IntroduçãoAmostragem por saturação é uma ferramenta conceitual freqüentemente empregada nos relatórios de investigações qualitativas em diferentes áreas no campo da Saúde, entre outras. É usada para estabelecer ou fechar o tamanho final de uma amostra em estudo, interrompendo a captação de novos componentes. Objetivamos, neste artigo, refletir criticamente sobre alguns fundamentos metodológicos e técnicos de seu emprego.O fechamento amostral por saturação teórica é operacionalmente definido como a suspensão de inclusão de novos participantes quando os dados obtidos passam a apresentar, na avaliação do pesquisador, uma certa redundância ou repetição 1 , não sendo considerado relevante persistir na coleta de dados. Noutras palavras, as informações fornecidas pelos novos participantes da pesquisa pouco acrescentariam ao material já obtido, não mais contribuindo significativamente para o aperfeiçoamento da reflexão teó-rica fundamentada nos dados que estão sendo coletados. Esta conotação/definição já vinha presente no texto que parece ter inaugurado o uso da expressão saturação teórica (theoretical saturation) 2 .No entanto, é necessário problematizá-la porque, embora possa parecer um procedimento decorrente de uma constatação facilmente atingível, muitas vezes a averiguação de saturação ARTIGO ARTICLE
A transparência metodológica na pesquisa qualitativa é fator que contribui para sua confiabilidade, devendo ser garantida pelos pesquisadores. A utilização do critério de fechamento amostral por saturação é freqüente, entretanto a maneira como este processo ocorreu é raramente explicitado nos relatórios de pesquisa. Dificuldades técnicas para uma constatação objetiva da saturação também são comuns entre os pesquisadores qualitativos. Neste texto, propõe-se um modo prático de organizar o trabalho de fechamento amostral por saturação, expondo uma seqüência de oito passos procedimentais de tratamento e análise de dados coletados em entrevistas não-dirigidas com questões abertas ou em grupos focais. Almeja-se contribuir para que os pesquisadores explicitem objetivamente o modo como a saturação ocorreu e, aos seus leitores, conhecer como se deu este processo.
RESUMO.A técnica de fechamento do número amostral por saturação teórica, comum nas pesquisas qualitativas, em particular na área clínica, dialoga com a teoria e a prática psicanalítica e delas pode se beneficiar. Embora certa imprecisão do número amostral final seja inevitável e inerente aos métodos qualitativos, ela não deveria levar a técnicas e a procedimentos que possam fragilizar ou comprometer a validade empírica e a credibilidade das análises e achados. Este ensaio aborda aspectos das dimensões cognitiva e afetiva, tanto do participante quanto do pesquisador, que se mostram presentes durante a coleta de dados por meio de entrevistas. Para isso são utilizados os conceitos freudianos de transferência e contratransferência e as noções de rêverie e de elementos beta, formuladas por Wilfred Bion. Postula-se que fenômenos dessas ordens ajudam a determinar quanto de dados advindos dos elementos amostrais é suficiente, necessário e possível para o adensamento teórico sobre o objeto estudado.Palavras-chave: Pesquisa qualitativa; coleta de dados; psicanálise. THEORETICAL SATURATION IN QUALITATIVE RESEARCH: PSYCHOANALYTICAL CONTRIBUTIONSABSTRACT. The methodological technique of theoretical saturation, commonly used in qualitative researches, particularly about clinic activities, dialogues and can benefit from theory and practice of psychoanalysis. Although some imprecision in the final sample size is inevitable and inherent to qualitative methods, it should not lead to techniques and procedures that can weaken or jeopardize the validity and credibility of both empirical analysis and findings. This essay discusses aspects of cognitive and affective dimensions present during data collection through interviews, in both informants and researchers. For this, the Freudian concepts of transference and countertransference are used, and also the notions of rêverie and beta elements, formulated by Wilfred Bion. It is postulated that these kind of phenomena help determine how much data coming from the sample elements are sufficient, necessary and possible to enhance the theoretical depth of the studied object. Background This study investigated the psychocultural perspectives concerning family quality of life among Brazilian families with children who have severe or profound intellectual disability. Methods Individual in‐depth semi‐structured interviews conducted with 15 mothers, selected by convenience, were analysed using a categorical thematic analysis technique. The themes were examined to allow for an interpretative approach of the results. Results Mothers revealed that their children with disabilities had insufficient access to services and support related to health care, transportation and recreation. Family quality of life was negatively affected by financial restrictions and social interaction difficulties. Caring for a child with disabilities seemed to be centred on the mother and religious coping appeared as a common psychological adjustment strategy. Conclusions Improving emotional and psychological cares, as well as social and practical measures comprising income support and access to appropriate health care, were inferred to be the mothers’ priorities to improve their families’ quality of life.
