Our findings suggest a higher risk for a severe course of malaria in the elderly. For this reason, extensive advice on the correct use of exposure- and chemoprophylaxis of malaria is especially important for travellers above the age of 60.
Als Martin im Jahre 1765 zum ersten Male fiber heredit~r bedingte Schltisselbeindefekte berichtete, war yon den der Wissensehaft bekannten Anomalien des Skcletsystems lediglich die Chondrodystrophie und die Osteopsathyrosis bekannt, und aueh als 100 Jahre sps Scheuthauer erkannte, dal~ zu diesem Formenbild in allen F~llen Anomalien der Sch~delverkn6cherung geh6rten, stand unser Wissen yon den Systemerkrankungen des Skeletsystems wenigstens beim Lebenden noch am Anfang einer erst dureh Einsetzen des R6ntgenverfahrens sp~unghaft emporschnellenden Entwick|ung. So ist es auch kein Zufall, da6 kurz nach der Entdeckung der R6ntgenstrahlen Marie und Sainton dies seit ]~ngerem bekannte Krankheitsbild zusammenfassend neu darstellten und ihm seinen heute allgemein anerkannten Namen Dysostosis eleidocranialis (D.~c.) verliehen. Die in der Folge in erster Linie in der orthop~disch-anatomischen Literatur beschriebenen F~tlle best~tigen im wesentliehen die Beobachtungen ihrer Vorg/~nger und fiigten an neuen Ergebnissen ]ediglieh die mitnnter vorhandenen Ver~nderungen am Becken (Pattau[) und eine seltener bei D.c.c. vorkommende Verkiirzung und Verbreiterung der Mittel-und Endphalangen der Finger und Zehen hinzu (Jansen). An grSl3eren Arbeiten fiber D.c.c. sind in diesem Zusammenhang vor allem die hervorragenden Arbeiten yon Hultkranz im Jahre 1908 zu erw~hnen und eine in jfingster Zeit erschienene Arbeit yon Eva Engel, die al|e bis Ende 1933 bekannten 228 Fs berficksichtigt. Valentin, der 1937 im Sehwalbe-Gruberschen Handbuch fiber die ,,Morphologie der Mil~bildungen" eine eingehende Betrachtung der D.c.c. vornimmt und einc fast vollsts Literaturangabe bringt, scheint ]eider gerade diese Arbeit yon Engel entgangen zu sein. Ira ganzen diirften bis Mitte 1938 etwa 250 F~lle der D.c.c. bekannt geworden sein, deren Haulatcharakteristica die fo|genden sind: 1. Ein-oder doppelseitige, teilweise oder vollsts Aplasie der Sehl/isseibeine. 2. Anomalien der Sch/idelossifikation und des Zahnsystems.
Das Krankheitsbild der spastischen Bronchitis des S/iuglings ist durch eine mit Giemen und Pfeifen verbundene anfallsweise auftretende exspiratorische Atemnot gekennzeichnet. Da unter den zahlreichen an Bronchitis erkrankten S/iuglingen nur ganz ausnehmend wenige sind, die diese typischen Befunde aufweisen, da weiterhin die Erfahrung zeigt, dab es h/~ufig die gleichen Kinder sind, die bei versehicdenen Anl/~ssen mit den geschilderten Zeiehen erkranken und da ferner solche Anf/~lle, so schnell sic kommen, auch rasch --oft schon nach Stunden, 1/ingstens Tagen --wieder vergehen, besteht aller AnlaB, schon in der Namengebung die asthmatiforme Art eines solchen Zustandes hervorzuheben. Dabei bleibe es unerSrtert, ob tats/ichlich ein Spasmus der Bronchialmuskulatur vorliegt, eine Veriinderung der Schleimhaut dieser Abschnitte der Atemwege oder sonstige Ursachen zu jener eigentiimlichen Dyspnoe fiihren. Die in vielen Fallen giinstige Wirkung krampflSsender Mittel spricht jedenfalls sehr dafiir, dab mindestens bei einem nicht geringen Teil derart erkrankter Kinder ein Bronchialspasmus eine Rolle spielt.Die Ursache fiir einen solchen Spasmus kann durchaus verschieden sein. Banale Infekte kSnnen ihn ausl6sen, eine Bronchitis, aber aueh eine Pneumonie, ein Keuchhusten kSnnen ihn veranlassen; andere Kinder wieder erkranken ohne solche Anl/isse rein mit dem spastischen Syndrom. Man sieht hierin eine weitere Parallele mit dem echten Bronchialasthma.Betraehtet man sieh sehliefllieh entspreehend dem Vorgehen Striimpells