Objective To identify clinical and demographic factors associated with missing strings and expulsion after postplacental insertion of copper T380A intrauterine devices (TCu380A IUD). Methods This is a secondary analysis of an implementation study. We followed participants who had a postplacental TCu380A IUD insertion, at two postpartum visits: 45–90 days (visit 1) and 6–9 months (visit 2). We used multiple log‐binomial regression models to evaluate the demographic and clinical variables associated with missing strings and with IUD expulsion. Results We included 705 participants who had a postplacental IUD insertion. We observed missing strings in 275 (47.9%) participants at visit 1, and in 127 (34.2%) participants at visit 2. We identified 61 expulsions (8.9%) by visit 2. In the multiple regression models, only the delivery type was associated with missing strings and expulsion. Compared with vaginal delivery, cesarean delivery increased the risk of missing strings (adjusted relative risk [aRR] 6.21; 95% confidence interval [CI] 4.29–8.99) but reduced the risk of IUD expulsion (aRR 0.24; 95% CI 0.13–0.43). Conclusion The delivery mode was the only factor associated with missing strings and expulsion after postplacental IUD insertion. Cesarean section was associated with an increased risk of missing strings but decreased risk of expulsion after postplacental TCu380A IUD insertion.
A inserção de métodos contraceptivos reversíveis de longa duração no momento do pós-parto promove melhorias no acesso ao planejamento reprodutivo. O dispositivo intrauterino (DIU) é considerado um método seguro, eficaz e reversível. Para tanto, objetivou-se analisar a produção científica sobre o seguimento do acompanhamento de DIU inserido no pós-parto. Trata-se de revisão integrativa, realizada em fevereiro de 2021, cujos critérios de elegibilidade foram artigos ou documentos oficiais que abordassem orientações no seguimento do DIU inserido no período pós-parto e puerpério. Após cruzamentos utilizando os descritores “Dispositivo Intrauterino” (Intrauterine Devices), “PPIUD”, “acompanhamento” (Follow-Up) e “Pós-parto/ Puerpério” (Postpartum Period) no Portal de Periódicos Capes, Scielo, Medline/Pubmed (Biblioteca Virtual de Saúde). A amostra final foi composta por quatro artigos. Os estudos mostraram que os efeitos colaterais como dor, sangramento e mau posicionamento podem ocorrer na primeira até oito semanas pós-inserção do DIU. Para avaliação do mau posicionamento do DIU deve ser realizada ultrassonografia e exame especular. As consultas de seguimento podem ser presenciais ou por contato telefônico, realizadas por profissionais da atenção básica treinados. Concluiu-se que quando as usuárias recebem as informações de forma clara, as chances de continuidade do método são significativamente maiores.
Objetivo: desenvolver tecnologia educativa audiovisual de animação gráfica sobre aceitabilidade de mulheres durante o pré-natal frente ao uso do Dispositivo Intrauterino pós-parto imediato. Método: elaboração do produto tecnológico em três etapas: pré-produção, produção e pós-produção. O produto foi iniciado após obtenção dos resultados do questionário com perguntas de resposta aberta. O storyboard foi dividido em conteúdo/roteiro, imagem/cena e som, com três personagens, repórter, médica e paciente. A tecnologia educativa audiovisual foi fragmentada em quatro partes, quais sejam: sinopse, argumentos, roteiro e storyboard. Resultados: das 746 mulheres, 46,9% não aceitaram a inserção do Dispositivo Intrauterino. Na análise, a não aceitação ao Dispositivo Intrauterino resultou em uma tecnologia leve-dura em formato audiovisiual para promoção do aumento do uso do dispositivo no pós-parto. Conclusão: o diagnóstico situacional apontou o desejo de outro método contraceptivo e medo como principais motivos de não aceitabilidade do Dispositivo Intrauterino, embasando a essência de construção da tecnologia educativa.
