These findings suggest that the intense VEGF expression in capsules around the polyurethane-coated silicone implant is able to improve the tissue vascularization, resulting in a softer capsule compared with the textured-surface silicone implant.
RESUMOObjetivo: verificar a prevalência de estreptococo do grupo B (EGB) em gestantes no terceiro trimestre da gravidez e explorar os fatores potencialmente associados à colonização. Métodos: uma amostra de 273 gestantes no terceiro trimestre da gravidez, provenientes do ambulatório de pré-natal do Hospital Universitário do Sul do Brasil, foi investigada. Culturas de amostra vaginal e anorretal foram obtidas e inoculadas em meio seletivo de Todd-Hewitt suplementado com 10 µg/ml de colistina e 15 µg/ml de ácido nalidíxico e posteriormente subcultivadas em ágar sangue de carneiro desfibrinado. Todas as colônias suspeitas foram submetidas ao teste de aglutinação para detecção do antígeno específico do grupo B. O teste de Camp foi utilizado para identificação do EGB das variedades não hemolíticas. Analisaram-se também os dados demográficos, socioeconômicos, reprodutivos e clínico-obstétricos. A razão de prevalência (RP) foi utilizada como medida de risco. Considerou-se como significante o intervalo de confiança no nível de 95% (α=0,05). Resultados: a prevalência de colonização pelo EGB foi de 21,6% (59), sendo que 9,9% (27) das gestantes tiveram positividade em ambos os sítios, 6,95% (19) foram positivas somente no sítio vaginal e 4,75% (13) da amostra tiveram positividade apenas no sítio anal. A prevalência de EGB foi ligeiramente mais alta nas gestantes com idade inferior a 20 anos, naquelas com menor escolaridade e nas gestantes primíparas, e o dobro entre aquelas que não relataram aborto espontâneo, porém sem significância estatística. Não foi encontrada diferença na prevalência de EGB de acordo com história de doenças sexualmente transmissíveis e tabagismo. Quando os dados foram analisados conjuntamente, os fatores detectados como potencialmente associados à colonização pelo EGB foram: primíparas com mais de 30 anos (RP=1,55) e mulheres com mais de um parceiro sexual e freqüência de atividade sexual aumentada (55,6 vs 20,5%; p<0,05). Conclusão: confirma-se a necessidade rotineira de cultura para EGB em ambos os sítios (vaginal e anal) de todas as gestantes no terceiro trimestre de gestação. The inflammatory alterations in the pre-operative period were mainly foveolar hyperplasia and chronic gastritis associated with H. pylori, and they were reduced in the post-operative period. A significant reduction of weight and BMI occurred, and a resolution of comorbidities was observed.
Breast augmentation with silicone implants is one of the most common procedures performed by plastic surgeons around the world. Capsular contracture is a frequent complication in breast augmentation and reconstructive surgery, that requires invasive intervention. The inflammatory response to implanted mammary prostheses appears to be directly associated to capsular contracture. This review discusses the evidences from rat models studies, on the role of inflammation and fibrosis in capsular contraction and its relation to silicone breast implants surface.
Purpose:In the search of a new material to repair duodenal wounds, a trial was conducted to assess the behavior of human amniotic membrane in the repair of the duodenal wall in rats. Methods: Fifty Wistar rats weighing between 250 and 350g, male, were submitted to duodenotomy and randomly distributed into two groups. Group A (n=8) had no treatment and was used as the control group. In Group B (n=42) the duodenal wound was treated with a patch of human amniotic membrane. Results: All animals in Group A died. In Group B no changes were observed with regards to death or the formation of duodenal fistula. All animals presented peritoneal adherences in the region on the duodenal wall repair and intestinal obstruction was observed in two animals. Healing of the duodenal wall in the region of the patch took place progressively as the post-operatory period increased, with regeneration of the mucosa and of the smooth muscle layer. Conclusion: From the clinical standpoint, the amniotic membrane proved to be a biological tissue which served as a temporary seal and allowed the wound to heal by second-intention, with re-establishment of the duodenal wall structure. RESUMOObjetivo: Na busca de um novo material para o reparo dos ferimentos duodenais, foi efetuado um estudo para avaliar o comportamento da membrana amniótica humana no reparo da parede duodenal em ratos. Métodos: Foram utilizados 50 ratos Wistar, com peso entre 250 e 350g, machos, distribuídos, aleatoriamente, em dois grupos. Grupo A (n=8), submetido à duodenotomia sem tratamento, utilizados como controle. Grupo B (n=42), submetido a um remendo de membrana amniótica humana para tratamento de ferimento duodenal provocado. Resultados: Todos os animais do grupo A foram a óbito. No grupo B não foram observadas alterações quanto a óbito ou formação de fístula duodenal. Observaram-se em todos os animais aderências peritoneais à região do reparo da parede do duodeno e obstrução intestinal em dois animais. O reparo da parede duodenal na área do remendo ocorreu de maneira progressiva com o aumento do período pós-operatório, com regeneração da mucosa e da camada muscular. Conclusão: A análise da membrana amniótica, do ponto de vista clínico, mostrou ser um tecido biológico que serviu para selar. Descritores: Âmnio. Duodeno. Curativos Biológicos. Ratos.
