Samenvatting Doel Het systematisch in kaart brengen van lacunes in het corpus van huisartsgeneeskundige kennis waarop de NHG-standaarden zijn gebaseerd.Methode Wij onderzochten 70 NHG-standaarden op lacunes in de wetenschappelijke onderbouwing van de verschillende richtlijnen. Deze lacunes werden opgeslagen in een database in de vorm van onderzoeksvragen. Negentien standaarden werden beoordeeld door twee onderzoekers om de beoordelaarsbetrouwbaarheid te bepalen.Resultaten De geı¨nventariseerde lacunes leverden 875 onderzoeksvragen op. De overeenkomst tussen de twee onderzoekers was goed. Het maken van selecties op standaard, ICPC-code, trefwoord en/of aandachtsgebied was goed mogelijk. De database is te vinden op internet (www.artsennet.nl/nhg).Conclusie Onze aanpak heeft geresulteerd in een goed toegankelijke en bruikbare database, die kan dienen als bron voor het stellen van onderzoeksprioriteiten en het formuleren van onderzoeksvragen.In de jaren 1989-2000 publiceerde het Nederlands Huisartsen Genootschap in totaal 70 NHG-standaarden met richtlijnen voor het medisch handelen bij veel voorkomende aandoeningen in de huisartspraktijk.1 Slechts een deel van deze richtlijnen kan worden onderbouwd met onderzoek dat de toets der kritiek kan doorstaan, terwijl veel onderzoek is verricht in de tweede lijn, zodat de resultaten moeten worden gee¨xtrapoleerd naar de huisartspraktijk. Daarnaast berust een belangrijk deel van de richtlijnen niet op wetenschappelijk onderzoek, maar op consensus binnen de werkgroep.Uit onderzoek naar de implementatie van standaarden is bekend dat wetenschappelijk goed onderbouwde richtlijnen beter worden toegepast in de praktijk.2 Wij maakten daarom een inventarisatie van alle richtlijnen die niet zijn onderbouwd door wetenschappelijk onderzoek en formuleerden de opgespoorde lacunes in de vorm van concrete onderzoeksvragen. Het in kaart brengen van lacunes heeft tot doel beleidsmakers en onderzoekers te stimuleren deze gaten in onze kennis te vullen. Dit past in het streven van het NHG, het stimuleren van onderzoek dat relevant is voor de huisartsgeneeskunde.3 Methode Wij baseerden ons bij het inventariseren van lacunes op de tekst en de noten bij de standaarden. Wij spraken van een lacune in de volgende gevallen:1. In de standaard staat dat er te weinig (toepasbare) gegevens zijn voor het formuleren van een specifieke richtlijn. 2. In de standaard staat dat de richtlijn summier is onderbouwd en/of vooral is gebaseerd op consensus binnen de werkgroep. 3. In de standaard staat dat de richtlijn is gebaseerd op onderzoek in de tweede lijn, en dat mag worden aangenomen dat de resultaten niet zonder meer gegeneraliseerd kunnen worden naar de huisartspraktijk.
Objectives: To study the long-term consistency in answers to questions on chronic respiratory symptoms of a standardised questionnaire and in the diagnosis of asthma established on the basis of the answers to these questions* M ethods: A comparison of data collected by means of a test and a retest after a one-year interval. Population: 246 subjects aged 16 to 23. Main outcome measure: Cohen's kappa as measure of agreement between the answers to the first and the second ques tionnaire. Results: Cohen's kappa values of the questions on symptoms ranged between 0.22 and 0.62 and was 0.50 for the asthma diagnosis. O f the 20% of the study group who had asthma on the basis of the first ques tionnaire, 32% did not report of one single asthma symptom in the second one. Conclusions: In young adults aged 16 to 23, the long term consistency in answers to questions on respirat ory symptoms in a self-administered questionnaire and in the diagnosis of asthma assessed on the basis of the answers to these questions is unsatisfactory.
