RESUMO: Introdução: A prevalência de obesidade está aumentando em um ritmo alarmante em muitos países. Uma alimentação não saudável e o sedentarismo são os principais fatores de risco para a obesidade. O objetivo deste artigo foi estudar a prevalência e identificar fatores associados à obesidade na população adulta brasileira com base nos dados coletados na Pesquisa Nacional de Saúde, 2013. Método: Amostra de 59.402 indivíduos adultos, excluindo-se as mulheres grávidas. O índice de massa corporal foi calculado por meio das aferições de peso e estatura. A obesidade foi definida por índice de massa corporal ≥ 30 kg/m2. Utilizaram-se modelos de regressão logística para identificar os fatores associados à obesidade. Resultados: As prevalências de obesidade foram de 16,8% para homens e 24,4% para mulheres. Idade avançada (a partir dos 50 anos), nível de instrução baixo (sem instrução ou ensino fundamental incompleto), raça/cor preta e viver com companheiro foram fatores de risco à obesidade. A atividade física no lazer e o hábito de assistir mais de 4 horas de televisão por dia mostraram associações significativas para ambos os sexos. Quanto à morbidade referida, em pessoas obesas, as chances de ter o diagnóstico de hipertensão, diabetes ou de alguma doença crônica não transmissível foram maiores. Homens e mulheres obesos tiveram a pressão arterial sistólica significativamente aumentada. Conclusão: Os achados enfatizam a importância de políticas públicas para a prevenção da obesidade e para a promoção de hábitos saudáveis na sociedade brasileira.
Background:Female sex workers (FSWs) are one of the most-at-risk population groups for human immunodeficiency virus (HIV) infection. This paper aims at identifying the main predictors of HIV infection among FSW recruited in the 2nd Biological and Behavioral Surveillance Survey in 12 Brazilian cities in 2016.Method:Data were collected on 4245 FSW recruited by respondent driven sampling (RDS). Weights were inversely proportional to participants’ network sizes. To establish the correlates of HIV infection, we used logistic regression models taking into account the dependence of observations resultant from the recruitment chains. The analysis included socio-demographic sex work characteristics, sexual behavior, history of violence, alcohol and drug use, utilization of health services, and occurrence of other sexually transmitted infections (STIs).Results:HIV prevalence was estimated as 5.3% (4.4%–6.2%). The odds ratio (OR) of an HIV-positive recruiter choosing an HIV-positive participant was 3.9 times higher than that of an HIV-negative recruiter (P < .001). Regarding socio-demographic and sex work characteristics, low educational level, street as the main work venue, low price per sexual encounter, and longer exposure time as a sex worker were found to be associated with HIV infection, even after controlling for the homophily effect. The OR of being HIV infected among FSW who had been exposed to sexual violence at least once in a lifetime (OR = 1.5, P = .028) and the use of illicit drugs at least once a week were highly significant as well, particularly for frequent crack use (OR = 3.6, P < .001). Among the sexual behavior indicators, not using condoms in some circumstances were significantly associated with HIV infection (OR = 1.8, P = .016). Regarding the occurrence of other STI, the odds of being HIV infected was significantly higher among FSW with a reactive treponemal test for syphilis (OR = 4.6, P < .001).Conclusions:The main factors associated with HIV infection identified in our study characterize a specific type of street-based sex work in Brazil and provided valuable information for developing interventions. However, there is a further need of addressing social and contextual factors, including illicit drug use, violence, exploitation, as well as stigma and discrimination, which can influence sexual behavior.
Objective: To analyze changes in the lifestyles of Brazilian adolescents during the COVID-19 pandemic. Methods: Cross-sectional study carried out with adolescents who participated in the survey “ConVid Adolescentes – Pesquisa de Comportamentos”. The indicators related to lifestyles before and during the COVID-19 pandemic were evaluated: consumption of healthy and unhealthy foods, physical activity and sedentary behavior, smoking and consumption of alcohol. Prevalence and 95% confidence intervals were calculated for the total population and according to sex and age group. Results: A total of 9,470 adolescents participated in the study. During the period of social distancing, there was an increase in the prevalence of vegetables consumption (from 27.34 to 30.5%), frozen foods (from 13.26 to 17.3%), chocolates and sweets (from 48.58 to 52.51%), and time in front of screens (from 44.57 to 70.15%). On the other hand, there was a decrease in the practice of physical activity (from 28.70 to 15.74%) and in the consumption of alcohol (from 17.72 to 12.77%). Differences were observed according to sex and age group. Conclusion: The results show changes in the lifestyle of adolescents and an increase in health risk behaviors.
