Introdução: Os linfomas difusos de células B (LDGCB) são grupos de linfomas que costumam apresentar-se com uma massa ganglionar de crescimento rápido e de natureza agressiva, mas, potencialmente, curável. Apesar de ainda envolver um alto custo, a técnica de monitoramento com o uso de DNA Tumoral Circulante (ctDNA) vem se mostrando promissora ao revelar ser um método minimamente invasivo e que ajuda determinar a doença e avaliar, com melhor precisão, medidas terapêuticas específicas para tal patologia. As limitações inerentes ao empreender uma técnica nova exigem uma constante revisão e análise, a fim de alcançar uma melhor compressão e aproveitamento desse método. Objetivo: Identificar os avanços no uso de ctDNA para a efetividade no tratamento do LDGCB. Material e métodos: Trata-se de uma revisão bibliográfica de natureza qualitativa realizada em janeiro de 2022. Foram utilizadas as bases de dados “EMBASE” e “MEDLINE”, dispondo dos descritores “Lymphoma, Large B-Cell, Diffuse”, “Circulating tumor DNA” e “Treatment Outcome” combinados pelo operador "AND". Na busca por artigos originais publicados entre os anos de 2017 e 2022, foram encontrados 31 artigos na língua inglesa e após a leitura do título e dos resumos, além da remoção de artigos pagos e de duplicatas, restaram 6 artigos para revisão. Resultados: Demonstrou-se que a detecção de ctDNA auxilia na tipificação do câncer, na avaliação da agressividade do LDGCB, na compreensão da resposta ao tratamento e verificação da possibilidade de recidiva. Certamente, é um biomarcador capaz de estratificar os riscos, já que um estudo de 2018 revelou que 98% dos pacientes acometidos obtiveram o ctDNA detectável. Essa contribuição cedida pela técnica do ctDNA é valiosa devido à alta precisão, ao ponto de conseguir prever a progressão da doença mais cedo que ao usar tomografia por emissão de pósitrons e computadorizada. Conclusão: Diante dos benefícios do rastreio de ctDNA contra o LDGCB, torna-se evidente a necessidade do maior desenvolvimento de pesquisas nesse eixo, com o propósito de acelerar a ratificação desse mecanismo como ferramenta de auxílio ao diagnóstico, ao prognóstico e à terapêutica.
