Multidrug-resistant tuberculosis (MDR-TB) is a severe and feared problem, that is difficult to control and has shown a tendency to increase worldwide.
Low antimycobacterial drug concentrations have been observed in tuberculosis (TB) patients under treatment. The lactulose/mannitol urinary excretion test (L/M), normally used to measure intestinal permeability, may be useful to assess drug absorption. The objective of this research was to study intestinal absorptive function and bioavailability of rifampin and isoniazid in TB patients. A cross sectional study was done with 41 patients and 28 healthy controls, using the L/M test. The bioavailabilities of rifampin (R) and isoniazid (H) were evaluated in 18 patients receiving full doses. Urinary excretion of mannitol and lactulose, measured by HPLC, was significantly lower in TB patients. The serum concentrations of the drugs were below the expected range for R (8-24 mcg/mL) or H (3-6 mcg/mL) in 16/18 patients. Analyzing the drugs individually, 12/18 patients had low serum concentrations of R, 13/18 for H and 8/18 for both drugs. We suggest that there is a decrease in the functional absorptive area of the intestine in TB patients, which would explain the reduced serum concentrations of antituberculosis drugs. There is a need for new approaches to improve drug bioavailability in TB patients.
Descritores -Tuberculose resistente a múltiplas drogas. Fatores de risco.Introdução: A tuberculose multirresistente é uma ameaça ao controle da tuberculose em todo o mundo. Tratamento inadequado é freqüentemente apontado como fator de risco para tuberculose multirresistente.Objetivo: Identificar fatores associados ao tratamento inadequado em portadores de tuberculose multirresistente. Método:Foram levantados e identificados os testes de sensibilidade para tuberculose, realizados no Laboratório Central do Estado do Ceará de 1990 a 1999. Foi realizado um estudo retrospectivo e prospectivo, em grupo de portadores de tuberculose multirresistente, comparando as características dos pacientes com tratamento adequado com as dos com tratamentos inadequados. Foi considerado multirresistente o bacilo resistente a pelo menos rifampicina + isoniazida e utilizado o método das proporções.Resultados: Dos 1.500 testes de sensibilidade realizados, 266 eram multirresistentes. Destes, identificaram-se apenas 153 pacientes, dos quais 19 foram excluídos, restando, no estudo, 134 pacientes. Pela análise univariada os fatores associados significativamente ao tratamento inadequado foram: não-adesão ao tratamento, pobreza extrema, intolerância medicamentosa, falha no atendimento, falta de medicação, dois ou mais tratamentos anteriores, lesões radiológicas bilaterais e grandes cavidades pulmonares. Foi encontrada ainda associação (p < 0,001) de alcoolismo e/ou tabagismo com a não-adesão. Na análise multivariada, permaneceram associados ao tratamento inadequado: dois ou mais tratamentos anteriores (p < 0,0001, OR = 5,9; IC 95%: 2,5-13,7), grandes cavidades pulmonares (p < 0,0217, OR = 2,7; IC 95%: 1,2-6,1) e lesões radiológicas bilaterais (p < 0,0226, OR = 3,2; IC 95%: 1,4-7,4). Conclusão:Neste estudo, observou-se que os fatores associados aos tratamentos inadequados são de grande abrangência. Deve existir uma tentativa para controlar melhor a doença, principalmente nos pacientes em retratamento e nos portadores de grandes cavidades e/ou lesões bilaterais na radiografia torácica. Tuberculose multirresistente (TBMR) é uma preocupação mundial, mas sua magnitude era desconhecida no Estado do Ceará, região Nordeste do Brasil. Objetivos: Determinar a prevalência da TBMR em nosso meio, bem como a sensibilidade do M. tuberculosis às drogas antituberculose. Método: Estudo retrospectivo foi conduzido utilizando a lista de 1.500 testes de sensibilidade (TS) para rifampicina (RFP), isoniazida (INH), estreptomicina (SM), pirazinamida (PZA), etambutol (EMB) e etionamida (ETH) realizados no Laboratório Central de Saúde Pública do Estado do Ceará (Lacen), no período de 1990-1999. Foi utilizado o método das proporções. Resultados: Resistência primária ou adquirida a pelo menos uma droga foi observada em 404 de 1.500 (27%) cepas; 266 (17,7%) casos foram classificados como TBMR devido à resistência a pelo menos RFP + INH. O total de casos de tuberculose (TB) notificados no Estado no período de estudo foi de 41.073, a prevalência da TBMR foi, então, de 0,65%. Dos examinados, 62,3% eram do sexo masculino. A média etária foi de 40,17 ± 14,62. A resistência a uma droga isolada foi de 8,1%, para INH = 5,7%, SM = 1,5% e RFP = 0,9%. A resistência a duas, três, quatro, cinco e seis drogas foi, respectivamente, de 10,8; 3,1; 0,9; 1,5 e 2,9%. A sorologia anti-HIV foi realizada em todos os internados e alguns ambulatoriais, totalizando 86 pacientes. Foi negativa em todos. A probabilidade de sobrevivência em cinco anos (138 pacientes analisados) foi significantemente maior no grupo com TBMR tratado com medicamentos de terceira linha, mesmo para o grupo de falência a essas drogas, comparado com o grupo não tratado com esses fármacos. Conclusão: A prevalência da TBMR nesse estudo foi intermediária, quando comparada com o Brasil e outras partes do mundo. Medidas preventivas, diagnóstico precoce e incentivo à pesquisa de novas drogas efetivas e seguras devem ser estimulados no combate à TBMR.
This study evaluates the serum concentrations of rifampin (RMP), isoniazid (INH), and intestinal barrier function in patients with multidrug-resistant tuberculosis (MDR-TB), drug susceptible tuberculosis (DS-TB), and health volunteers (HC; controls). Peak serum concentrations of RMP were significantly lower in MDR-TB and DS-TB as compared with HC (odds ratio [OR] = 3.125, confidence interval [CI] [1.037-9.418] and OR = 4.025, CI [1.207-13.418], respectively). The INH peak serum concentration was not significantly different between MDR-TB versus DS-TB or DS-TB versus HC. The percent of mannitol excretion was significantly lower in the MDR-TB group compared with DS-TB (13.18 versus 16.03, analysis of covariance [ANCOVA], P = 0.0369) and compared with HC (13.18 versus 16.61, ANCOVA, P = 0.0291) the other study groups. These data suggested a lower peak serum concentration of RMP for both MDR-TB and DS-TB as compared with the HC group. The data also showed a lower intestinal area of absorption in patients with tuberculosis and even worse in MDR-TB.
It was observed that the spoligopatterns family distribution was similar to that reported for South America, prevailing the LAM and H lineages. A high rate-case among the resistant TB group occurs as a result of transmitted and acquired resistance. A more effective surveillance program is needed in order to succeed in reducing tuberculosis in Northeast Brazil.
Background:Since the first years of antituberculosis chemotherapy, there has been controversy regarding the transmissibility, infectiousness, virulence and pathogenicity of susceptible and drug-resistant strains of Mycobacterium tuberculosis. Objective:To determine the incidence of active tuberculosis (TB) among individuals cohabiting with patients infected with susceptible and multidrug-resistant tuberculosis (MDR-TB). Methods:A case-control study was conducted. Cases of MDR-TB were defined as those infected with M. tuberculosis strains resistant to at least rifampin and isoniazid. Susceptible TB cases (controls) were defined as those first treated at approximately the same time as the first treatment of the MDR-TB cases -and cured by the time of the interview. Study cases were selected on the basis of the results of susceptibility tests, using the proportion method, carried out at the Central Laboratory of Public Health of the State of Ceará. The control group consisted of patients enrolled in the Tuberculosis Control Program between 1990 and 1999. Results:We evaluated 126 patients and 176 controls. The number of individuals sharing the household with patients was 557 in the MDR-TB group and 752 in the controls. The average number of exposed individuals per index case was 4.42 and 4.27 among patients and controls, respectively. Of the 557 MDR-TB-exposed individuals, 4.49% (25) received antituberculosis treatment after the respective index case had begun treatment, compared to 5.45% (41/752) among the controls (p = 0.4468). Microepidemics of MDR-TB were confirmed in eight families. Conclusion:Our results suggest that the incidence of active TB is comparable between households inhabited by MDR-TB patients and those inhabited by susceptible-TB patients.
