BACKGROUND:Nasopharyngeal Angiofibroma is a rare neoplasm in the sphenopalatine foramen. This tumour is histologically benign, but clinically malignant because it can erode the bone and surrounding structures, such as the pterygopalatine fossa, paranasal sinuses, and nasal cavity. It is a highly vascular tumour, sometimes from multiple Feeding arteries, and tends to bleed easily.CASE PRESENTATION:In these cases, series, we reported four cases of nasopharyngeal angiofibroma in children and one case in an elderly patient. The diagnosis was made by history taking, physical examination and Cerebral MSCT Angiography, as well as Digital Subtraction Angiography (DSA). After identification of the Feeding artery, we performed transarterial embolisation using polyvinyl alcohol (PVA) foam particles.CONCLUSION:Preoperative embolisation in the highly vascular tumour, such as nasopharyngeal angiofibroma, is very useful to reduce peri-operative complication of surgery. This procedure can reduce blood loss during resection of the tumour and gives better outcomes.
Penelitian ini bertujuan mengetahui korelasi derajat leukoaraiosis berdasarkan CT-Scan kepala dengan faktor-faktor risiko stroke dan keparahan stroke berdasarkan derajat klinis pada pasien stroke iskemik. Metode yang digunakan adalah cross sectional, dilakukan secara retrospektif pada penderita stroke iskemik yang menjalani pemeriksaan CT-Scan kepala di Bagian Radiologi RSUP dr. Wahidin Sudirohusodo, Makassar periode Januari 2020 sampai Juli 2020. Sampel sebanyak 46 orang dengan usia 40 tahun yang mengalami serangan stroke pertama dengan onset 1 bulan. Derajat leukoaraiosis dinilai dengan menggunakan skala Van Swieten dan derajat klinis dinilai dengan menggunakan National Institute of Health Stroke Scale (NIHSS). Uji statistik yang digunakan adalah uji korelasi Spearman. Hasil penelitian menunjukkan bahwa ada korelasi bermakna yang sedang dan searah antara derajat leukoaraiosis dan kelompok umur dengan nilai p sebesar 0,004 (0,05) dan nilai r sebesar 0,415. Tidak ada korelasi bermakna antara derajat leukoaraiosis dengan jenis kelamin, hipertensi, diabetes melitus, dislipidemia dan derajat klinis dengan nilai p masing-masing secara berurutan sebesar 0,146; 0,520; 0,779; 0,185; dan 0,537 (0,05). Namun tampak kecenderungan bahwa pasien dengan hipertensi tidak terkontrol memiliki derajat leukoaraiosis yang lebih berat.
Takayasu's arteritis is one of the causes of ischemic stroke at a young age, but the occurrence is still rarely reported in Indonesia. Here we report the case of a 19-year-old Indonesian female who was admitted to the Wahidin Sudirohusodo Hospital with a sudden onset of right-sided hemiplegia. Other features of Takayasu’s arteritis were observed, including a discrepancy of more than 10 mmHg between the left and right arm systolic blood pressure, claudication of the extremities, and decreased brachial artery pulse. Left hemisphere cerebral infarction was confirmed on the computed tomography (CT) scan. Angiogram abnormalities were also present, showing occlusion of the left proximal inner carotid artery after branching at the aortic arch, and occlusion of the left subclavian artery. Although no fever was observed on presentation, the patient had an elevated erythrocyte sedimentation rate (ESR) of 44 mm/hour. In conclusion, Takayasu’s arteritis should be considered as a potential cause of ischemic stroke in young adults.
