Objectifs : Analyser les résultats de l'implantation de ballonnets périurétraux (Pro-ACT ® ) dans la cure d'incontinence urinaire par insuffisance sphinctérienne (IU/IS) de l'homme, par une étude monocentrique rétrospective portant sur 60 patients consécutifs traités entre 2006 et 2009. Patients et méthodes : Nous avons rétrospectivement étudié les dossiers de 60 patients traités consécutivement, dans notre service, pour IU/IS par ballonnets Pro-ACT ® , entre avril 2006 et janvier 2009 (119 ballonnets ; un ballonnet non implanté du fait d'une brèche vésicale peropératoire). Un temps de rétrovi-sion du col vésical par cystoscopie souple a été pratiqué pour chaque patient, lors de l'implantation à partir du 21 e patient. Cinquante-six patients sur 60 (93,3 %) présentaient une IU/IS après prostatectomie radicale (PR). L'âge moyen lors de l'implantation était de 68,78 ans . Le délai moyen entre PR et implantation était de 4,04 ans [0-15]. Le nombre moyen de protections quotidiennes avant traitement était de 2,3 (/47 patients) [0 étui pénien]. Soixante patients (100 %) présentaient des fuites diurnes et 17 patients des fuites nocturnes (28,3 %). 98,3 % des patients (58/59) présentaient des fuites à l'effort, et 17,2 % (10/58) des fuites au repos. Cinq patients (8,5 %) ont eu un traitement par radiothérapie externe (quatre avant et un après implantation). Cinq patients avaient eu un traitement chirurgical de l'IU/IS préalable à l'implantation des ballonnets (8,5 %) [quatre sphincters artificiels urinaires [SUA] et une bandelette sous-urétrale [BSU] Invance ® ]. Le suivi moyen était de 8,9 mois (médian = 7,57 mois ; [0,9-29,3]).Résultats : Cinquante et un patients (85 %) se sont déclarés améliorés par le traitement, au moins lors d'une consultation de suivi, et cette amélioration a été constatée lors de plusieurs consultations (deux ou plus) consécutives, pour 80,4 % (41/51) d'entre eux. Le pourcentage moyen de l'amélioration maximale obtenue (score subjectif) sur ce groupe de 51 patients était de 67,5 %. Vingt-cinq de ces patients (49,1 %) avaient un score d'amélioration compris entre 80 et 100 %. 23,3 % des patients ont vu leurs fuites diurnes disparaître (14/60). 94,1 % des patients ont vu leurs fuites nocturnes disparaître (16/17). Le nombre moyen de protections est passé de 2,5/j en préopératoire à 1,24/j à la fin du suivi actuellement disponible (42 patients). Quatorze patients sur 42 évalués ne portent plus de protections (33,3 %). Le nombre moyen d'ajustements postopératoires par ballonnet et par patient était de 2,7 [0-7]. Le volume total moyen, injecté dans chaque ballonnet par le(s) ajustement(s) postopératoire(s), était de 1,79 ml [0-5,7]. Des complications à type de douleur, d'hématome, de rétention aiguë d'urines (RAU), de résidu postmictionnel (RPM) ou de dysurie ont été constatées chez 40 % des patients (24/60) et ont été transitoires pour la plupart. Vingt ballonnets ont dû être explantés (16,8 %) chez 12 patients (20 %). Les raisons de ces explantations ont été : quatre érosions urétrales, trois échecs...
