The authors review the literature examining the validity and significance of cannabis withdrawal syndrome. Findings from animal laboratory research are briefly reviewed, and human laboratory and clinical studies are surveyed in more detail. Converging evidence from basic laboratory and clinical studies indicates that a withdrawal syndrome reliably follows discontinuation of chronic heavy use of cannabis or tetrahydrocannabinol. Common symptoms are primarily emotional and behavioral, although appetite change, weight loss, and physical discomfort are also frequently reported. The onset and time course of these symptoms appear similar to those of other substance withdrawal syndromes. The magnitude and severity of these symptoms appear substantial, and these findings suggest that the syndrome has clinical importance. Diagnostic criteria for cannabis withdrawal syndrome are proposed.
Withdrawal symptoms following cessation of heavy cannabis (marijuana) use have been reported, yet their time course and clinical importance have not been established. A 50-day outpatient study assessed 18 marijuana users during a 5-day smoking-as-usual phase followed by a 45-day abstinence phase. Parallel assessment of 12 ex-users was obtained. A withdrawal pattern was observed for aggression, anger, anxiety, decreased appetite, decreased body weight, irritability, restlessness, shakiness, sleep problems, and stomach pain. Onset typically occurred between Days 1-3, peak effects between Days 2-6, and most effects lasted 4-14 days. The magnitude and time course of these effects appeared comparable to tobacco and other withdrawal syndromes. These effects likely contribute to the development of dependence and difficulty stopping use. Criteria for cannabis withdrawal are proposed.
Among patients receiving buprenorphine-naloxone in primary care for opioid dependence, the efficacy of brief weekly counseling and once-weekly medication dispensing did not differ significantly from that of extended weekly counseling and thrice-weekly dispensing. Background: The relationship between marijuana smoking and pulmonary function or respiratory complications is poorly understood; therefore, we conducted a systematic review of the impact of marijuana smoking on pulmonary function and respiratory complications. Methods: Studies that evaluated the effect of marijuana smoking on pulmonary function and respiratory complications were selected from the MEDLINE, PsychINFO, and EMBASE databases according to predefined criteria from January 1, 1966, to October 28, 2005. Two independent reviewers extracted data and evaluated study quality based on established criteria. Study results were critically appraised for clinical applicability and research methods. Results: Thirty-four publications met selection criteria. Reports were classified as challenge studies if they examined the association between short-term marijuana use and airway response; other reports were classified as studies of long-term marijuana smoking and pulmonary function or respiratory complications. Studies have found that a variety of drug dependent groups discount delayed rewards more than matched-controls. This study compared delay discounting for a hypothetical $1000 reward among dependent marijuana users, former dependent marijuana users, and matched-controls. Discounting of marijuana was also assessed in the currently marijuana-dependent group. No significant difference in discounting was detected among the groups, however currently dependent users showed a trend to discount money more than the other two groups. Within the dependent marijuana group, marijuana was discounted more than money, and discounting for money and marijuana were significantly and positively correlated. Regression analyses indicated that delay discounting was more closely associated with tobacco use than marijuana use. A variety of questionnaires were also administered, including impulsivity questionnaires. Dependent marijuana users scored as significantly more impulsive on the Impulsiveness subscale of the Eysenck Impulsiveness-Venturesomeness-Empathy questionnaire than controls. However, the three groups did not significantly differ on several other personality questionnaires including the Barratt Impulsivity Scale-11. The Stanford Time Perception Inventory Present-Fatalistic subscale was positively correlated with money and marijuana discounting, indicating that a greater sense of powerlessness over the future is related to greater delay discounting. Results suggest that current marijuana dependence may be associated with a trend toward increased delay discounting, but this effect size appears to be smaller for marijuana than for previously examined drugs.
