Aim: This study aims to identify the causes of medication administration errors and the reasons stated by nurses working in surgical clinics and surgical intensive care units for not reporting these errors, and to compare the number of errors nurses witness and the number of errors actually reported. Method: The research sample of this descriptive cross-sectional study consists of 125 nurses working in surgical clinics and surgical intensive care units. Data were collected using the face-to-face survey method. The Nurse Personal Information Form and the Medication Administration Error Scale were used to collect data. Results: The perceived medication administration error rate was found to be 59%; however, 61.6% of the nurses reported that only 0-20% of medication administration errors were actually reported. The main reasons for not reporting medication errors are administrative response (4.10±1.18) and fear (3.28±0.99). The most important reason for not reporting errors due to the administrative response is that no positive feedback is given after correct medication administration (4.18±1.53). The perceived cause of medication administration error is pharmacy-related and system-related, which explains 26% of the reason for not reporting the error due to fear. The fact that the reason for medication administration error originates from the physician, pharmacy and system explains 51% of the reason for not reporting the error due to disagreement over the error. Conclusion: There is a major difference between the medication administration errors nurses witness and the errors they actually report. The errors are not reported due to administrative response and fear.
Amaç: Araştırmanın amacı bir yıllık dönemde Acil servise başvuran travmalı çocukların özelliklerinin ve acil servishemşirelerinin acil hemşirelik girişimlerinin tanımlanmasıdır. Yöntem: Bu araştırma retrospektif yöntemle yapılmış olup acil servisde travma tanısı koyulmuş 700 çocuğun dokümanı incelenmiştir. Veriler Hemşirelik Girişimleri Sistemi, Hemşirelik Yönetmeliği ve Acil Hemşireliği Derneği güncel kılavuzları ışığında kayıtlı aktiviteler sınıflandırılmıştır. İstatiksel analiz sonuçları kategorik veriler için frekans ve yüzde olarak sunulmuştur. Bulgular: Çocuk travmalarında en önemli etyolojik faktörlerin yüksekten düşme (%38.3) ve trafik kazaları (%24.9) olduğu bulunmuştur. Çocukların ergenlik (%28.7) döneminde travmalara karşı daha savunmasız oldukları tespit edilmiştir. Erkek çocukların (%65.6) kız çocuklardan (%34.4) iki kat daha fazla travmaya maruz kalma riskine sahip olduğu saptanmıştır. Acil servise travmalı çocukların başvuru sıklığı akşam saatlerinde (%57) ve yaz mevsiminde (%41.6) artış göstermektedir. Travmalı çocukların hemşirelik bakımına ilişkin kayıtların içeriğini en çok vital bulgulara ilişkin bilgiler (%83.3) oluşturmaktadır. İkinci sırada sıklıkla uygulanan girişimlerin dolaşım problemleri (%55.6) ile ilişkili olduğu saptanmıştır. Acil servis hemşirelerinin hasta tanısı koyma, nörolojik takip ve hasta öyküsünü alma (%0.7) girişimlerini neredeyse hiç uygulamadıkları veya kayıt etmedikleri bulunmuştur. Sonuç: Acil servis hemşirelerinin travmalı çocuklara yapılan uygulamalarda Acil Hemşireliği Derneği güncel kılavuzlarına ve Hemşirelik Yönetmeliği’ne yeterli uyumunun olmadığı bulunmuştur. Çocukların travmaya maruz kalmasına neden olan en önemli etyolojik faktörlerin yüksekten düşme ve trafik kazaları olduğu saptandı. Ergenlik problemlerine yönelik savunma mekanizmalarının güçlendirilmesi ve ailelerin bu konuda desteklenmesi travmalı çocuk sayısının azalmasına katkı sağlayabilir.