beim erwachscnen Asthmatiker solche kranken Kinder genauer, so findet man immer wieder die schon im Jahre 1911 yon v. P/aundler niedergelegte Auffassung best/~tigt, iIn S/iuglingsasthma den nervSsen Anteil einer exsudativen Diathese manifestiert zu sehen, und erkeImt den engen Zusammenhang zwischen dieser Erkrankungsform und den anderen, in diesen Kreis gehSrenden katarrhalisch-exsudativen Erseheinungen, so vor allem der Urticaria, Ekzem, Idiosynkrasien, im weiteren aber aueh mit arthritisehen Ver/inderungen, labilen Temperaturen, u. a. (Cerny, v. P/aundler, Baagoe, Faerber).Bestehen also Beziehungen zwisehen dem oft qualvollen Leiden des Asthmatikers und einer asthmaartigen Erkrankung eines Situglings,
Background:Psoriasis (Pso) is one of the most common chronic inflammatory skin diseases in Europe. Psoriatic arthritis (PsA) is closely associated to Pso whereas the skin manifestation appears usually years before PsA-related symptoms emerge. Up to 30% of Pso patients develop PsA, but there is no clear correlation between disease duration of Pso and PsA development. Therefore, biomarkers for its early detection are of major importance. In early PsA, changes in synovial vascularisation combined with increased expression of proangiogenic factors appear first. Therefore, imaging biomarkers for detection of changes in vascularisation might be useful for early detection of musculoskeletal disease. Fluorescence-optical imaging (FOI) is a new method to detect changes in microvascularisation of the hands.Objectives:To determine the number of positive PsA diagnosis within a 24 month follow-up period in PsO only patients with subclinical MSK-inflammation detected in FOI.Methods:Sensitivity of FOI for detection of subclinical signs of musculoskeletal inflammation as biomarker for early PsA was observed in a prospective, multicentre study (XCITING) including patients with dermatological confirmed skin psoriasis. 411 patients were included from dermatology care units across Germany without diagnosis of PsA but potential risk factors for its development (nail psoriasis and/or joint pain or swelling within the last 6 months). Clinical examination (CE; swollen (66) and tender (68) joint count, enthesitis, dactylitis assessment) and standardised ultrasound (US) assessment was performed by a qualified rheumatologist to assess musculoskeletal inflammation. FOI was performed additionally. Data was analysed in focus on increased vascularisation of musculoskeletal structures as inflammatory markers. In case of discrepant results (positive FOI and negative CE and US), MRI was performed to prove the findings. In case of MRI negativity, a follow-up period of 24-months was performed including FOI, CE, US and MRI assessment.Results:83 of the 411 patients of the cohort were negative in all assessments (Pso only), 136 of the 411 patients were classified as PsA by rheumatologic assessments. 119 patients showed subclinical signs of musculoskeletal inflammation in the central reading of FOI, whereas CE and US were negative. In 37,5% of those patients, subclinical inflammation was confirmed by MRI assessment. 22 patients of the cohort without MRI positivity were willing to be followed up until month 24. 5 (7.5%) patients developed a clinical PsA until month 24 whereas 7 (10.5%) patients converted to be FOI negative. In 5 patients an additional MRI examination was performed in which one patient showed positive signs for inflammation.Conclusion:FOI is an innovative method for measurement of changes in microvascularisation in the hands. 6/22 patients initial only positive in FOI (no clinical signs for PsA, negative US, negative MRI) developed either clinical evident PsA (n=5) or new inflammation in MRI (n=1) during follow-up of 24 months.
