Objetivo: realizar uma análise retrospectiva das características demográficas e clínicas de pacientes com artrite reumatóide (AR), em acompanhamento ambulatorial no Estado de São Paulo, Brasil. Métodos: foram revisados 1.381 prontuários de pacientes com artrite reumatóide, atendidos entre 2002 e 2005. Os prontuários foram transcritos em protocolos padronizados, coletando-se idade, sexo, raça, peso corpóreo, tempo de doença, gravidade da doença, classificação do estado funcional, fator reumatóide, condição social, presença de dor na última consulta, avaliação de progressão radiológica, envolvimento extra-articular, emprego de questionários de Qualidade de Vida ou de Atividade da Doença, tipos de tratamentos e de programa de reabilitação. Resultados: dos 1.381 pacientes, 86% eram mulheres, caucasianas, estavam entre a quarta e quinta década, tempo médio de doença de 7,2 anos e o peso corpóreo médio de 65,6 kg. Uma minoria (5%) foi classificada como grave, e a maioria apresentava classe funcional I e II. As manifestações extra-articulares ocorreram em 23,3%, e o fator reumatóide era positivo em 71% dos pacientes. Somente um terço possuía acompanhamento radiológico e estava trabalhando. O medicamento mais utilizado foi o metotrexato (dose 15-19 mg/semana). Um quarto dos pacientes estava freqüentando programas de reabilitação, e um terço possuía avaliações de qualidade de vida e de atividade da doença. Conclusão: essa análise proporcionou uma visão parcial dos pacientes brasileiros com AR, identificando pontos importantes em relação às características demográficas e clínicas, os tipos de tratamentos farmacológicos e a pequena utilização de marcadores de doença e questionários de qualidade de vida, bem como a limitação de serviços de reabilitação disponíveis para os pacientes.

Behçet's disease is a multisystemic vasculitis of unknown etiology, which is characterized by recurrent urogenital ulceration, cutaneous eruptions, ocular manifestations, arthritis and vasculitis, and its diagnosis is based on clinical criteria. Superior vena cava (SVC) thrombosis is a rare but well-recognized manifestation of Behçet's disease, whereas SVC syndrome due to vasculopathy, without evidence of thrombosis, has not yet been described in the literature. The authors report the case of a patient with Behçet's disease, who presented SVC syndrome with reduction in the lumen of the SVC due to thickening of the vessel wall, without evidence of thrombosis upon computed tomography and magnetic angioresonance. The patient received early anticoagulant therapy, corticosteroid and monthly cyclophosphamide pulse therapy. Clinical control without recurrence was observed after 6 months of follow-up. Behçet's disease should be suspected in young patients presenting with SVC syndrome, in the absence of thrombosis or of a hypercoagulable state.

ABSTRACT. We measured NF-κB, IKK, c-Fos, and GRα mRNA expression and in vivo glucocorticoid sensitivity in patients with rheumatoid arthritis. A very low dose intravenous dexamethasone suppression test and real-time PCR quantitation of mRNA of these genes were performed on blood samples from 21 rheumatoid arthritis patients who were not on glucocorticoids during the previous four months and on blood samples from 20 healthy individuals. Mean rheumatoid arthritis duration was 8.8 years, and mean disease activity, as assessed by Disease Activity Score 28 (DAS28), was 4.45. Basal cortisol and the percentage of cortisol reduction after the very low dose intravenous dexamethasone suppression test, as well as NF-κB, IKK, c-Fos, and GRα mRNA expression, were similar among groups. We did not observe significant correlations between glucocorticoid in vivo sensitivity and DAS28. In the multivariate analysis, only NF-κB mRNA remained as an independent variable for predicting DAS28.

ABSTRACT. Glucocorticoids (GCs) are key drugs in the treatment of systemic lupus erythematosus (SLE). GC dose reduction during remission is related to disease activity, GC dose used, length of treatment, and individual GC sensitivity. We compared GC receptor α (GRα) isoform and nuclear factor kappaB (NF-κB) messenger RNA quantitation and in vivo GC sensitivity between SLE patients during remission and healthy controls. We performed a cross-sectional study of 19 women aged 22-49 years, including 9 SLE patients in clinical remission taking ≤5 mg prednisone and 10 matched controls. We evaluated GC sensitivity using 2 cortisol suppression tests: a very-low-dose intravenous dexamethasone suppression test (VLD-IV-DST) and a low-dose oral dexamethasone suppression test. GRα and NF-κB mRNA were quantified using real-time polymerase chain reaction. Although basal cortisol and adrenocorticotropic hormone levels were similar between the groups, the percentage of cortisol reduction after the VLD-IV-DST was 56% lower in SLE patients than in controls (P = 0.014). GRα and NF-κB gene expression levels were similar between the groups. The low-dose oral dexamethasone test caused intense cortisol suppression in all individuals, limiting the ability of this test to discriminate individual GC sensitivity. A positive correlation was found between the extent of cortisol suppression in vivo (VLD-IV-DST) and the number of days elapsed since the last flare of lupus activity. Despite clinical remission, SLE patients displayed partial GC resistance recognized by the VLD-IV-DST. The mechanism of this resistance is unrelated to altered GRα and NF-κB mRNA expression.

Aim:To assess drug switching, rates of remission and disease activity in Brazilian patients with rheumatoid arthritis (RA) treated with biologic agents.Materials & methods:Using a retrospective method, a total of 94 adult patients were included.Results:Anti-TNF was the first choice therapy in 85 (90.4%) patients. After an average of 8 years of follow-up, 55 (59%) patients were taking anti-TNF, 18 (19%) abatacept, eight (9%) tocilizumab and 13 (14%) rituximab. In this period, 99 switches of biological therapy were registered in 55 patients.Conclusion:After 8 years of follow-up, 54% of the RA patients on biological therapy were still experiencing high or moderate activity despite established treatment, including switching between different biologic agents.

Objective: To evaluate the lipid profile in rheumatoid arthritis and its association with the disease activity. Methods: This was a prospective study with 21 female patients who had rheumatoid arthritis and whose progress was accompanied for 1 year. Total cholesterol, LDL-cholesterol, HDL-cholesterol, triglycerides, apolipoproteins A and B, lipoprotein (a), ESR, as well as HAQ and disease activity through DAS28, were evaluated. Results: After 1 year, the total cholesterol level was increased in 66.6% of the patients (p=0.019) and the LDL-cholesterol level was also elevated in 80.9% of the patients (p=0.008). In contrast, the lipoprotein (a) level was reduced in 33.33% of the cases (p=0.003). A significant decrease also occurred in ESR, HAQ and DAS28. However, there was no association between the alteration of the lipid profile and the disease activity. Conclusion: Although the reduction in the disease activity (DAS28) was significant, the lipid profile worsened.