A ConVid - Pesquisa de Comportamentos foi realizada no Brasil de 24 de abril a 24 de maio de 2020, com o objetivo de investigar as mudanças nos estilos de vida e nas condições de saúde durante a pandemia de COVID-19. Neste artigo, apresentamos a concepção e metodologia da pesquisa. Estudo de corte transversal com a utilização de um questionário pela Internet, com questões validadas em inquéritos de saúde anteriores. O método de amostragem foi o “bola de neve virtual” e foram usados os procedimentos de pós-estratificação. Os resultados relativos às doenças crônicas não transmissíveis e estilos de vida pré-pandemia foram comparados às estimativas da Pesquisa Nacional de Saúde de 2013 e da Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico de 2019. A amostra total foi de 45.161 pessoas. Após a ponderação dos dados, as distribuições amostrais das variáveis demográficas foram semelhantes às populacionais. Apenas as pessoas de baixo nível de instrução foram sub-representadas. A comparação com os resultados anteriores mostrou similaridade na maioria das estimativas: consumo recomendado de frutas e legumes (22,1%), atividade física recomendada (35,2%), fumo de cigarros (12,3%), consumo frequente e abusivo de álcool (6,7%), obesidade (21,2%), prevalências autorreferidas de hipertensão (18,6%), diabetes (7,1%) e doença do coração (4,4%). O inquérito online possibilitou conhecer as condições de saúde da população durante a pandemia. A similaridade dos indicadores com os obtidos em pesquisas tradicionais permitiu validar as estimativas médias. Estudos são necessários para investigar como os efeitos endógenos das redes sociais virtuais podem ser levados em consideração na estimação da variância.
This cross-sectional study utilizes data from a nationwide web-based survey aimed to identify the factors affecting the emotional well-being of Brazilian adolescents aged 12–17 during the period of school closures and confinement. Data collection took place from 27 June to 17 September 2020. We used the “virtual snowball” sampling method, and students from private and public schools were included. A total of 9470 adolescents were analyzed. A hierarchical logistic regression model was used to find the factors associated with reporting at least two of three self-reported problems—sadness, irritability, and sleep problems. The main proximal factor was loneliness (AdjOR = 8.12 p < 0.001). Problems related to school closures also played an important role. Regular intake of fruits and vegetables, as well as physical activity, demonstrated a positive influence on emotional well-being, while excessive screen time (AdjOR = 2.05, p < 0.001) and alcohol consumption negatively affected outcomes (AdjOR = 1.73, p < 0.001). As for distal variables, less affluent adolescents were the most affected, and males reported fewer emotional problems than females. Uncertainty regarding the disease in a context of socioeconomic vulnerability, together with rises in unhealthy behaviors and isolation from their immediate social circles, have negatively affected adolescents’ emotional status throughout the COVID-19 pandemic.
Objective: To investigate the variation of anthropometric indicators from 2013 to 2019 and the factors associated with obesity in Brazil, using information from the National Health Survey. Methods: Cross-sectional study with cluster sampling and simple random sampling in the three stages. Measurements of weight and height among participants in 2013 (n=59,592) and in 2019 (n=6,672) were used. Differences in obesity prevalence were tested by Student’s t test for independent samples. To identify the sociodemographic factors and health problems associated with obesity, we used Poisson regression models with robust variance and crude and age-adjusted prevalence ratios to test the associations. Results: From 2013 to 2019, prevalence of obesity increased significantly, from 20.8 to 25.9%. Among men, the greatest increases were found in the 40-59 age group (9.1%) and in the median income category (8.3%). Among women, the greatest rises were found among those with low education (8.7%) and non-white ones (6.0%). For both males and females, factors associated with obesity were age, to live with a partner, level of instruction directly associated among men, and inversely associated among women. In 2019, for males, the crude and adjusted prevalence ratios were significant for high cholesterol, high blood pressure and at least one chronic non-communicable disease and, for females, for poor self-rated health, high blood pressure, diabetes, and at least one chronic non-communicable. Conclusion: It is necessary to implement intersectoral policies to promote changes in eating habits and encourage the practice of physical activity, taking into account economic, social, cultural, and environmental aspects.
Introdução: O câncer de mama, tendo em vista sua alta prevalência, sobretudo em sociedades cuja transição epidemiológica se completou, faz por merecer uma série de estudos de base populacional em prol de um melhor conhecimento da história natural da doença, do adoecimento e da mortalidade. Especula-se que a mortalidade possua diferentes padrões de acordo com a faixa etária. Objetivo: Avaliar a evolução da tendência de mortalidade por câncer de mama por grupos de faixa etária no Brasil entre 1980 e 2011. Método: Foi realizado um estudo descritivo, retrospectivo de série temporal, baseado em dados secundários coletados do Departamento de Informática do Sistema Único de Saúde. As faixas etárias foram estratificadas em dois grupos: ate 50 anos e maiores de 50 anos. Para a análise de tendência, utilizaram-se a regressão polinomial e o joinpoint. Resultados: Os resultados apontam para uma tendência de aumento na mortalidade por câncer de mama para mulheres com até 50 anos, e redução para as mulheres com mais de 50 anos. Conclusão: E necessária uma maior discussão a respeito do custo efetividade e validade da implantação da mamografia como opção de rastreamento para mulheres mais jovens no Brasil.
