Perinatal mood and anxiety disorders are among the most common mental
health conditions encountered by women of reproductive age. When left untreated,
perinatal mood and anxiety disorders can have profound adverse effects on women
and their children, ranging from increased risk of poor adherence to medical
care, exacerbation of medical conditions, loss of interpersonal and financial
resources, smoking and substance use, suicide, and infanticide. Perinatal mood
and anxiety disorders are associated with increased risks of maternal and infant
mortality and morbidity and are recognized as a significant patient safety
issue. In 2015, the Council on Patient Safety in Women’s Health Care
convened an interdisciplinary workgroup to develop an evidence-based patient
safety bundle to address maternal mental health. The focus of this bundle is
perinatal mood and anxiety disorders. The bundle is modeled after other bundles
released by the Council on Patient Safety in Women’s Health Care and
provides broad direction for incorporating perinatal mood and anxiety disorder
screening, intervention, referral, and follow-up into maternity care practice
across health care settings. This commentary provides information to assist with
bundle implementation.
WBG, a replicable model integrated into primary care visits, affords a unique opportunity to intervene consistently and early, providing families in at-risk communities with increased provider time, intensive education, and ongoing support. Further study of group well-child care for primary obesity prevention is warranted to confirm the effectiveness of the model.

Further research is needed to clarify the challenges that obese women face in accessing care and to evaluate strategies such as ensuring the availability of appropriate equipment and supplies, the use of alternative screening methodologies, and more culturally sensitive counseling approaches that may improve screening rates in obese women.

Strategies identified in this study such as providing focused education directed at nonpregnant nulliparous women, stressing portion control, helping women better manage their cravings, and providing more pragmatic support and resources need to be explored in future research.
Lead poisoning remains a significant problem in the United States affecting the health of women and children. Although the damage is greatest at higher levels of accumulation, no level of lead has been found to be safe. Over the last 25 years, the blood level of lead thought to be associated with toxicity has dropped dramatically, from to 60 mcg/dL in 1960 to 10 mcg/dL today. Studies confirm that exposure to lead causes kidney damage, encephalopathy, and impaired cognitive function in children and in adults. Recent evidence indicates children with levels less than 10 mcg/dL may suffer from compromised development and intellectual performance later in life. This article discusses the case of a woman found to have lead poisoning during pregnancy. Environmental sources of lead, implications for the immediate and future health of the fetus and mother, and techniques clinicians can use in their practice to minimize the adverse effect of lead on their clients are reviewed.
While air pollution levels have fallen in recent years, air quality in the United States is still poor and adversely affects the health of millions of persons. Because of physiologic changes in pregnancy, pregnant women and their fetuses are among the most vulnerable. This paper reviews the current state of our air quality, the impact that exposure to air pollution has on general health and the health of a pregnancy, and offers suggestions on how to minimize exposures.
