In the United States, colorectal cancer (CRC) is the third most common cancer diagnosed among men and women and the second leading cause of death from cancer. CRC largely can be prevented by the detection and removal of adenomatous polyps, and survival is significantly better when CRC is diagnosed while still localized. In 2006 to 2007, the American Cancer Society, the US Multi-Society Task Force on Colorectal Cancer, and the American College of Radiology came together to develop consensus guidelines for the detection of adenomatous polyps and CRC in asymptomatic average-risk adults. In this update of each organization's guidelines, screening tests are grouped into those that primarily detect cancer early and those that can detect cancer early and also can detect adenomatous polyps, thus providing a greater potential for prevention through polypectomy. When possible, clinicians should make patients aware of the full range of screening options, but at a minimum they should be prepared to offer patients a choice between a screening test that primarily is effective at early cancer detection and a screening test that is effective at both early cancer detection and cancer prevention through the detection and removal of polyps. It is the strong opinion of these 3 organizations that colon cancer prevention should be the primary goal of screening.

INTRODUCTIONLast year, the American Cancer Society (ACS) announced that it was inaugurating a yearly report on its cancer detection guidelines. 1 The annual report would be a single summary source on current ACS guidelines for the early detection of cancer, including background and rationale for guidelines that had been updated in the prior year, announcements of upcoming guideline reviews, and a summary of the most recent data on adult cancer screening rates.During 2000, the ACS reviewed and revised early detecion guidelines for prostate cancer, colorectal cancer, and endometrial cancer, and updated the narrative related ABSTRACT Updates to the American Cancer Society (ACS) guidelines regarding screening for the early detection of prostate, colorectal, and endometrial cancers, based on the recommendations of recent ACS workshops, are presented. Purpose
Demographic, behavioral and environmental factors have been associated with increased risk of colorectal cancer (CRC). We reviewed the published evidence and explored associations between risk factors and CRC incidence.
Methods
We identified 12 established non-screening CRC risk factors and performed a comprehensive review and meta-analyses to quantify each factor’s impact on CRC risk. We used random effects models of the logarithms of risks across studies: inverse variance weighted averages for dichotomous factors and generalized least squares for dose-response for multi-level factors.
Results
Significant risk factors include inflammatory bowel disease (RR = 2.93, 95% CI: 1.79–4.81); CRC history in first-degree relative (RR = 1.79, 95% CI: 1.60–2.02); body mass index (BMI) to overall population (RR = 1.10 per 8 kg/m2 increase, 95% CI: 1.08–1.12); physical activity (RR = 0.88, 95% CI: 0.86–0.91 for 2 standard deviations increased physical activity score); cigarette smoking (RR = 1.06, 95% CI: 1.03–1.08 for 5 pack-years), and consumption of red meat (RR = 1.13, 95% CI: 1.09–1.16 for 5 servings/week), fruit (RR = 0.85, 95% CI: 0.75–0.96 for 3 servings/day), and vegetables (RR = 0.86, 95% CI: 0.78–0.94 for 5 servings/day).
Conclusions
We developed a comprehensive risk modeling strategy that incorporates multiple effects to predict an individual’s risk of developing colorectal cancer. Inflammatory bowel disease and history of CRC in first-degree relatives are associated with much higher risk of CRC. Increased BMI, red meat intake, cigarette smoking, low physical activity, low vegetable consumption, and low fruit consumption were associated with moderately increased risk of CRC.
Adenomatous polyps are the most common neoplastic findings uncovered in people who undergo colorectal screening or have a diagnostic workup for symptoms. It was common practice in the 1970s for these patients to have annual follow-up surveillance examinations to detect additional new adenomas as well as missed synchronous adenomas. As a result of the National Polyp Study report in 1993, which demonstrated clearly in a randomized design that the first postpolypectomy examination could be deferred for 3 years, guidelines published by a gastrointestinal consortium in 1997 recommended that the first follow-up surveillance be 3 years after polypectomy for most patients. In 2003, these guidelines were updated, colonoscopy was recommended as the only follow-up examination, and stratification at baseline into lower and higher risk for subsequent adenomas was suggested. The 1997 and 2003 guidelines dealt with both screening and surveillance. However, it has become increasingly clear that postpolypectomy surveillance is now a large part of endoscopic practice, draining resources from screening and diagnosis. In addition, surveys have demonstrated that a large proportion of endoscopists are conducting surveillance examinations at shorter intervals than recommended in the guidelines. In the present paper, a careful analytic approach was designed addressing all evidence available in the literature to delineate predictors of advanced pathology, both cancer and advanced adenomas, so that patients can be more definitely stratified at their baseline colonoscopy into those at lower or increased risk for a subsequent advanced neoplasia. People at increased risk have either three or more adenomas, or highgrade dysplasia, or villous features, or an adenoma Ն1 cm in size. It is recommended that they have a 3-year follow-up colonoscopy. People at lower risk who have one or two small (Ͻ1 cm)
Chemoradiation prior to pancreaticoduodenectomy ensures that all patients who undergo resection complete multimodality therapy, avoids resection in patients with rapidly progressive disease, and allows radiation therapy to be delivered to well-oxygenated cells before surgical devascularization. Twenty-eight patients with cytologic or histologic proof of localized adenocarcinoma of the pancreatic head received preoperative chemoradiation (fluorouracil, 300 mg/m2 per day, and 50.4 Gy) with the intent of proceeding to resection; all 28 completed this preoperative therapy. Hospital admission because of gastrointestinal toxic effects was required in nine patients, yet no patient experienced a delay in operation. Restaging was performed 4 to 5 weeks after completion of chemoradiation, and five patients were found to have metastatic disease; the 23 patients without evidence of progressive disease underwent laparotomy. At laparotomy, three patients were found to have unsuspected metastatic disease, three patients had unresectable locally advanced disease, and 17 patients were able to undergo pancreaticoduodenectomy. One perioperative death resulted from myocardial infarction, and perioperative complications occurred in three patients. Histologic evidence of tumor cell injury was present in all resected specimens. Our results suggest that pancreaticoduodenectomy can be performed with a low incidence of complications after chemoradiation for localized adenocarcinoma of the pancreas.
