INTRODUÇÃO: A raquianestesia está indicada em vários procedimentos cirúrgicos, em especial cirurgias infraumbilicais e de membros inferiores, como as cirurgias ortopédicas, ginecológicas/obstétricas e urológicas. Bem como em associação com outras técnicas anestésicas para conferir analgesia no pós operatório. Para melhorar a eficácia da raquianestesia, adjuvantes de diferentes classes farmacológicas são usados para aumentar a ação do anestésico local, prolongar tempo da anestesia, diminuir necessidade de dose altas de anestésicos locais e reduzir os efeitos colaterais dose-dependentes. OBJETIVO: Fazer uma revisão na literatura sobre os principais adjuvantes utilizados na raquianestesia, suas características e efeitos. MÉTODOS: Revisão integrativa de literatura. CONCLUSÃO: Adjuvante aos anestésicos locais é um campo em evolução e excitante da prática anestésica com novas tecnologias que prometem melhorar a satisfação e a segurança do paciente.
