One and a half years into the pandemic, SARS-CoV-2 is still here to stay. Whilst rapid several effective COVID-19 vaccines have been developed and are being rolled out, the critical questions remain whether vaccines provide widespread protection against infection and reinfection, and what the duration of protection is. Community wide control cannot be obtained until almost everyone is immune. Vaccine production must be ramped up to cover the world population. The price of herd immunity through natural infection is high mortality in the elderly and morbidity in other age groups including children and Long-COVID. We must expect a new wave in the coming winter. The severity will depend on the proportion of the population with immunity from natural infections or immunisation. Therefore, control rests on a population wide immunisation including children, which may or may not need to be repeated if new SARS-CoV-2 variants evolve that can escape immunity from either previous infections or immunisations. Preventing long term sequelae of COVID-19 also remains a priority.
Bağışık yetmezliklilerde intestinal parazitik enfeksiyonlar sıktır. Gelişmekte olan ülkelerde HIV (insan immünyetmezlik virüsü) pozitif olgularda parazitik enfeksiyonlara bağlı ishal sıklığı %90'ın üzerindedir. Bu çalışmada HIV pozitifliği nedeni ile takip edilen ve gastrointestinal yakınmaları olan hastalarda bağırsak protozoonlarının varlığının retrospektif olarak incelenmesi amaçlandı. Yöntemler: Çalışmaya 14 kadın (%21,5), 51 erkek (%78,5) olmak üzere toplam 65 HIV pozitif hastanın dışkı örneği dahil edildi. Hastalara ait laboratuvar sonuçları laboratuvar bilgi sistemi kullanılarak retrospektif olarak taranırken, klinik veriler hasta dosyalarından elde edildi. Hastaların yaş, cinsiyet, dışkı incelemelerinde saptanan parazitler, CD4 sayısı, HIV-RNA düzeyi ve antiretroviral tedavi bilgileri kaydedildi. Bulgular: Dışkı örneklerinden 14'ünde (%21,5) Cryptosporidium spp., 2'sinde (%3,1) Cyclospora spp., 7'sinde (%10,8) Blastocystis spp., 1 örnekte ise Cryptosporidium spp.+ Blastocystis spp. (%1,5) tespit edildi. Dışkısında parazite rastlanan hastalarda antiretroviral tedavinin ortanca süresi 3 ay, dışkısında parazite rastlanmayan hastalarda 12 ay olarak hesaplandı. Dışkısında parazite rastlanmayan hastalarda antiretroviral tedavi süresi anlamlı olarak yüksek bulundu (p=0,002). Dışkıda parazit saptanması ile CD4+ T hücre sayısı ve HIV-RNA düzeyleri arasında anlamlı korelasyon saptanmadı. Sonuç: Bu çalışmanın bulgularına göre, HIV ile enfekte hastalarda antiretroviral tedavinin hastanın bağışıklık sistemi üzerindeki olumlu etkilerinin intestinal paraziter enfeksiyon riskini azalttığı ve korunma açısından bu tedavinin önemli rol oynayabileceği sonucuna varılmıştır.

O ver the past 38 years following the introduction of HIV infection (1), which affected the lives of 75 million people, many scientific advances in prevention, diagnosis, treatment and care have been recorded, and the lives of people living with HIV (PLWH) have been transformed. Currently, life expectancy for PLWH who are diagnosed early and who initiate antiretroviral treatment (ART) immediately is almost equivalent to that of HIV negative individuals. The 90-90-90 goals announced in 2014 and several milestones in prevention scaled-up efforts to eliminate HIV infection reducing the number of new infections by 16% since 2010 and AIDS-related deaths by more than 56% since the peak in 2004 while increasing ART coverage from 7% to 62% between 2005 and 2018 (2,3). Pre-exposure prophylaxis was shown to be highly effective in reducing new HIV infections both among heterosexual men and women and men who have sex with men (MSM) (4-8).
