El tetanos ocupa un lugar especial entre las cnfermedades infecciosas. No es una enfermedad contagiosa en el sentido estricto de la palabra . Es el resultado del contacto del hombre con su medio ambiente y la incideneia dependera de la ecologia de la comunidad. Las esporas del Clostridium tetani, pueden sobrevivir en la tierra durante meses o anos, siempre que no esten expuestas a la luz solar. Se introducen en la herida, que es habitualmente leve, como esporas apareciendo formas vegetativas al reducirse el potencial local de oxido reduction. Estas formas vegetativas producen una exotoxina, la tetanospasmina, que es responsable de las manifestaciones neurologicas .Esta toxina tiene poder de fijacion en SNC, teniendo esto importancia en la terapeutica, ya que en el LCR los niveles de antitoxina tetanica son 400 veces menores que en sangre . En la literatura se describen diversas tecnicas para aumentar la concentration de la antitoxina en LCR . Es asi que, a partir de experiencias de Speransky, se ha empleado el metodo de bombe o de liquido cefalo raquideo. En nuestro pais se empleo con exito, habiendose abandonado su practica actualmente .La colocacion de antitoxina en LCR producia efectos indeseables, es por esto que recientemente se la ha administrado asociada a corticoides, describiendose un descenso de la mortalidad en tetanos de recien nacidos desde 90% a 10% 10 .Tambien se ha ensayado, con buen resultado, la administration de gama globulina hiperinmune int ratecal, adicionada de corticoides 8 .Anualmente fallecen cerca de 500.000 personas por tetanos en el mundo 3 . La Imipramina sintetizada, en 1948, por Hafliger 4 , fue resultado de la busqueda de sustancias que tuvieran propiedades sedantes, analgesicas o antiparkinsonianas. Una de estas fue la fmipramina, un compuesto de tipo Benzazepina y que difiere de las fenotiazinas por reemplazo del sulluro por un enlace etileno. Sorprestvamente, al ensayarse (Kuhn 1958) 4 mostro tener actividad antidepresiva. Hoy, profusamente usada en adultos en ciertas formas de depresion y especialmente en nirios en tratamiento de enuresis.Absorcwn. La Imipramina se absorbe en el tubo gastrointestinal, pasa a la sangre, donde se une a proteinas plasmaticas, y se deposita rapidamente en los tejidos.Una inyeccion de 100 mg por via intramuscular produce niveles plasmaticos moderados y por lapso breve de 30 minutos lb .Distnbucwn. Se concentra especialmente en cerebro, corazon y pulmones. Hay referencias que senalan que las concentraciones en tejidos son del orden de 10 a 30 veces mayores que las del plasma '.Metaboiismo. A nivel hepatico, la droga se glucoroniza, demetila y se hidroxila 1 ' . Este fenomeno es rapido, y en dosis terapeuticas a 'Hospital Luis Calvo Mackenna.las 24 horas no hay droga detectable en los tejidos 13 .Eliminacion. La eliminacion por la orina es escasa* Experiencias en animales (conejo) con dosis de 50 mg por kg y forzando diuresis con Manitol, solo se elimina por orina el 2 % de lo administrado. En el hombre, la excresion renal, aun forzando la diuresis, no llega mas alia del 5%.Hay informes que la Imipramina se excretaria por la mucosa gastrica. Una comunicacion verbal de E.W. Flint, en 1967, sugiere tal posibilidad . Esto motive en casos de intoxicacion, el uso del lavado gastrico continue con aspiracion posterior.Los resultados fueron desalentadores. Un nino ingiere 425 mg de Imipramina, le efectuan lavado gastrico y aspiracion durante 24 horas y solo recuperan 13,4 mg, o sea, el 3,2% de lo ingerido . En otro paciente intoxicado realizan lavado gastrico durante 5 1/2 horas, y re-*Para reconocer Imipramina en orina se ocupa: -Solucion de Dicromato depotasio {K2 Cr2 07), 3,7 grs. --Acido sulfurico concentrado, 280 ml. -Agua destilada, 500 ml.
