El tetanos ocupa un lugar especial entre las cnfermedades infecciosas. No es una enfermedad contagiosa en el sentido estricto de la palabra . Es el resultado del contacto del hombre con su medio ambiente y la incideneia dependera de la ecologia de la comunidad. Las esporas del Clostridium tetani, pueden sobrevivir en la tierra durante meses o anos, siempre que no esten expuestas a la luz solar. Se introducen en la herida, que es habitualmente leve, como esporas apareciendo formas vegetativas al reducirse el potencial local de oxido reduction. Estas formas vegetativas producen una exotoxina, la tetanospasmina, que es responsable de las manifestaciones neurologicas .Esta toxina tiene poder de fijacion en SNC, teniendo esto importancia en la terapeutica, ya que en el LCR los niveles de antitoxina tetanica son 400 veces menores que en sangre . En la literatura se describen diversas tecnicas para aumentar la concentration de la antitoxina en LCR . Es asi que, a partir de experiencias de Speransky, se ha empleado el metodo de bombe o de liquido cefalo raquideo. En nuestro pais se empleo con exito, habiendose abandonado su practica actualmente .La colocacion de antitoxina en LCR producia efectos indeseables, es por esto que recientemente se la ha administrado asociada a corticoides, describiendose un descenso de la mortalidad en tetanos de recien nacidos desde 90% a 10% 10 .Tambien se ha ensayado, con buen resultado, la administration de gama globulina hiperinmune int ratecal, adicionada de corticoides 8 .Anualmente fallecen cerca de 500.000 personas por tetanos en el mundo 3 .
