Objective:To evaluate intervening factors in patient safety, focusing on hospital nursing staff. Method: The study is descriptive, with qualitative approach, excerpt from a larger study with analytical nature. It was undertaken in a public hospital in Fortaleza, CE, Brazil, between January and June 2013, with semi-structured interviews to 70 nurses, using Thematic Content Analysis. Results: The principal intervening factors in patient safety related to hospital nursing staff were staff dimensioning and workload, professional qualification and training, team work, being contracted to the institution, turnover and lack of job security, and bad practice/disruptive behaviors. These aspects severely interfere with the establishment of a safety culture in the hospital analyzed. Conclusion: It is necessary for managers to invest in nursing staff, so that these workers may be valued as fundamental in the promotion of patient safety, making it possible to develop competences for taking decisions with focus on the improvement of quality care. Objetivou-se identificar as melhores evidências sobre estratégias de educação em saúde utilizadas para ensino-aprendizagem de pessoas com diabetes mellitus e pé em risco neuropático. Revisão integrativa realizada nas bases de dados PubMed, LILACS, CINAHL e SCOPUS em janeiro de 2015, com uma amostra de 14 artigos analisados na íntegra. Nos resultados, as evidências foram apresentadas em quadro-síntese e discutidas em categorias, abrangendo as diferentes estratégias de educação em saúde para prevenção/manejo do pé em risco neuropático (grupal; individual nas consultas ou por telefone; e utilizando tecnologias interativas) e a síntese das melhores evidências da efetividade destas intervenções na redução de complicações do pé diabético. Concluiu-se que todas as estratégias educativas são efetivas na promoção do autocuidado do pé diabético. Porém, as estratégias grupais mostraram maior eficácia, possibilitando melhora significativa nos conhecimentos, atitudes e práticas do cuidado com os pés e com a saúde, em geral, de pacientes diabéticos.
RESUMO: Objetivou-se descrever o cuidado de enfermagem prestado em serviço hospitalar de hemodiálise com base em metas internacionais de segurança do paciente. Estudo descritivo realizado no serviço de hemodiálise de hospital público brasileiro, referência no atendimento de pacientes renais agudos e crônicos, entre agosto e outubro de 2013. Participaram 25 profissionais de enfermagem. Os dados foram coletados com entrevista, observação e checklist baseados nas metas de segurança da Joint Commission International. Observou-se que as metas em conformidade foram: comunicação efetiva; redução do risco de infecções associadas aos cuidados de saúde; e redução do risco de lesões decorrentes de quedas. As demais metas, que são: identificação correta do paciente; segurança no manuseio de medicamentos de alta vigilância; e assegurar procedimento em local correto, merecem atenção por parte da equipe estudada e pela instituição avaliada. Constatou-se a necessidade de implementação de protocolos no serviço para a segurança do paciente e da equipe. A descriptive study was conducted at the hemodialysis service of a Brazilian public hospital, reference in care for acute and chronic kidney patients, between August and October, 2013. Twenty-five nursing professionals participated in the study. Data were collected through interviews, observation and a checklist based on the safety goals of the Joint Commission International. The goals found to be in conformity were: improving effective communication; reducing the risk of healthcare-associated infections; and reducing the risk of patient harm resulting from falls. The remaining goals, namely: identifying patients correctly; improving the safety of high-alert medications; and ensuring safe surgery, deserve attention from the researched team and the assessed institution. Dimensioning the instrumentation: Exploratory or confi rmatory factor analysis?Re: Quality of nursing care: Instrument development and validation Dear Editor, Martinez et al. in their research entitled as "Quality of nursing care: Instrument development and validation", published in volume 69 and issue 5 in 2016, have done a valuable work in relation to the design and psychometry of instrument as well as the relevant areas (Nurses' perceptions of activities leading to increased quality of care by nurses) (1) . There are some tips in some parts of the research methodology that need to be considered. Explaining them seems to be capable of improving the work in future research and to be a guidance for other researchers in the field of design and validity of the instrument. Conceptualization and generating items in this study were performed with a deductive approach and by using OE -Ordem dos Enfermeiros (Order of Nurses) standard related to Portugal standards. Sampling in large sample size and focusing on the validity and reliability of the provided tool are as the strengths of the mentioned study.Papers appraisal is essential for the growth of knowledge and improves the quality of scientific research (2) . Scientific review of articles can be useful for readers and researchers regarding the increase of the article quality (3) . The mentioned study is criticized from two major aspects: In evaluating the newly designed instrument validity, only the qualitative content analysis (intelligibility, clarity and communication) has been mentioned. Although this evaluation is proper and accurate, but it is not enough, and CVI or CVR have not been calculated (4)(5) , while focusing on these indices is crucial in instrumentation and represents the issue that whether the instrument content fully assesses the studied structure or not (5) . The important, and in fact, the main thing to notice is that followed by sampling, the exploratory factor analysis method was used to assess the construct validity, while the dimensions have been specified in the beginning of the study and it is even determined that which items would be placed in which dimensions. In this context, the factor analysis of choice will be of confirmatory type and not the exploratory kind (5)(6)(7) . Since, the confirmatory type is the test hypothesis, suggesting that whether the statements included within each dimension are really fit and belong to that dimension or not (4) . The exploratory type, as the name implies, involves the detection and determining the dimensions. Sometimes, the texts can be used to state the dimensions of a structure. However, to specify which statements need to be placed in the made instrument, the exploratory factor analysis prediction should be employed in such circumstances.The purpose of the aforementioned is helping the illustration of key points in the design of the instrument and mentioning of some necessary points.
