Проведен анализ работы двух отделений, оказывающих неотложную помощь больным с ургентной сосудистой патологией в г.Казани.Рутинными методами диагностики заболеваний магистральных и перифери-ческих сосудов являются ультразвуковое исследование (УЗИ) и реже ангиография. Недостатки этих методик диктуют необходимость внедрения методов магнитно-резонансной томографии (МРТ) и компьютерной томо-графии (КТ) в ангиорежимах, как наиболее чувствительных, безопасных методов, с минимальным процентом ложноотрицательных результатов в работу отделений сосудистой хирургии. Основное место по количеству экстренной госпитализации занимают острые тромбозы глубоких вен нижних конечностей. Данная категория больных нуждается в длительном лечении и инструментальном наблюдении, что ведет к большим матери-альным и моральным затратам. Решением данной проблемы может явиться наличие дневного стационара. Второе место занимают тромбозы периферических артерий. Наличие у этих больных тяжелой сопутствующей патологии требует применения сочетанных «открытых» и рентгенэндоваскулярных вмешательств в условиях гибридной операционной. Современное состояние сосудистой хирургии диктует необходимость в профессио-нальной переподготовке ангиохирургов по специальности «рентгенэндоваскулярная диагностика и лечение». Назрела необходимость разработки скрининг-программ по раннему выявлению наиболее распространенных заболеваний сосудов, технической оснащенности современным оборудованием отделений сосудистой хирур-гии, функционирования консультативных кабинетов и дневных стационаров, организации помощи больным, нуждающихся в формировании постоянного сосудистого доступа, а также больным с критической хронической ишемией нижних конечностей. Казани, Россия Реферат. Статья посвящена ретроспективному анализу лечения больных с тромбозом глубоких вен (ТГВ). С 2007 по 2012 г. было пролечено 704 больных с ТГВ. 590 получали консервативное лечение. В 114 наблюдениях применялась активная эндоваскулярная или хирургическая тактика. Основным и определяющим тактику лечения методом исследования было компрессионное ультразвуковое дуплексное ангиосканирование (УЗДС). Исследование показало тенденцию роста выявляемости больных с ТГВ. В группе больных с консервативным лечением умерло 5 больных, из них 4 от тромбоэмболии легочной артерии (ТЭЛА) и 1 от венозной гангрены. Все умершие от ТЭЛА имели окклюзивную форму ТГВ. Анализ причин летальности заставляет сомневаться в активизации больных с окклюзивными формами венозного тромбоза и рекомендовать «активный» постельный режим с постоянной лечебной гимнастикой. КОгДа ВЕРСТаЛСя НОМЕР женских группах испытуемых, проживших на Севере РФ менее 10 лет, показал увеличение расстояний (z ij ) между центрами квазиаттракторов, что свидетельствует об адекватной реакции адаптации функционального состояния организма в ответ на влияние экстремальных условий проживания. В свою очередь, при сравнении межаттракторных расстояний групп женщин 36-55 лет, проживающих на Севере до 10 и более 10 лет, отмечалось наименьшее значение (z ij = 294.84 у.е. ), что свидетельствует о снижении способности к адаптации организма в возрасте 36-55 лет.В свою очередь, в группах мужчин в возрасте 21-35 лет, проживающих в условиях Севера РФ, значения межаттракторных расстояний имеют наименьшее значение (z ij = 1 534.51 у.е. ), что может свидетельствовать о снижении способности к адаптации в этих возрастных группах.3. Новые методы изучения состояния механизмов ФСО могут быть использованы для оценки адекватности и эффективности работы системы гомеостаза населения, длительно проживающего на Севере РФ, с использованием методов системного анализа и синтеза. Это позволяет подойти к научному прогнозированию изменений системы гомеостаза во время пребывания населения в экстремальных условиях Югры.4. Учитывая все сказанное, крайне важно внедрять в медицинскую практику раннее распознавание неадекватности реакции организма на пребывание в столь сложных условиях климатической зоны Югры методом расчета матриц межаттракторных расстояний квазиаттракторов ВСОЧ.
