This is the first demonstration from a large database that not only average 24 h PP and SBP values, but also 24 h BP fluctuations, are associated with, and possibly determinants of, the alterations of large artery structure in hypertension.
The recently developed method of ultrarapid immunohistochemistry (IHC) was applied to the intraoperative examination of sentinel lymph nodes (SLNs) in breast cancer patients. In a prospective study of 50 patients with invasive breast carcinomas, a total of 60 SLNs were studied. Among them, 33 SLNs from 30 patients were studied intraoperatively using a direct immunoperoxidase method with anticytokeratin antibody clone MNF116. This technique has a turnaround time of less than 20 minutes. Ultrarapid IHC revealed 15 positive SLNs compared to 14 positive SLNs using hematoxylin and eosin (H and E) frozen sections. The one SLN missed in H and E frozen sections presented with cytokeratin-positive isolated tumor cells in the lymph node sinus. After paraffin embedding, H and E-stained serial step sections of the SLN specimens detected another two patients with isolated tumor cells. We also examined the remaining axillary lymph nodes (ALNs) by H and E-stained serial step paraffin sections. From 17 of the 30 patients with positive SLNs, 6 patients also had metastatic involvement of the ALNs of level I or II. Thus ultrarapid IHC was a very sensitive and rapid technique for the intraoperative detection of metastatic involvement of SLNs in breast cancer patients. This technique may be a useful complementary tool for the intraoperative study of SLNs, particularly in tumors that are a diagnostic challenge, such as lobular carcinoma.
Einleitung Das Bundesministerium für Bildung und Forschung (BMBF) hat Kompetenznetze in der Medizin mit dem Ziel eingerichtet, Kommunikationsstrukturen in Forschung und Therapie zu verbessern und die Interaktion von Wissenschaft und Patientenversorgung zu fördern. Die Ergebnisse der Forschung in den Universitätskliniken und anderen Forschungszentren sollen schneller und transparenter für die medizinische Regelversorgung zur Verfügung gestellt werden. Die Beteiligung der Regelversorgung an der Diskussion von Methodik und Inhalten von Forschung ist hierbei durchaus erwünscht. Das Kompetenznetz Maligne Lymphome (Sprecher Prof. V. Diehl, Köln) ist eines von insgesamt neun Kompetenznetzen in der Medizin (drei im Bereich maligner Erkrankungen). In die Förderung dieses Kompetenznetzes sind zur Zeit 11 Teilprojekte integriert, darunter ein Netz aus niedergelassenen Hämato-Onkologen. Im Teilprojekt der Schwerpunktpraxen haben sich 72 Praxen zusammengeschlossen, um im Sinne der oben skizzierten Ziele eine Plattform für Informationstransfer und Qualitätsmanagement für den Bereich der hämatoonkologischen Regelversorgung aufzubauen. Im Rahmen dieses Projektes haben wir einen Kurs "Evidence-Based Medicine (EBM)" durchgeführt, in dem eine wissenschaftliche Bewertung medizinischer Leistungen mit standardisierten Methoden vorgenommen wurde. Dieser Kurs vermittelt neben den Methoden auch die "Entscheidungskultur", die notwendig ist, um EBM, Leitlinien und das Health Technology Assessment (HTA) in der Praxis umzusetzen. Bei Umsetzung der EBM werden Entscheidungen für einen individuellen Patienten, bei der Leitlinie für den gesamten Einflussbereich einer medizinisch verantwortlichen Person (Leiter einer Praxis oder Krankenhausabteilung) und beim HTA für ein Gesundheitssystem getroffen [1, 2]. Um dem Anspruch neuer Kommunikationsstrukturen im Bereich des Qualitätsmanagements gerecht zu werden, haben wir ein Beispiel aus diesem Kurs, das Unterrichtsmodul "Bewertung eines diagnostischen Verfahrens", für eine Präsentation in einer wissenschaftlichen Zeitschrift aufgearbeitet. Ziel dieser Präsentation ist, die Strategie und Taktik vorzustellen, die angewandt wird, um für klinische Entscheidungen neben dem eigenen Wissen und der eigenen Erfahrung (internal evidence) auch das Wissen anderer (external evidence) verfügbar zu haben. Methode Das Szenario Um ein neues Thema mit den Methoden der EBM [3] aufzugreifen, hat es sich bewährt, das Problem anhand eines Szenarios aus dem klinischen Alltag darzustellen. Anschließend wird EBM in sechs standardisierten Schritten vollzogen.
38 Aim of our study was to evaluate the relationship of carotid intima-media thickness (IMT) with clinic (C) and 24 h ambulatory blood pressure (ABP) and with 24 h BP variability (V), from baseline data of the European Lacidipine Study on Atherosclerosis (ELSA). Design and Methods: In 1663 hypertensive patients in washout from treatment (56.2± 7.6 years; 54.4% males) several measures of maximum IMT were obtained from up to 12 different carotid sites by intensively trained and repeatedly certified sonographers. CBP was the average of 3 sitting measurements. From ABP recordings (Spacelabs 90207 or Takeda TM2021) we obtained: 1) 24 h, day and night average SBP and DBP, 2) day-night SBP and DBP differences of means ( -Δ), 3) 24h, day and night SBP and DBP standard deviations (SD, measure of BPV).Similar calculations were done for PP and HR. The influence of ABP and C variables on IMT was investigated by univariate and multiple regression analyses. Results: In the univariate analysis IMT was not related to C or 24h DBP, but was significantly related to C SBP (r from 0.16 to 0.21; p<0.001 for all), to C PP (r from 0.18 to 0.23; p<0.001) and even more to 24h SBP (r from 0.17 to 0.23; p<0.001) and to 24h PP (r from 0.24 to 0.32; p<0.001). In the multiple regression analysis, after accounting for age and average 24 h SBP or PP (p<0.001), IMT was positively associated with SBP and PP 24h SD (p<0.001) and negatively with SBP and PP day-night Δ and with 24h HR SD (p<0.05). Conclusions: This is the first demonstration from one of the largest available databases that the vascular adverse consequences of hypertension 1)are related more closely to 24h PP and SBP means than to C values, and 2)are related not only to mean BP levels but also to the degree of PP and SBP fluctuations over 24h.