COLECTA DE DATOS EN LA INVESTIGACIÓN CLÍNICO-CUALITATIVA: EL USO DE ENTREVISTAS NO-DIRIGIDAS DE PREGUNTAS ABIERTAS POR LOS PROFESIONALES DE SALUD
BackgroundIn Brazil, alcohol consumption is estimated to range from 7 to 40% in pregnant women. This research investigated the motivation for alcohol consumption or abstinence during pregnancy in a purposive sample of Brazilian women.MethodsClinical-qualitative research was conducted, in which 14 women participated, identified as risk-drinkers during pregnancy by the T-ACE screening tool. Data were collected through semi-structured individual interviews. Reports were recorded, transcribed and investigated by a thematic content analysis approach. The results were discussed based on a comprehensive and interpretive framework.ResultsSixteen themes were extracted and these were then classified into three thematic categories: (a) general motives for alcohol use, (b) specific motives for drinking during pregnancy, and (c) reasons for partly or fully abstaining from drinking during pregnancy. Social motives were the most relevant, particularly due to deeply rooted cultural values. Enhancement motives were widely mentioned and associated with a hedonic posture. Consumption also aimed to deal with negative emotions, characterizing two types of coping specifically to pregnancy: first, perceiving pregnancy as a period of less opportunity for social interaction and, therefore, drinking alcohol at home to compensate; secondly, perceiving pregnancy as a period of greater irritability, and hence experiencing difficulties to abstain. On the other hand, concern about fetal health, disapproval of consumption by relatives and health professionals, as well as the social construction of mothering were reported as reasons to abstain.ConclusionsSome specificities influence the decision to continue or discontinue alcohol consumption during pregnancy. To reduce consumption, we suggest educational actions based on a collective health perspective, articulated with individualized measures that allow identification and proper intervention for women at risk.
A Estratégia Saúde da Família é responsável por reorganizar o Sistema Único de Saúde brasileiro por meio da Atenção Primária. O aumento substancial de programas e vagas para residência em Medicina de Família e Comunidade, ocorrido desde 2002, é uma das estratégias para suprir o crescente mercado de trabalho correspondente. Entretanto, menos da metade dessas vagas são ocupadas. A literatura brasileira apresenta poucas evidências sobre o motivo desta baixa procura.Alguns países que optaram pelo fortalecimento da Atenção Primária em seu sistema de saúde também experimentam uma crise aparente na escolha desta carreira pelos egressos médicos. Neste ensaio, revisamos algumas questões envolvidas nesta escolha, apontando sua complexidade e a necessidade de investigações sistematizadas sobre as motivações dos alunos de graduação em optarem ou não por esta especialidade médica, particularmente no Brasil. A B S T R A C TThe Family Health Strategy is responsible for supporting the reorganization of Brazil's Unified National Health System (SUS) through primary care. A substantial increase in available residency positions in family and community medicine is one of the main strategies for supplying the growing professional demand related to this health policy. However, few medical students have chosen the family health career in recent years, and more than half of the vacancies remain unoccupied. The Brazilian literature shows little evidence to explain this fact. Some countries with a strong background in primary health care have also experienced a crisis in the primary care career choice. The current article discusses several arguments involved in this issue, emphasizing its complexity and the need for more scientific data concerning undergraduate students' motivations in choosing this medical specialty as compared to others, particularly in Brazil.