PURPOSE: Evaluating histologically the silicone peri-implant coated by polyurethane inflammation associated to the use of anti-microbial and bacterial contamination. METHODS: It was used 35 Wistar rats. The animals were divided in seven groups: I - Control; II - implant cavity contamination with10 bacteria/ml; III - implant cavity contamination with 10 bacteria/ml; IV - implant cavity contamination with 10 bacteria/ml; V - identical contamination to group II and implant immersions in anti-microbial solution; VI - identical contamination in group III and implant immersions in the anti-microbial solution; VII - identical contamination of group IV and implant immersions in anti-microbial solution. It was evaluated morphometrically the peri-implant capsules after 30 days of introduction. RESULTS: The factors with more discriminating power were the giants cells of a strange body and the mononuclear. There was no correlation between the bacterial concentrations and the histological alterations. CONCLUSION: 1) The histological standard of the inflammatory reaction around the silicone implant coated with polyurethan is chronic granulomatosis type of a strange body; 2) There isn´t correlation between concentration of Staphylococcus epidermidis and histological changes; 3) The use of anti-microbial solution decreased the mononuclear cell reactions, with the increase of giant cells in a strange body.
PURPOSE: To assess the tensile strength of polypropylene and polypropylene associated with polyglactin meshes (Vypro II® - Ethicon®, Somerville, NJ, USA) in a situation of partial separation of abdominal muscle aponeurosis on rats. METHODS: Thirty rats were used of the Wistar strain, which were randomized into two groups of 15 specimens each. In both groups an aponeurotic-muscle deformity was created on the abdominal wall measuring 3.0 x 1.0 cm, which was closed with polypropylene mesh (polypropylene group) or Vypro® mesh (vypro group). After 28 days the rats underwent euthanasia and an area was removed from the abdominal wall with which a strip was made measuring 2.0 cm in length and 6.0 cm in width comprising the abdominal muscles with the implanted mesh. This sample was placed in a mechanical test machine in which a constant force was applied contrary to the tissue strips. Maximum force expressed in Newton was considered until full rupture of the sample occurred. The non-parametric Kruskal - Wallis test was used for statistical analysis admitting p<0.05. RESULTS: Out of the thirty animals, there were two deaths in the vypro group and one unit in the polypropylene group was lost. One animal in the polypropylene group developed hernia during the study and another one developed granuloma of the abdominal wall. All animals in both groups developed epiplon adherence to the mesh. The average force was 48.08 N for the polypropylene group and 45.32 for the vypro® group. CONCLUSION: In these experimental conditions it could be observed that there is no statistically significant difference in the rupture force of the polypropylene and Vypro® meshes (p=0.54).
OBJECTIVESTo identify the most cited articles in general surgery published by Brazilian authors.INTRODUCTIONThere are several ways for the international community to recognize the quality of a scientific article. Although controversial, the most widely used and reliable methodology to identify the importance of an article is citation analysis.METHODSA search using the Institute for Scientific Information citation database (Science Citation Index Expanded) was performed to identify highly cited Brazilian papers published in twenty-six highly cited general surgery journals, selected based on their elevated impact factors, from 1970 to 2009. Further analysis was done on the 65 most-cited papers.RESULTSWe identified 1,713 Brazilian articles, from which nine papers emerged as classics (more than 100 citations received). For the Brazilian contributions, a total increase of about 21-fold was evident between 1970 and 2009. Although several topics were covered, articles covering trauma, oncology and organ transplantation were the most cited. The majority of classic studies were done with international cooperation.CONCLUSIONSThis study identified the most influential Brazilian articles published in internationally renowned general surgery journals.
