Intrathoracic ectopia is an unusual ectopia that represents less than 5% of renal ectopia. The association of a duplex system and an intrathoracic renal ectopia is very rare. We report a case of a girl affected from these two anormalities, being the first case published in the literature reviewed. Presentation, diagnosis and therapeutic aspects are discussed.
Horseshoe kidney is the most frequent fusion abnormality of the kidney. The incidence of renal carcinoma in patients with horseshoe kidney is similar to those with normal anatomy. Its special anatomical features must be borne in mind for both surgical approach and conservative surgery. We present a horseshoe kidney isthmus carcinoma case report in which we performed conservative surgery of both renal units.
RESUMENSCHWANOMA RETROPERITONEAL MALIGNO Presentamos un caso infrecuente de Schwanoma retroperitoneal maligno de diagnostico casual. Describimos sus manifestaciones clínicas, exploraciones diagnósticas, y el tratamiento utilizado. NOTA CLÍNICA L os tumores que asientan en el retroperitoneo suelen ser poco frecuentes y se suelen clasificar según el tejido del que se originan. Dentro de los tumores derivados de las vainas nerviosas se encuentra el Schwanoma, considerándose que supone entre 1 -10% del total de neoplasias primarias retroperitoneales 1 . En el presente caso clí-nico presentamos un Schwanoma maligno diagnosticado de forma accidental, como suele ocurrir en este tipo de cánceres, tras la realización de una nefrectomía radical izquierda. CASO CLÍNICOV.P.C. Varón de 66 años con antecedentes de hipercolesterolemia, HTA, exfumador desde hace 4 años y disfunción eréctil. Ingresa a través del Servicio de Urgencias por deterioro del estado general. La exploración es anodina y no se observan ningún estigma cutáneo significativo. Tras ecografía abdominal se observa masa hipoecogé-nica heterogénea de polo superior de riñón izquierdo. El TAC muestra masa de contornos polilobulados e hipodensos, de unos 11 cm de diámetro y situada en el polo superior de riñón izquierdo (Fig. 1). Se observa afectación de la grasa perirrenal pero no adenopatías locorregionales. Los estudios analíticos sanguíneos y de orina fueron normales.Se realiza Nefrectomía renal izquierda ampliada por vía anterior, extrayéndose una masa de tamaño considerable.El estudio anatomopatológico describe un tumor cuyas dimensiones son de 12,5 x 9,5 x 9,5 cm, que invade la cápsula renal pero no el parén-quima ni la vía excretora del citado riñón. A microscopio se observan células fusiformes de borde impreciso dispuestas en fascículos y con cierto pleomorfismo nuclear. El estudio inmunohistoquímico es positivo a Vimentina S-100, así como a Enolasa neuroespecífica, todo ello compatible con Schwanoma maligno, que en este caso se localiza en el retroperitoneo (Fig. 2).Durante el seguimiento el paciente ha presentado un ACV por infarto isquémico temporal derecho sin apenas secuelas. En un TAC de control a los 2 años, se aprecia un engrosamiento de pared anterior gástrica que tras endoscopia digestiva alta y biopsia, se diagnostica de adenocarcinoma gástrico, siendo sometido, posteriormente, a gastrectomía subtotal.

RESUMENPAPILOMA INVERTIDO VESICAL CON RECIDIVA EN URETRA PROSTÁTICA El papiloma invertido del tracto urinario es una lesión generalmente solitaria y con poca tasa de recidivas.Su comportamiento es desconocido, no estando aclarado su potencial de recidiva y/o progresión. Presentamos un caso de papiloma invertido vesical con recidiva en uretra prostática a los cuatro años de seguimiento. Revisamos aspectos etiológicos, clínicos, diagnósticos y de tratamiento así como la discusión actual sobre su potencial de malignización.Palabras clave: Papiloma invertido. Recidiva. Potencial maligno. ABSTRACTINVERTED PAPILLOMA OF THE BLADDER WITH RECURRENCE IN PROSTATIC URETHRA Urothelial inverted papilloma is usually a solitary lesion with rare recurrences. Its behaviour is unclear, its potential for recurrence or progression is not well-known. We present a case of inverted papilloma of the bladder with recurrence in prostatic urethra after four years of follow-up. We review etiology, clinical presentation, diagnose, treatment and the present discussion about its malignancy ability. NOTA CLÍNICA E l papiloma invertido es una lesión poco frecuente del tracto urinario. Su frecuencia se estima en un 2,2% de todas las lesiones tumorales de la vía urinaria 1-9 . La localización más habitual es en cuello (44,7%) y trígono (36,8%) vesicales 6,9,10 , aunque también puede aparecer en otras zonas del tracto urinario como pieloureteral (7%), ureteral (12%) y con mucha menor frecuencia en uretra prostática ( 3%) 1,2,[5][6][7]9,11 .Clínica y macroscópicamente se confunde con facilidad con el carcinoma urotelial aunque debido a sus específicas características histológicas, es considerado como una entidad propia 6,8 .Histopatológicamente todavía existe hoy en día controversia acerca de su relación con el carcinoma del tracto urinario 3,4,9,[12][13][14][15] , hecho al que contribuye el que la mayoría de publicaciones existentes correspondan a series cortas de pacientes, siendo quizá necesarios estudios con mayor tiempo de seguimiento y número de casos.Presentamos un nuevo de caso de papiloma invertido vesical con recidiva en uretra prostática a los 4 años. CASO CLINICOPaciente varón de 61 años en la actualidad, fumador de 20 cigarrillos/día, sin otros antecedentes de interés, es remitido a nuestra consulta en Febrero del año 2000 por cuadro de hematuria total monosintomática, esporádica y autolimitada de inicio aproximado dos meses antes. En la anamnesis refiere sintomatología prostática leve, de predominio obstructivo de varios años de evolución.
