86Sayın editör, Liken sklerozus (LS), çocukluk çağında vulvar kaşıntının nadir görülen nedenlerinden biridir.Genital bölgede vulva, penis ve anüsü etkileyebilen hastalık, kadınlarda erkeklerden daha sık olarak görülmektedir (1,2). Bu makalede polikliniğimizde LS tanısı koyulan bir çocuk olgu sunulmakta ve literatür bilgileri tartışılmaktadır. Dokuz yaşında kız çocuğu polikliniğimize genital bölgesinde bir yıldır devam eden kaşıntı nedeniyle gönderildi. Son bir yılda bu yakınması nedeniyle çeşitli doktorlara başvurmuştu. Vitiligo ve mantar enfeksiyonu tanılarıyla çeşitli kremler kullanmıştı. Ancak lezyonlar devam etmişti. Bilinen herhangi bir hastalığı yoktu. Dermatolojik muayenesinde mons pubis alt sınırından başlayarak, labiya majörleri içine alan ve anüse kadar uzanan beyaz renkli, atrofik plak lezyon görüldü (Resim 1). Plaktan LS ve liken planus ön tanılarıyla bir adet "punch" biyopsi örneği alındı. Alınan biyopside epidermisde hiperkeratoz, retelerde hafif silinme, bazal tabakada vakuoler bozulma, papiler dermisde yaygın homojenizasyon, retiküler dermisde yaygın kronik iltihap görülerek LS tanısı desteklendi (Resim 2). İstenilen laboratuvar testleri normal sınırlardaydı. Hasta çocuk psikiyatrisi ile konuşuldu ve cinsel istismar dışlandı. Hastaya topikal kortikosteroidli krem ve oral antihistaminik verilerek poliklinik izlemlerine gelmesi söylendi. Liken sklerozus, deri ve mukozaları etkileyen, enflamatuar bir deri hastalığıdır. İlk kez 1887 yılında, Hallopeau tarafından tanımlanmıştır. Çocukluk döneminde ergen kızlarda 1/900, erkek çocuklarda 1/200 oranında bildirilmiştir. Anogenital bölge tutulumu %85-98 arasında değişir (3,4). Klinik olarak, genital bölgede eritemli makül ve papüler olarak başlar. Zamanla beyaz, atrofik, parlak plaklar gelişir. Lezyonlarda ikincil olarak kanama, hiperkeratoz, erozyon ve bül görülebilir (5). Bizim hastamız dokuz yaşında bir kız çocuğuydu. Vulva ve anüsde tutulum var olup kaşımaya ikincil kanamalarda meydana gelmişti. Çocukluk çağında dermatit, mantar enfeksiyonları ve bakteriyal enfeksiyonlar vulvar kaşıntının sık görülen nedenleridir. Liken sklerozus ise nadir görülmekte ve genellikle başka hastalıklarla karıştırılmaktadır (6). Genital bölgede ekimoz ve peteşiyal odakların görülmesi nedeniyle cinsel istismar da ayırıcı tanıya girmektedir. Bu tür hastaların dermatoloji kliniklerine yönlendirilmesi doğru tanı konulması açısından önem taşımaktadır (7). Ayırıcı tanıya giren diğer hastalıklar arasında oksiüriyazis, psöriasis, kontakt dermatit ve morfea gibi hastalıklar yer alır (4). Bu hastalıklarla ayırıcı tanısı dikkatli bir fizik muayene ve biyopsi gibi yöntemlerle yapılabilir. Liken sklerozus, kaşıntı ve kanama yanında zaman içerisinde gelişen skar oluşumu nedeniyle anatomik yapıyı da bozmakta, vajinal darlık, miksiyon güçlüğü, kabızlık ve fimozis gibi sorunlara neden olabilmektedir (5). Lip carcinoma is one of the most common cancers of the head and neck region. Lower lips are more frequently affected. Squamous cell carcinoma accounts for more than 95% of the cases. The aim of treatment is to achieve the highest cure rates with adequate excision with clear surgical margins while leaving the smallest defect sizes behind for reconstruction. Excised tissue is known to shrink when removed, in relation with its retractile properties. In this study, the evaluation of the amount of shrinkage in excised lip cancer specimens and determination of the relation between surgical and histopathologic margins were aimed. The study included 21 consecutive excised lip specimens for the treatment of squamous cell carcinoma of the lip. The volume of the specimen, tumor length, and margin distance measurements were measured and recorded step by step from before surgical excision to histopathologic evaluation. Specimens were evaluated in a standardized fashion by the same pathologist after 48 hours of fixation. For data analysis, Number Cruncher Statistical System 2007 and Power Analysis and Sample Size 2008 Statistical Software (Kaysville, UT) were used. The surgical margins of the excised lip specimen shrink up to 41% to 47.5% from before excision to histopathologic evaluation. The most significant step was the excision step, followed by the formalin fixation step. The histopathologic process itself and the duration of formalin fixation did not result a significant change in surgical margins. The tumor tissue itself did not show a significant shrinkage.
Sinonasal adenocarcinoma is a rare neoplasm which is classified as 'intestinal' or 'nonintestinal' type, depending on its resemblance to gastrointestinal mucosa. These tumors are associated with occupational and environmental carcinogens. In this study, a fifty-year-old oil-painter male patient with a low-grade nonintestinal type sinonasal adenocarcinoma originating from the left middle concha and ethmoid sinus is presented. Microscopical examination revealed many infiltrative glandular structures, most of which were cystically dilated and some of which were smaller in diameter, arranged back to back in loose fibrous stroma as well as intraglandular papillary and micropapillary structures forming complex branches or a cribriform pattern. The glands were lined by epithelial cells that were faintly eosinophilic and relatively abundant cubical/ cylinderical cytoplasms and mildly pleomorphic round/oval nuclei, with rare mitotic figures. Intraluminal and focally intracytoplasmic mucin was demonstrated with Alcian Blue, mucicarmin and PAS stains. Immunohistochemically, tumor cells were strongly and diffusely positive with CK7; focally and weakly positive with CK20 and negative with CDX2 in accordance with the nonintestinal type. S-100, Actin and p63, applied for investigating the myoepithelial and salivary glandular origins, were all negative. Prognostic markers, TTF-1 and p53 were negative; while the Ki-67 index was 2%. The fact that intestinal type sinonasal adenocarcinomas are generally high grade, while nonintestinal tumors are histologically low grade makes this morphological and immunohistochemical-based classification valuable in predicting the prognosis of the disease. athological diagnosis of ovarian tumors seen in young adults is quite difficult. Sertoli-Leydig cell tumor (SLCT), a rare ovarian sex-cord stromal tumor should always be kept in mind in young cases of ovarian masses, since, beside its broad spectrum differential diagnosis and various variants, distinctive histological grades can complicate the diagnosis as well. 1,2 In this study three distinct SLCT cases, a well-differentiated, of intermediate differentiation and that with a retiform component were reported, emphasizing their differential diagnoses. Additionally we aimed to introduce an interesting clinical presentation of this tumor as seen in our third case with a retiform component which had a thyroid neoplasm. CASE REPORTS CASE 1 A 32 year-old woman admitted to emergency clinic with abdominal pain, nausea and vomiting lasting for 3 days. On physical examination, an appendectomy incision scar at the right lower quadrant and rebound tenderness at the left lower quadrant were observed.
