Prognostic scores besides age and fungal infections are risk factors for mortality and help to differentiate between secondary and tertiary peritonitis. The combination of prognostic scores is comparable to the APACHE II and superior compared to the MPI in regard to detection of tertiary peritonitis.
In patients with gastroesophageal reflux disease the esophagogastric junction cannot be identified by endoscopy but requires histopathology of multilevel biopsies. The squamocolumnar junction harbors the highest yield of intestinal metaplasia.
Zusammenfassung. Grundlagen: Barrett-Ö sophagus (BÖ ) entsteht durch gastroösophagealen Reflux und ist die Vorstufe zum Adenokarzinom der Speiseröhre. Derzeit wird die Diagnose entsprechend einem Mix aus Endoskopie und Histopathologie gestellt. Dieses Vorgehen wird durch eine kürzlich publizierte neue histopathologische Klassifikation in Frage gestellt.Methodik: Literaturübersicht zur Häufigkeit von Barrett-Ö sophagus und kritische Analyse der neuen histopathologischen Klassifikation.Ergebnisse: Wir inkludierten 40 Studien und 95.386 Patienten (mittleres Alter 53 Jahre), bei denen Endoskopie und Biopsien des ösophagogastralen Ü bergangs durchgeführt worden waren und fanden, dass 1,6%-2,0% der Kontrollen und 0,8% bis 36% jener mit Dyspepsie und Refluxsymptomen einen Barrett-Ö sophagus hatten. BÖ kam bei jenen mit und ohne endoskopisch sichtbarem Zylinderepithelösophagus vergleichbar häufig vor. BÖ scheint häufiger Männer zu befallen und nimmt mit der Länge des sichtbaren Zylinderepithelösophagus und dem Alter zu. Entsprechend der neuen histopathologischen Paull-Chandrasoma-Klassifikation des Zylinderepithelösophagus zeigt sich, dass der endoskopische proximale Magen (Kardia) in Wirklichkeit einem von Zylinderepithel ausgekleidetem dilatiertem Ö sophagus (dilated end stage esophagus) entspricht, in dem auch intestinale Metaplasie, Dysplasie und Karzinom entstehen können. Eine kritische Analyse einer publizierten Studie zeigte, dass die Häufigkeit von BÖ in der Bevöl-kerung durch Fehlklassifikation deutlich unterschätzt wird.Schlussfolgerungen: Barrett-Ö sophagus kommt häu-figer vor als bis dato angenommen und kann auch jene betreffen, die keine Symptome der Refluxkrankheit haben. Currently diagnosis is established upon a mixture of endoscopic and histopathologic criteria. Recently a novel histopathology classification questions current policies regarding diagnosis of Barrett's esophagus.Methods: Review on published frequency of Barrett's esophagus and critical analysis of the novel histopathology classification of columnar lined esophagus.Results: Our review, including 40 studies and 95,386 persons (mean age 53 years) undergoing endoscopy and biopsy sampling from the esophagogastric junction revealed Barrett's esophagus (BE) in 1.6-2.0% of controls and 0.8-36% of individuals with dyspepsia and GERD symptoms and was similar for those with and without endoscopic visible columnar lined esophagus. As cholesterol concentrations increased and LMWC D4 decreased similarly in both SiO brands, their stability might be regarded as equal. The smallest LMWC molecules (D4) appear to have the greatest capability to diffuse into the surrounding ocular tissues. The low grade of emulsification (12.9 %) in our patients could not be related to LMWC or cholesterol changes.
