AimTo test the effectiveness of motivational interviewing (MI) to reduce the risk of an alcohol exposed pregnancy (AEP) in a high-risk population.Design Randomized controlled trial.Setting Rural population in the Western Cape, South Africa.Participants A total of 165 women aged 18-44 years at risk of AEP.Intervention Five-session MI intervention.Measurements Structured questionnaires were administered pre-intervention and at 3 and 12 months follow-up. The primary outcome measure was AEP at 12 months. Secondary outcomes were AEP at 3 months, and alcohol use and effective contraception at 3 and 12 months.Findings There was a significant difference in the decline in the proportion of women at risk for an AEP in the MI group at 3 months (50 versus 24.59%; P = 0.004), maintained at 12 months (50.82 versus 28.12%; P = 0.009). In an intention-to-treat analysis these differences were also significant (32.93 versus 18.07%; P = 0.029; and 37.80 versus 21.69%; P = 0.024, respectively). The odds ratio for no longer being at risk of an AEP (MI versus control) at 12 months was 2.64 [95% confidence interval (CI): 1.18-5.94]. In the intention-to-treat analysis this ratio was 2.19 (95% CI: 1.05-4.65).Conclusions A five-session motivational interviewing intervention was found to be effective with women at risk of an alcohol-exposed pregnancy, and could be implemented as part of routine primary care clinic services in similar populations. The message of 'no alcohol in pregnancy' should be adapted to include better family planning and early recognition of pregnancy.

BackgroundEliminating silicosis is a priority of the International Labour Organization and the World Health Organization. Prevalence is particularly high in developing countries.ObjectivesWe describe trends in silicosis among South African gold miners who had had an autopsy between 1975 and 2007 and quantify the contributions of age at autopsy and employment duration to these trends.MethodsSouth African miners and ex-miners are eligible for autopsy examination for occupational lung disease, regardless of the clinical cause of death, and the families of deceased mine workers may receive compensation from the government of South Africa. Miners who died from external causes and who had been employed in the gold mines for > 1 year were stratified by population group because of differences in exposure, patterns of employment, and autopsy referral patterns. We extracted data from PATHAUT (Pathology Automation System) and used Stata 10 to estimate trends in relative proportions of silicosis that were standardized for age and employment duration.ResultsThe crude proportion of silicosis for white miners was six times that of black miners in 1975. By 2007, it was 1.5 times higher for black miners. The proportion of miners with silicosis increased from 0.03 to 0.32 for black miners and from 0.18 to 0.22 for white miners. The increase can be explained by increasing age and employment duration for white miners. For black miners, it can be only partly explained by these two factors.ConclusionAs miners continue to age and work for longer periods, the burden of silicosis will continue to rise. South Africa is committed to global efforts to eliminate silicosis by 2030. The autopsy database allows for disease surveillance, which is necessary to monitor the success of this initiative.

BackgroundThere is scanty or inconclusive evidence on which cervical cancer screening tool is effective and suitable for human immunodeficiency virus (HIV)-seropositive women. The aim of this review was to assess, synthesise and document published evidence relating to the available cervical cancer screening modalities for HIV-seropositive women in developing countries. This paper did not review the issue of human papillomavirus (HPV) prophylactic vaccine on HIV-seropositive women.MethodsFive electronic databases were systematically searched from inception to January 2018 for relevant published original research examining cervical cancer prevention modalities for HPV infection, abnormal cytology and direct visualisation of the cervix amongst HIV-seropositive women in developing countries. Extra studies were identified through reference list and citation tracking.ResultsDue to methodological and clinical heterogeneity, a narrative synthesis was presented. Of the 2559 articles, 149 underwent full-text screening and 25 were included in the review. Included studies were of moderate quality, and no exclusions were made based on quality or bias. There is no standard cervical cancer screening test or programme for HIV-seropositive women and countries screening according to available resources and expertise. The screening methods used for HIV-seropositive women are the same for HIV-negative women, with varying clinical performance and accuracy. The main cervical cancer screening methods described for HIV-seropositive women are HPV deoxyribonucleic acid/messenger RNA (DNA/mRNA) testing (n = 16, 64.0%), visual inspection with acetic acid (VIA) (n = 13, 52.0%) and Pap smear (n = 11, 44.0%). HPV testing has a better accuracy/efficiency than other methods with a sensitivity of 80.0–97.0% and specificity of 51.0–78.0%. Sequential screening using VIA or visual inspection with Lugol’s iodine (VILI) and HPV testing has shown better clinical performance in screening HIV-seropositive women.ConclusionAlthough cervical cancer screening exists in almost all developing countries, what is missing is both opportunistic and systematic organised population-based screenings. Cervical cancer screening programmes need to be integrated into already existing HIV services to enable early detection and treatment. High rates of mortality and of HIV infection and pulmonary tuberculosis were found in this cohort after leaving the South African goldmines. Continuing lung function loss was also apparent. A partnership between the South African mining industry and governments in labor-sending areas of Southern Africa is needed to provide continuity of care and access to HIV and tuberculosis treatment and prevention services. Active silicosis surveillance and an improved statutory compensation system are also needed. These findings can serve as a baseline against which the impact of such interventions can be assessed.
