BackgroundIdentifying perceived access barriers to preventive dental services is one of the basic steps to improve the public health.ObjectivesThis study aimed to determine the perceived barriers affecting access to preventive dental services in one of Tehran dental clinics in 2012.Patients and MethodsThis research was a cross-sectional descriptive-analytical study conducted in one of Tehran dental clinics in 2012 using decision–making trial and evaluation laboratory (DEMATEL) method. The study sample included all patients (100 patients) who had referred to the endodontic treatment department from 26 - 31 May, 2012. The required data were collected using a questionnaire. Collected data were analyzed using SPSS 18.0 and MATLAB 7.9.0 SPSSS 18.0, as well as, some descriptive and analytical tests including Mean, Standard Deviation (SD), and Independent T- Test.ResultsThe five determinants of cost, inconvenience, fear, organization, and patient-dentist relationship were determined as barriers to access to dental services among which the cost and patient-dentist relationship were identified as the first and last priorities with the coordinates (1.4 and 1.4) and (1.25 and -0.65), respectively.ConclusionsHigh cost of dental care has led to not referring patients to the clinic. Oral health costs are too high; however insurance organizations have no commitment to support such services. Policymakers, administrators, and insurance organizations have a major role in improving access to dental services. These decision-makers in making their policies can provide the required financial resources, shift the available resources towards preventive care and periodic checkups, and consider providing proper and sufficient places for dental care facilities.
BackgroundKidney transplantation from living donors has been increased recently. Preoperative evaluation of living donor is important to select the appropriate kidney for transplantation and to decrease donor surgical complications.ObjectivesThe aim of this study was to compare the accuracy of the use of multidetector computed tomography (MDCT) to evaluate vascular anatomy in living kidney donors with traditional angiography.Patients and MethodsA total number of 60 living kidney donors who underwent open surgical approach for transplantation were selected: Kidney anatomy of donors evaluated by CT angiography (group 1) or traditional angiographic examination (group 2). Renal vessels anatomy was compared with surgical findings in both groups.ResultsThe accuracy for detecting number of main renal arteries were not different in both groups which were 96.7% in CT angiography group and 90% in traditional angiography group (P = 0.15). The accuracy for detection of main renal veins were 100% in group 1 and 96.7% in group 2 (P = 0.31).ConclusionsMDCT has the same accuracy as traditional angiography to detect renal abnormalities in living kidney donors.
Background: Job burnout, defined as a syndrome derived from prolonged exposure to stressors at work, is often observed in health care workers. Shift work and job satisfaction are considered two of the occupational risks for burnout in nurses. Nurses have stress and health complaints. In addition, nurses are likely to job burnout. Objectives: The current study aimed to determine the prevalence of job burnout and its association with work schedules and job satisfaction among Iranian nurses in a public hospital. Methods: This cross-sectional study was conducted in one of the largest Iranian public hospitals among 362 nurses (response rate: 80.44%) in Tehran, Iran. The Maslach burnout inventory (MBI-22) and demographic factors questionnaire were used in the present study. The relationship between job burnout with work schedules and job satisfaction was investigated with multiple logistic regression analysis. Results: The mean age and work experience of the participants were 36.14 ± 8.59 and 15.23 ±9.30 years, respectively. The result indicated a relatively high prevalence of burnout (particularly, personal accomplishment) among the study population. In general, 64.4% of participants reported low personal accomplishment level. The nurses engaged in shift work reported higher levels of emotional exhaustion (odds ratio (OR) = 1.02, 95% confidence interval (CI) = 1.006-1.041, P-value = 0.008); there was no relationship between work schedules with depersonalization and personal accomplishment. The result showed significant relationship between job satisfaction and emotional exhaustion (OR = 0.945, 95% CI = 0.928-0.963, P-value < 0.001) and personal accomplishment (OR = 1.003, 95% CI = 1.014-1.058, P-value = 0.001). Conclusions: The current study revealed that the Iranian nurses are exposed to a considerable risk of personal accomplishment. Also, job burnout is in association with shift working and low job satisfaction level. In this regard, working pressure, type of job and income may affect job burnout. Ergonomic interventional programs are recommended to improve the working conditions.
Background: Nutrition can be regarded as an important part of any preparation program, as well as an important part of life. It seems essential to develop nutritional science and improve eating habits with the purpose of developing a healthy diet and avoiding the outcomes that arise from a lack of nutrition. Proper nutrition is one of the important factors for the development of health. Lack of sufficient awareness about nutrition can result in improper eating habits. Objectives: The present study evaluated the effect of knowledge about healthy nutrition and nutrition science on the knowledge development approach. Materials and Methods:In this cross-sectional descriptive study, the statistical population consisted of 190 experts and other scholars in the area of nutritional science. A questionnaire based on demographic details, nutritional science, nutrition education, research, proper solutions to individuals' nutritional problems, and a culture of nutrition was used for data collection. Results: A relatively strong positive correlation was found between the knowledge development approach, nutritional science, nutrition education and research, and proper solutions for individual nutritional problems and a culture of nutrition (P < 0.001). Conclusions: 1) Effective enhancement and participation in an academic community will be important in the future of food and nutrition security; however, major gaps and weaknesses also exist in this context; 2) The main weaknesses in relation to the lack of clear policies and procedures include focusing on only Tehran, Iran; the need to overcome bureaucracy; and problems related to motivation, capital, and international communications; 3) Qualitative and quantitative improvement of research is not possible without access to skilled experts and researchers; 4) To solve these problems, it will be beneficial to pay more attention to the role of universities, facilitate intellectual communication among professors in Tehran and other provinces, ensure that Iranian professors are employed abroad, develop educational and research cooperation with foreign countries, and ensure that facilities for international cooperation put in place by united nations (UN) agencies are used.
