Background The Parkinson's Disease‐Cognitive Rating Scale (PD‐CRS) assesses posterior‐cortical and frontal‐subcortical cognitive functioning and distinguishes mild cognitive impairment in Parkinson's disease (PD‐MCI); however, it was not evaluated in Brazil. Objectives To investigate PD‐CRS's reliability, validity, normative data, and accuracy for PD‐MCI screening in Brazil. Methods The effects of age, education, and sex on PD‐CRS scores were explored. The instrument was tested in 714 individuals (53% female, 21–94 years), with a broad range of education and no neurodegenerative disorder. Trail Making, Consonant Trigrams, Five‐Point, and semantic fluency tests were administered for comparison. A second study enrolled patients with PD and intact cognition (n = 44, 59.75 ± 10.79 years) and with PD‐MCI (n = 25, 65.76 ± 10.33 years) to investigate criterion validity. PD‐CRS subtests were compared with the Cambridge Automated Neuropsychological Battery memory and executive tasks. Results PD‐CRS was unidimensional and reliable (McDonald's ω = 0.83). Using robust multiple regressions, age, and education predicted the total and derived scores in the normative sample. At the 85‐point cutoff, PD‐MCI was detected with 68% sensitivity and 86% specificity (area under the curve = 0.870). PD‐CRS scores strongly correlated with executive and verbal/visual memory tests in both normative and clinical samples. Conclusions This study investigated the applicability of PD‐CRS in the Brazilian context. The scale seems helpful in screening for PD‐MCI, with adequate internal consistency and construct validity. The PD‐CRS variance is influenced by age and educational level, a critical issue for cognitive testing in countries with educational and cultural heterogeneity.
INTRODUÇÃO: A doença de Parkinson atinge cerca de 3% da população acima de 60 anos noBrasil. Além dos sintomas motores, existem diversos sintomas não motores, incluindoaspectos cardiovasculares, gastrintestinais, relacionados ao sono, psiquiátricos, entre outros.No que concerne aos sintomas psiquiátricos, inclui-se não só ansiedade e depressão, mastambém os transtornos de controle de impulso (TCIs). A prevalência de transtornos docontrole do impulso (TCIs) em portadores da doença de Parkinson é de cerca de 15 a 20%.Esse sintoma pode se apresentar como hipersexualidade, hiperfagia, compulsão por jogosou por compras. A abordagem deve visar a identificação precoce e o ajuste do tratamentoda doença de Parkinson, a fim de prevenir consequências sociais, financeiras e legais. Aescala QUIP-RS é uma ferramenta pra avaliação dos TCIs, que aborda a gravidade eprogressão dos sintomas, mas ainda não foi traduzida ou validada para o Portuguêsbrasileiro. OBJETIVO: Traduzir e validar a QUIP-RS para o português brasileiro.METODOLOGIA: A escala QUIP-RS e sua folha de instruções foram traduzidas,retrotraduzidas e aprovadas pelo autor da versão original. A escala foi aplicada a 13controles e 39 paciente portadores de doença de Parkinson. Foram analisadas aspropriedades psicométricas aceitabilidade, consistência externa e interna, dimensionalidadee precisão. Foram aplicadas, também, as escalas MDS-UPDRS, NMSS, QUIP-CS, PDSS-BR,PDQ-8 e MoCA para melhor análise do perfil do paciente e para análise da validadedivergente. Os dados foram analisados no software R. RESULTADOS E DISCUSSÃO: Avaliou-seos resultados referentes aos paciente com doença de PraHouve efeito solo importante, oefeito teto foi menor do que 15%, o alfa de Cronbach foi de 0.78, demonstrando aconfiabilidade da escala. A análise gráfica exploratória e a fatorial confirmatóriacomprovaram que é uma escala unidimensional. A validade convergente foi demonstradapela relação estatisticamente significativa entre as pontuações de cada transtorno na QUIPRSe as questões binárias referentes a cada transtorno no QUIP-CS. A validade divergente foicomprovada pela falta de relação ou relação negativa com escalas que avaliam outrosconstrutos. CONCLUSO: A escala QUIP-RS, conforme foi traduzida parece ser uminstrumento útil na identificação de TCIs, mas ainda são necessários estudos com análiseteste-reteste para sua validação
RESUMOIntrodução: O Hematoma Subdural Crônico (HSDC) é uma condição neurocirúrgica comum, em que os pacientes sintomáticos são, em geral, tratados com trepanação e drenagem. Estudos, porém, sugerem que o uso de TXA pode prevenir quadros sintomáticos e recorrências pós-cirúrgicas. Neste trabalho, tais evidências serão revisadas. Metodologia: Foi realizada uma revisão bibliográfica qualitativa que incluiu artigos publicados entre os anos de 2006 e 2020. As buscas foram realizadas nas bases de dados bibliográficas PubMed, Scielo e Google Scholar, a partir de artigos científicos escritos em inglês e português. Discussão: O 1º estudo sobre o tema avaliou 18 pacientes com HSDC tratados apenas com TXA. Observou-se redução do volume dos hematomas, média de 55,6 ml para 3,7 ml, e ausência de recorrência ou progressão destes. Porém, um questionário sobre terapêuticas utilizadas por neurocirurgiões no HSDC mostrou que, em relação ao tratamento conservador, 21% adotam essa conduta regularmente. Dentre esses, o uso de TXA é adotado por 45%, mas só é utilizado raramente. Um relato de caso demonstrou a eficácia do TXA para HSDC em pacientes em anticoagulação. Após recorrências do HSDC, na 5ª cirurgia, utilizaram-se doses de TXA, com resolução dos sintomas e, após 9 meses, ausência de hematoma residual. Estudo comparou pacientes com e sem uso de TXA após o tratamento cirúrgico: os volumes residuais encontrados foram menores no grupo que recebeu a medicação, e não foram evidenciados efeitos colaterais do TXA. Uma revisão sistemática foi feita com 26 estudos randomizados controlados em andamento, com 5 estudos analisando o uso de TXA, avaliando sua eficácia tanto como monoterapia quanto adjunto ao cirúrgico. Um protocolo de tratamento conservador (TRACS) tem sido proposto e encontra-se em fase IIB e propõe realizar 20 mg/kg/48h EV em 1 mês e em seguida 10 mg/kg/48h VO, com suspensão do TXA após resolução completa na TC de crânio. Conclusão: É evidente que os últimos estudos realizados demonstram certo benefício no uso de TXA em pacientes com HSDC, no entanto, não está claro ainda se maior benefício seria atingido naqueles pacientes em tratamento apenas conservador ou cirúrgico associado, ou ainda quais critérios seriam utilizados para indicar o uso do medicamento.

Foram aplicadas, também, as escalas MDSUPDRS,QUIP-CS, PDSS-BR, PDQ-8 e MoCA para melhor análise do perfil dos pacientes e paraanálise da validade divergente. RESULTADOS E DISCUSSÃO: Avaliaram-se os resultadosreferentes aos pacientes com doença de Parkinson. O alfa de Cronbach foi de 0.79,demonstrando a confiabilidade da escala. A análise gráfica exploratória e a fatorialconfirmatória comprovaram que é uma escala unidimensional. A validade convergente foidemonstrada pela relação estatisticamente significativa entre as pontuações de cadatranstorno na QUIP-RS e as questões binárias referentes a cada transtorno no QUIP-CS. Avalidade divergente foi comprovada pela falta de relação ou relação negativa com escalas queavaliam outros construtos. CONCLUSÃO: A escala QUIP-RS, conforme foi traduzida é uminstrumento útil na identificação e avaliação da gravidade e da progressão de TCIs no Brasil.
