Endometrial cancer (EC) is the most common gynecological malignancy in women over 50 years of age. Age is a specific, significant predictor of the survival outcome in EC. Thrombotic complications (TC) are the other frequencies due to death in cancer patients. The problem of thrombosis is relevant and occupies a large proportion of the immediate causes of death in patients with oncological practice. Identification of risk groups, the implementation of a personalized approach to the prevention of TB is very practical in patients with EC.The aim of the study – to conduct an assessment of the Charleson Comorbidity Index and identify the link between an increased risk of thrombotic formation in patients with EС and associated pathology at the preoperative stage.Materials and Methods. The study used general clinical methods of examination: laboratory, specific for determining the state of hemostasis (D-dimer, antithrombin III, protein C, fibrinogen B, ACTH , IF, PM ), ultrasound examination (abdominal organs, small pelvis, lower vessels limbs and pelvic plexus; elastography of the vessels of the lower extremities and pelvic plexus). Histologic methods of tumor investigation – to determine the relationship between histological type, depth of invasion, degree of malignancy of the tumor. Mathematical, statistical – for analysis and generalization of data in the package "Statgraph" (v.3.0).Results and Discussion. On the basis of the obtained data and analysis of the risk factors of the occurrence of TC on the scale of Caprini, we came to the conclusion that the whole surveyed group of patients in the EC referred to the high risk of TC since they had 6 or more points. This is due to age, overweight, increased body surface area and body mass index, cancer pathology, concomitant pathology, which coincides with the data of world literature. Based on the integrated assessment of the Charleson Comorbidity Index and the assessment of the risk of developing TC on the Caprini scale, we noted that the most common risk factors for thrombosis are: abnormal fat metabolism, hypertonic disease, atherosclerosis, vascular disease. The combination of two or more risk factors for thrombosis was noted at 46.25 %. Relevant statistical discrepancies (p <0.05) in the hemostasis system in patients, depending on anthropometric indices, were confirmed by correlation bonds. We established a statistically significant (0.001 <p <0.05) positive relationship (r2> 0.17) between pelvic vein thrombosis and weight, age, BM I, Caprini scale, PPT , proper weight and PPT , PM , ACTCH Fibrinogen, D-dimer and a reliable negative (r2 <-0.17) relationship between thrombosis of the veins of the small pelvis and: the relation between the level of the MS and the level of D-dimer; PM / D-dimer; AT III / D-dimer.Conclusions. Based on the correlation data, it can be argued that the calculation of the Charleson Comorbidity Index is an important element in predicting survival rates in patients, but it does not provide an opportunity to predict the risk of thrombotic complications in patients with PE . The parallel compilation of the Charleson Comorbidity Index and the determination of the risk of developing thrombotic complications in endometrial cancer patients at the preoperative stage allows the isolation of high and high-risk groups of TU , especially in women over the age of 50 with existing comorbidity.
Мета: підвищити ефективність хірургічного лікування хворих на рак ендометрія шляхом оптимізації діагностичної тактики та хірургічної профілактики тромботичних ускладнень у ранній післяопераційний період. Матеріали і методи. Обстежено 3 групи жінок: І – група 30 соматично здорових жінок; ІІ – контрольна група (група порівняння) – ретроспективно проаналізовано 120 карт стаціонарних хворих на рак ендометрія, яким виконано пангістеректомію в період 2010–2013 рр.; ІІІ – основна група жінок, хворих на рак ендометрія, яким виконували хірургічне втручання (пангістеректомію) – 147 хворих, які перебували на хірургічному лікуванні в 2014–2017 рр. Результати. На першому етапі нашого дослідження проаналізовано основні антропометричні характеристики обстежених жінок і ризик виникнення тромботичних ускладнень за шкалою J. Сарrіnі. В основній групі обстежених і в групі порівняння виявлено зростання ризику виникнення тромбоемболій порівняно з групою здорових осіб, що вказувало на високий і вкрай високий ризик їх виникнення на операційному етапі лікування. При вивченні гемостазіологічних показників в основній групі та групі порівняння у доопераційний, інтраопераційний і ранній післяопераційний періоди виявлено гіперкоагуляцію з ознаками внутрішньосудинного згортання, що проявлялась достовірним зниженням протромбінового індексу та скороченням ТЧ і АЧТЧ з одночасним підвищенням рівня ПЧ, ФГ, D-димеру. Ознаки гіперкоагуляції зростали під час операції та на 1–5 добу післяопераційного періоду і поступово знижувались до доопераційних значень, за винятком D-димеру, пік якого спостерігали на 8 добу післяопераційного періоду. Створено і впроваджено в практику охорони здоров’я алгоритм оптимізації діагностичної та хірургічної профілактики тромбоемболій у хворих на рак ендометрія з врахуванням ризику їх виникнення. Висновки. 1. Досягнуто зниження смертності в ранній післяопераційний період у хворих на рак ендометрія від тромбоемболії легеневої артерії з 4,17 до 0 % і післяопераційних тромботичних ускладнень – з 11,67 до 1,36 %. 2.Розроблено і впроваджено в клінічну практику алгоритм оптимізації діагностичної тактики та хірургічної профілактики можливих венозних тромбоемболічних ускладнень у хворих на рак ендометрія з урахуванням стану активності системи гемостазу та антропометричних даних і вмотивовано надано дієві практичні рекомендації.
