Diphallus (penile duplication) is very rare and seen once every 5.5 million births. It can be isolated, but is usually accompanied by other congenital anomalies. Previous studies have reported many concurrent anomalies, such as bladder extrophy, cloacal extrophy, duplicated bladder, scrotal abnormalities, hypospadias, separated symphysis pubis, intestinal anomalies and imperforate anus; no penile duplication case accompanied by omphalocele has been reported. We present the surgical management of a patient with multiple anomalies, including complete penile duplication, hypogastric omphalocele and extrophic rectal duplication.
Objective: Large congenital bladder diverticula (LCBD), congenital bladder diverticula (CBD) larger than 2 cm diameter, is a rare anomaly. The aim of this study was to report long-term surgical and clinical outcomes of children with LCBD. Methods: Medical charts of all children who were diagnosed with LCBD at our institution between April 2005 and December 2017, with at least 2 year follow-up were retrospectively reviewed. Patients’ demographics, symptoms, operative technique, diverticulum size and localization, surgical outcomes and complications were recorded. Results: Fourteen patients with 18 LCBD, all male and age between 7 and 240 months (mean age: 53.5 months) were included in the study. Urinary tract infection was the main complaint in 10. Vesicoureteral reflux was detected in eight patients. Diverticula were 2–5.5 cm (mean 3.3 cm) in size. All diverticulectomies were performed transvesically and ureteroneocystostomy was added in 12 patients, 5 of whom were bilateral. No postoperative infection or recurrent reflux were observed. The median follow-up period was 4.5 years (2–12 years). Conclusion: Treatment of LCBD is mostly surgical and transvesical approach for diverticulectomy was found to be a safe and effective surgical procedure in long term follow-up.
Background. Urological involvement is rare in patients with coronavirus disease 2019 (COVID-19). Priapism, one of the urological involvements, was reported as one of the COVID-19 comorbidities in the elderly male patient group but has rarely been reported in the pediatric age group. Case. Herein, a previously healthy 8-year-old patient with COVID-19-associated priapism, which is rare in children, is presented. Conclusions. During the pandemic, in pediatric cases with priapism of unknown etiology, COVID-19 should be one of the diagnoses to be considered.
Objective: The study aimed to retrospectively evaluate postoperative results of patients with duplex system ureterocele (DSU) who underwent endoscopic ureterocele incision (UI), lower urinary tract reconstruction (LUTR) or both, and investigate the necessity of upper pole heminephrectomy (UPH) in ureterocele treatment. Patients and methods: In total, 44 patients with DSU who were operated on in December 2005 and December 2018 were evaluated for patient characteristics, ureterocele location, differential renal function (DRF), vesicoureteral reflux (VUR), postoperative incontinence, proteinuria, hypertension and urinary tract infection (UTI). Results: UI was performed in 27 of 44 cases (61.3%) as the first-line treatment. Ten of them (37%) had no urinary infection or renal function loss at a mean of 3 years (1.5–7 years). LUTR was needed in 17 patients after UI due to VUR in 14 patients and bladder outlet obstruction (BOO) in 3 patients. In total, 34 patients who underwent LUTR, proteinuria or incontinence were not detected in the mean 6 years’ follow-up. Thirty-three patients (97%) had identical renal scans and similar DRF before and after the operation. No patient underwent UPH. Conclusion: UI must be considered the first-line treatment in DSU in infancy. In older patients, especially with VUR, LUTR is effective and safe and UPH mostly is not necessary. Level of evidence: 4
ÖZET AMAÇ: Çocukluk çağında düşük dereceli künt böbrek yaralanmalarında konservatif tedavi yöntemleri kullanılır. Ancak dördüncü derece yaralanmalarda tedavi protokolleri tam olarak belirlenememiştir. Çalışmamızın amacı, kliniğimize ürinom ile başvuran veya üriner ekstravazasyon saptanan hastalarda tedavi yöntemlerini tartışmaktır. GEREÇ VE YÖNTEM:Kliniğimize 2003-2012 yılları arasında başvurmuş olan ve dördüncü derece renal travma saptanmış olan sekiz olgu (K/E=1/7, ortalama yaş: 6 yaş) uygulanan tedavi yöntemleri açısından geriye dönük olarak incelendi. BULGULAR:Dört olguda yüksekten düşme, üç olguda batına künt travma ve bir olguda kereste makinesine sıkışma nedeniyle renal yaralanma vardı. Dört olguda sağ, dört olguda sol renal travma saptandı. Hiçbir olguda hemodinamik instabilite nedeniyle acil eksplorasyon gerekmedi. Bir olgunun ilk incelemelerinde üriner ekstravazasyon gösterilmiş olmasına rağmen izlemde ürinom saptanmadı. Ürinom gelişen yedi olgunun beşi drenaj yöntemleri ile (üç olguda JJ kateter, bir olguda perkütan drenaj (PD) kateteri, bir olguda PD sonrasında JJ kateter yerleştirilerek) tedavi edildi. Geri kalan iki olguda ilk tedavi olarak PD uygulanmasına rağmen üriner sistem bü-tünlüğünün bozulmuş olması nedeniyle alt pol nefrektomi ve pelviplasti uygulandı. Bir olguya bulguları geriledikten sonra üreteropelvik darlık nedeniyle piyeloplasti uygulandı. Tüm kateterler üriner sistemde ekstravasyon olmadığı gösteridikten sonra çekildi. JJ kateter çekilme süresi ortalama dört ay, PD kateter çekilme süresi 1.5 ay idi. SONUÇ:Çocukluk çağındaki dördüncü derece renal yaralanmalar minimal invaziv yöntemlerle başarıyla tedavi edilebilmektedir. Cerrahi uygulanan olgularda bile öncelikle minimal invaziv yöntemlerin seçilmesi böbreğin korunma şansını artırmaktadır.Anahtar sözcükler: Böbrek travması; çocukluk çağı; ürinom. [2] Düşük dereceli yaralanmaların neredeyse tamamı konservatif olarak tedavi edilebilmektedir.[3] Ancak özellikle vasküler olmayan dördürcü derece yaralanmalardaki tedavi protokolleri tam olarak belirlenememiştir. Bu tip yaralanmalarda zarar gö-ren renal dokunun yanında üriner kaçak da geliştiğinden morbidite yükselmektedir.Bu çalışmada, travma sonrasında üriner ekstravazasyon saptanan veya izlemde ürinom gelişen olguların tedavi yöntemleri değerlendirildi.
