ZusammenfassungAnhand einer Literaturübersicht wird der derzeitige Kenntnisstand über die Diagnostik, Therapie und Prävention der Peritonitis beim Rind unter besonderer Berücksichtigung ihres Auftretens nach geburtshilflichen Maßnahmen dargestellt. Die klinische Diagnostik einer Peritonitis beim Rind bereitet im Endstadium der Erkrankung in der Regel keine Schwierigkeiten. Dagegen ist die Früherkennung auch unter Berücksichtung labordiagnostischer Parameter nicht möglich. Dies gilt insbesondere im peripartalen Abschnitt des Reproduktionszyklus, da synergistische Effekte von Geburtsverlauf, Puerperium und Wundheilung nach operativen geburtshilflichen Maßnahmen Einfluss auf klinische und labordiagnostische Parameter nehmen. Der Einsatz der Sonographie scheint als bildgebendes Verfahren dazu geeignet, diese diagnostische Lücke zu schließen. Im Gegensatz zur Endoskopie, die ebenfalls in dieser Indikation eingesetzt werden kann, stellt die transabdominale Sonographie ein nicht invasives und mit weniger Aufwand verbundenes Verfahren dar. Die Fragestellung einer effektiven Prophylaxe und Therapie im Anschluss an Operationen ist ebenfalls nicht gelöst, weil bisher nicht feststeht, welche Menge an Flüssigkeit für diese Indikation im Abdomen des Rindes notwendig ist. Die Therapiekonzepte älterer Studien lassen sich mit den heutigen pharmakologischen Vorstellungen und den geltenden rechtlichen Bestimmungen teilweise nicht mehr in Einklang bringen.
ZusammenfassungAnhand einer Literaturübersicht wird der derzeitige Kenntnisstand über die Peritonitis beim Rind unter besonderer Berücksichtigung ihres Auftretens nach geburtshilflichen Maßnahmen dargestellt. Der vorliegende erste Teil dieser Arbeit beschreibt die physiologischen Grundlagen, die Pathogenese und die Häufigkeit dieser Erkrankung. Das Peritoneum stellt einen hochaktiven Zellverband dar, der die intraperitoneale Flüssigkeitsbalance reguliert und einen mechanischen und biologischen Schutz für die intraadominellen Strukturen gewährleistet. Da Rinder eine im Vergleich zu anderen Tierarten geringere peritoneale fibrinolytische Aktivität besitzen, reagieren sie auf eine Traumatisierung des Peritoneums besonders intensiv mit der Ausbildung fibrinöser Auflagerungen und Adhäsionen. Diese helfen einerseits, pathologische Prozesse zu lokalisieren, andererseits führen sie jedoch zu einer hohen Anzahl postoperativer Verwachsungen. Neben der Entwicklung einer Peritonitis im Zusammenhang mit der Reticuloperitonitis traumatica und gastroduodenalen Ulzera treten Bauchfellentzündungen gehäuft nach Maßnahmen der operativen Geburtshilfe auf. Obwohl postoperative Inflammationen des Peritoneums beim Rind in der Regel lokalisiert bleiben und dann als permanente oder zeitlich begrenzte Adhäsionen in Erscheinung treten, sind die lokale, abszedierende Bauchfellentzündung und die diffuse Peritonitis der häufigste Grund für eine Euthanasie nach einer Sectio caesarea. Dabei liegt die Anzahl von Tieren mit intraabdominellen Adhäsionen nach einer Sectio caesarea in der linken Flanke am stehenden Tier zwischen 5 und 56%. Die Entwicklung einer Peritonitis diffusa wird mit einer Häufigkeit von 2,2-9,0% angegeben. Eine höhere Inzidenz zeigt sich bei verschleppten Geburten und Operationen am liegenden Tier. Dagegen weisen nur 1,2% der Kühe nach einer Fetotomie eine Bauchfellentzündung auf. Den größten Einfluss auf die Entwicklung einer Peritonitis haben die Operationsmethode und die Dauer des Partus vor dem Eingriff.
Retrospective HLA-DR-typing and the influence of HLA-DR antigen on transplantation prognosis was studied in 90 kidney donor-recipient pairs. It was clearly demonstrated that HLA-DR compatible donor kidney provides a significantly better transplant prognosis than if there is HLA-DR incompatibility. Donor kidneys with only one identical HLA-DR antigen gave a six-month survival rate of 80%. Only HLA-AB identical cadaver kidneys ("full house identity") give similar survival times. Because of relatively lower polymorphism of the HLA-DR alloantigen system, HLA-DR identical donor organs are discovered more frequently than when HLA-AB antigens are taken into consideration. HLA-DR identical donor kidneys (identical for both HLA-DR antigens) have an even better transplant prognosis than "full house identical" kidneys, since the survival rate in the former is 87% after six months.