Мета роботи: провести оцінку стану системи гемостазу, розробити індивідуальні підходи для визначення груп ризику та профілактику тромботичних ускладнень (ТУ) у хворих на рак ендометрія (РЕ) на доопераційному етапі. Оптимізувати хірургічну тактику при виявленні тромбів у судинах малого таза при гістеректомії.Матеріали і методи. При дослідженні використано загальноклінічні методи обстеження: лабораторні, ультразвукове дослідження, гістологічні методи; математичний, статистичний – для аналізу та узагальнення данних у пакеті Statgraph (v.3.0).Результати досліджень та їх обговорення. До операції рівень D-димеру у 58,50 % хворих перевищував норму, що свідчило про активацію системи зсідання крові та загрозу розвитку ТУ, що було підтверджено у 5,4 % хворих, завдяки проведеню еластографії судин малого таза. До операції спостерігали збільшення рівня D-димеру у 86 (58,50 %) хворих, при операційному втручанні у 109 хворих (74,15 %).Під час гістеректомії проведено ревізію кукс судин малого таза на наявність тромбів. У 11,56 % хворих при ревізії кукс судин малого таза виявлені тромботичні маси за лінійним розміром були від 0,3 до 5,4 см, що підтверджено гістологічним дослідженням. У 17 пaцієнток під час проведення тромбектомії було виділено 19 тромбів.Створено математичну модель для рівняння множинної регресії, яка може бути використана для прогнозу наявності (відсутності) тромбів у венах малого таза.Розроблено алгоритм оптимізації діагностичної та хірургічної тактики у хворих РЕ з врахуванням ризику можливих ТУ, який дозволяє оптимізувати діагностичну і хірургічну тактику, що сприяє активному виявленню тромбів у хворих із РЕ.
Neoplastic processes are a pathogenic trigger for the development of thrombotic complications, including deep vein thrombosis and pulmonary artery thromboembolism. Post-operative thrombosis is one of the leading mortality causes of oncological patients due to its asymptomatic course, and therefore due to the late diagnosis and treatment. Purpose — to estimate the coagulation system condition in women after the type I hysterectomy depending on their body mass index. Materials and methods. The study of hemostasis parameters of 50 patients with the uterus and uterine appendages oncological diseases depending on the body mass index was carried out. The results were processed by the method of variation statistics, which became the basis for their generalization. Calculations of the parametric Student's t-test and methods of nonparametric statistics (Mann–Whitney test) were used. Differences between comparable parameters were found statistically significant at p<0.05. Results. The analysis of hemostasis parameters in oncological patients has revealed a twice higher concentration of fibrinogen and soluble fibrin monomer complexes in comparison to the control group. All three groups were characterized by hyperthrombocytopenia. The surgery provoked further activation of I, II, and III phases of coagulation on the background of negative thrombodynamics caused by intraoperative blood loss. In women with I and II obesity grade no statistically significant differences in coagulogram were observed, whereas in patients with III obesity grade the thrombotic potential was significantly more aggressive. Conclusions. Oncological diseases of the female reproductive system are associated with the impairment of coagulation status: lower activated partial thromboplastin, hyperfibrinogenemia, and increased prothrombin index. The surgery was a significant trigger of the increase of hemostatic potential with the progressive increase of the markers of I, II, and III coagulation phases. III obesity grade is an independent risk factor for thrombotic complications. The research was carried out in accordance with the principles of the Helsinki declaration. The study protocol was approved by the Local Ethics Committee of all participating institution. The informed consent of the patient was obtained for conducting the studies.
