Background: Intertrochanteric fracture is common injury and is found in both elderly and the young. Proximal femoral nail (PFN) has been in widespread use for the same in the recent past. There are advocates both for and against this implant. The aim was to study the results of PFN in the treatment of intertrochanteric fractures and their functional evaluation using the modified Harris hip score.

Methods: A prospective study done on 104 patients operated for intertrochanteric fractures with PFN. Patients were followed up at 6 weekly intervals and evaluated at each visit using the modified Harris hip score (HHS).

Results: The study consisted of 104 patients aged from 36 to 96 years with average age of 67.8 years. Most patients belonged to 60-80 years category. Malunion was observed in three patients (2.8%) and non union was not seen in any patient. Excellent to good results were seen in 73% patients 18% patients had a fair outcome, 7.7% had poor and 2% patients had very poor outcome.

Conclusions: PFN is a good implant for intertrochanteric fracture treatment that demands a sound technique and a good reduction prior to insertion. The assessment of results using HHS has been too similar to those obtained using Kyle’s criteria in the previous studies, proving its role in the same.

In this paper, electronic dispersion compensation (EDC) based on minimum mean square error optimization has been employed to improve the performance of 16 channels, gigabit capacity, back haul DWDM OADM ring network.. It is observed that EDC significantly reduce BER by e-33 and improved Q2 dB by 3 dB, thus resulting in improved eye opening and considerably reduced inter-symbol interference.
In this paper, Decision Feedback Equalization (DFE) and Feed forward Equalization (FFE)based on minimum mean square error optimization has been employed to improve the performance of 32 channels, gigabit capacity WDM-PON network. An optical line terminal (OLT) installed at the transmitter end which continuous transmit 32 channels having different colors (wavelengths) of 20GB/s data rate , which is multiplexed by optical multiplexer and transmit through standard single mode fiber (SMF) having length 40km. A link design for 1:64 optical splitters is used as Passive Optical Network (PON) element which creates communication between OLT to different end users. It is observed that FFE and DFE significantly reduce BER by 1.0798e -010 , 1.1556e -011 , 1.2960e -010 and 8.8979e -009 at four different wavelengths 1550nm, 1559.6nm, 1562.8nm and 1574.8nm by using NRZ (non return zero) modulation format at 20GB/s for 64 users having 40km fiber length respectively. Sharp increase in BER 2.6916e -002 , 4.6519e -002 , 3.8804e -001 and 5.1570e -002 occurs if data rate increases up to 25 GB/s.
Background: Fracture of calcaneum accounts for about 2% of all fractures and 75% of all calcaneum fractures are intra-articular. Numerous controversies existed regarding optimal treatment of displaced intraarticular calcaneum fractures. In this study evaluate post-operative functional outcome and complication of fracture calcaneum treated with locking calcaneum plate. Methods: This hospital based prospective descriptive study was conducted on 108 patients (120 calcaneum fracture) operated between July 2016 to December 2018 by open reduction and internal fixation with locking calcaneum plate through extensive lateral approach at Department of Orthopaedics, SMS Medical college and hospital, Jaipur. All close displaced intraarticular calcaneal fracture was included in the study. Results: Average duration between injury and surgery was 8.3±2.97 days. Out of 120 calcaneum fracture 52 fractures (43.33%) were Sander's type II, 52 fractures (43.33%) were Sander's type III, and 16 fractures (13.33%) were Sander's type IV. Pain on weight bearing was noted in 20 patients (16.66%) implant prominence was noted in 8 patients and delayed wound healing was seen in 4 patients. Maryland foot score was excellent in 44 fracture (36.67%), good in 56 fractures (46.67%), fair in 8 fractures (6.67%), and poor in 12 fractures (10%). Conclusions: Open reduction and internal fixation (ORIF) with locking calcaneum plate in an indicated case, with respect to soft tissue envelope and early rehabilitation, leads to better therapeutic results as compared to other operative technique.
Dispersion in an optical fiber is one of the major limitations which affect its data carrying capacity and causes overall performance degradation. In this paper, Electronic Equalization Technique (EET) has been employed in a multiuser Optical Code Division Multiple Access (OCDMA) network to compensate the chromatic dispersion. It has been observed that in the proposed network the Bit Error Rate (BER) is significantly reduced by e -16 and Q 2 db factor reported is 19dB with EET.
In this paper, we discussed about the imperfect items.
In practice items may get damaged due to production or
transportation conditions. Each lot receives some imperfect items.
This model also considers the effects of business strategies such as
optimal order size of raw materials, production rates and unit
production costs, and idle time in different areas on the
cooperation of marketing systems. The model can be used in
industries such as textiles and footwear, chemicals, food. We
develop an inventory model based on imperfect products and
shortages. We consider demand is constant and continuous.
Purpose of this study is not only to find the retailer`s optimal
replenishment policies but also to minimize the total average cost.
Finally, a numerical example is presented to illustrate the
proposed model and sensitivity analysis of the optimal solution
concerning parameters is carried out using the Mathematica 10.0
software.
