crescimento puberal e o correto manejo ortodôntico permitiram obter resultados estéticos e funcionais, sem a necessidade de extrações ou futura cirurgia ortognática, podendo ser uma estratégia anti-bullying, com efeitos positivos na autopercepção de adolescentes, além de ser considerado uma alternativa que pode proporcionar benefícios estéticos, funcionais e melhora relacionada à qualidade de vida que não seria obtida no tratamento em uma fase.