Background: Nurses have a key role in ensuring the safety of patients, reducing the likelihood of errors and improving patient outcomes. Aims: This study aimed to determine the factors affecting patient safety, with a focus on the culture and attitudes of nurses working in emergency units. Method: This cross-sectional, descriptive, correlational study was conducted between 10 January and 30 August 2015 among 282 nurses who worked at emergency units of 19 hospitals in the north-central Black Sea Region of Turkey. Data were obtained using descriptive information forms, the Patient Safety Attitude Scale (PSAS) and the Patient Safety Culture Scale (PSCS). Results: The mean total PSAS score was 152.26 [standard deviation (SD) 22.54; range 46–230], while the mean total PSCS score was 2.56 (SD 0.52; range 1–4). Around a quarter of the participants reported errors, such as medication errors and patient falls, which threatened patient safety in the emergency units. The case report forms were not filled when these errors occurred. Conclusion: Attitude and culture of nurses in hospital emergency units towards patient safety differed according to their sociodemographic and work–life characteristics, including being satisfied with working in the emergency room, quality of work–life, level of job satisfaction, and number of years working in the emergency room.
The study was conducted to reveal the knowledge university students studying sports sciences about first aid and basic life support (CPR). A total of with a mean age of 21.15±2.12 years who reached 803 university students. Data were collected using a questionnaire which consisted of a total of 36 questions under three parts. The questionnaire was prepared based on the 2015 guidelines of the American Heart Association and European Resuscitation Council. It was found that 75,6 % of the university students had low level of first aid knowledge, and 99,7 % had low level of basic life support knowledge 83,3% of the university students received first aid training before, and 40 % reported that they received the training two years ago. One significant finding is that the first aid (25,82±14,26) and CPR (45,23±22,88) scores of the university students who considered themselves competent in applying first aid were found to be lower than the others(p<0,05). The first aid knowledge level (56,40±21,12) of those who received applied first aid and CPR training was higher than those who received only theoretical ( 53,13±21,65) training (p<0,05). The students of the Coaching Education Department gave the highest number of correct answers to the First Aid and CPR questions. Almost all university students (88,4%) stated they wanted to receive first aid and CPR training. Our findings revealed that 90,4% of the Department of Physical Education and Sports Teaching students wanted to receive training, which shows that the students in the teaching programs are more sensitive about first aid and CPR. It was found that the first aid and CPR knowledge level of the university students studying sports sciences were low and, they were aware of this situation. Having received first aid training within the last year and both theoretical and practical training positively affected the level of knowledge.
Amaç: Araştırmanın amacı hemşirelerin profesyonel değerler algılarının yaşlı ayrımcılığı tutumları üzerine etkisinin belirlenmesidir. Yöntem: Araştırmaya bir üniversite hastanesinde çalışan 207 (%98.1) hemşire katılmıştır. Verilerin toplanmasında “Hemşireleri Tanıtıcı Form”, “Hemşirelerin Profesyonel Değerler Ölçeği (HPDÖ)” ve “Yaşlı Ayrımcılığı Tutum Ölçeği (YATÖ)” kullanılmıştır. Verilerin analizinde parametrik ve non-parametrik testler yapılmıştır. Araştırmanın hemşirelerin yaşlı ayrımcılığı tutumu bağımlı değişkeni ile profesyonel değer algısı bağımsız değişkeni arasındaki ilişki Pearson korelasyon, etki ise regresyon analizi ile test edilmiştir. Bulgular: Hemşirelerin YATÖ toplam puanı (82.28±9.12) yaşlı ayrımcılığı tutumunun olumlu yönde yüksek olduğunu ve HPTÖ toplam puanı (123.42±19.97) profesyonel değer algılarının yüksek olduğunu göstermektedir. YATÖ alt boyutlarından yaşlı Yaşamını Sınırlama (32.93±4.26) alanında ve HPDÖ alt boyutlarından İnsan Onuru (44.43±7.23) alanında hemşirelerin en yüksek toplam puana sahip oldukları görülmüştür. Ayrıca hemşirelerin profesyonel değer algısı ile yaşlı ayrımcılığı tutumu arasında pozitif yönde zayıf bir ilişki olduğu (r=0.147; p=0.035>0.05) bulunmuştur. Hemşirelerin yaşlı ayrımcılığı tutumu değişkeninin %2.1’inin hemşirelerin profesyonel değer algıları ile açıklandığı tespit edilmiştir. Sonuç: Hemşirelerin mesleki değer algısı ile yaşlı ayrımcılığı tutumu arasındaki ilişki zayıftır. Hemşirelerin yüksek insan onuru ve sorumluluk değer algısına sahip olmaları, yaşlı ayrımcılığı tutumlarını olumlu yönde etkilemektedir.
