Parole chiave: PML, encefalopatia da HIV, AIDS RIASSUNTO -La leucoencefalopatia multifocale progressiva (PML) e una malattia caratterizzata da demielinizzazione progressiva della sostanza bianca cerebrale; essa rientra nel gruppo dell'encefalopatie sostenute da virus lenti a DNA ed e secondaria ad infezione con papovavirus nei soggetti immunodepressi.Da una casistica personale di 351 pazienti affetti da sieropositivita per l'HIV con o senza AIDS, presentanti sintomatologia neurologica ejo neuropsicologica, sono stati selezionati 10 pazienti affetti da PML (percentuale del 3,5% sul totale dei casi e del 18,9% sui casi con AIDS conclamato).In 4 pazienti il tipo di reperti riconduceva ad una forma diffusa con esordio peritrigonale ed estensione controlaterale attraverso il corpo calloso, anteriore e caudale lungo le vie bianche.In 4 pazienti l'esordio era regionale lobare o sublobare (generalmente parietale), con comparsa sincrona o metacrona di altre lesioni in sedi distanti.In 2 pazienti le lesioni erano di medio numero, di medie e piccole dimensioni, a configurazione ovalare o rotonda.In RM le aree colpite, generalmente asimmetriche, hanno forme e dimensioni variabili da piccole aree ovalari a zone piu' estese a localizzazione lobare o translobare, fino alle forme diffuse con passaggio interemisferico. Le lesioni, senza effetto massa, sono dotate di lungo T1 e T2, con segnale ipointenso nelle sequenze a breve TR e iperintenso nelle sequenze a lungo TR, in rapporto all'estensione della necrosi indotta dai papovavirus. L'impregnazione marginale delle lesioni dopo somministrazione di mdc e rarissima. In pazienti non AIDS questi aspetti sono suggestivi per la diagnosi di PML, mentre nei pazienti AIDS la diagnosi differenziale con l'encefalopatia da HIV pone notevoli problemi per la similarita di aspetti neuroradiologici. SUMMARY -Progressive multifocal leukoencephalopathy ( PM L ) is characterized by the progressive demyelination of the white cerebral matter. It belongs to the group of encephalopathies that are caused by slow DNA viruses and is secondary to papovavirus infection in immunodepressed patients.This rare pathology can be studied by MRI of HIV positive patients. The increasing number of fully blown AIDS cases is allowing a better understanding of the existing anatomopathological correlations and images. 10 PML patients (3 .5% of all cases and 18.9% of fully blown AIDS cases) were selected from a personal case series including 351 HIV positive patients with and without AIDS, showing neurological andjor neuropsychological symptoms. In four patients the findings referred to a diffuse form with peritrigonal origin, contralateral extension through the callosum, anterior and caudal extension along the white matter connections.Four patients showed a regional, either lobar or sub lobar, origin (usually parietal) with synchronic or metachronic onset of other lesions in distant locations. 411
IntroduzioneI linfomi primitivi del SNC costituiscono un capitolo relativamente recente della neuro-oncologia, essendo stati riconosciuti come tali non prima degli anni '70. Identificata la vera natura di queste neoplasie e confermata l'equivalenza anche classificativa con le piu comuni forme nodali ed extranodali, sono stati evidenziati tuttavia aspetti peculiari che ne giustificano un approccio separato.I linfomi sono tumori del cosiddetto «sistema linfatico», ossia neoplasie primitive degli organi linfatici e del tessuto linforeticolare in genere.Essi fanno parte, insieme a leucemie linfatiche e sindromi immunoproliferative, del piu ampio gruppo delle malattie linfoproliferative, condizioni morbose derivanti dalla trasformazione neoplastica di un clone linfocitario.A seconda infatti dello stadio di differenziazione raggiunto dagli elementi proliferanti potremo avere un linfoma, una leucemia linfatica o una sindrome immunoproliferativa, con possibilita di conversione di una forma nell'altra. E utile precisare che la diagnosi di neoplasia primitiva e stata posta dopo aver ragionevolmente escluso la presenza di focolai linfomatosi in altra sed e.La frequenza di un coinvolgimento di sedi esterne al SNC all'esame autoptico, insieme alla impossibilita di rilevare l'ordine cronologico di interessamento delle varie sedi, autorizzano a non considerare come determinante la demarcazione dei linfomi in forme primitive e secondarie.Tradizionalmente distinguiamo fra il linfoma di Hodgkin e la lunga serie dei linfomi non-Hodgkin. Linfomi Non-HodgkinConsiderati complessivamente, i linfomi nonHodgkin rappresentano il 3-5% di tutti i tumori; prediligono inoltre il sesso maschile e l'eta adulta e avanzata. La localizzazione abituale e rappresentata dai linfonodi, tuttavia un numero non indifferente di casi deriva da sedi extranodali, alcune delle quali, tipico l'esempio del SNC, normalmente prive di tessuto linfatico. IncidenzaSi tratta di neoplasie alquanto rare, rappresentando lo 0,7-2 di tutti i linfomi e lo 0,2-2% dei tumori del SNC. I pazienti affetti da questa patologia hanno generalmente un'eta compresa tra i 30 ed i 70 anni. E rilevabile un picco di incidenza nel 6 decennio.Eccezionali sono i casi in eta pediatrica. Il sesso maschile risulta maggiormente colpito rispetto a quello femminile, con un rapporto approssimativo di 3:2. U n'incidenza particolarmente elevata e riscontrabile tra soggetti immunodepressi e, piu in generale, in condizioni patologiche che comportino comunque un disordine del sistema immunitano.Per quanto concerne la categoria dei soggetti immunodepressi, relativamente alle forme congenite di immunodeficienza, risultano ben documentate le associazioni con la sindrome di Wiskott-Aldrich, la sindrome di Chediak-Higashi, l'atassia-teleangectasia e l'immunodeficienza combinata severa 1 · 2 ; tra le forme acquisite, decisamente piu frequenti, figurano 1' AIDS e la immunodepressione dei trapiantati d'organo, causata dalla terapia antirigetto. 117
RIASSUNTO-Dopo una revisione delle possibilita diagnostiche di CT ed RM nell'ambito delle neoplasie adenoipofisarie, gli autori presentano i risultati della espsrienza personale con RM a medio campo e con l'uso di Gd-DTPA. Viene valutata l'opportunita di scelta di ognuno dei parametri in gioco nella diagnostica dei tumori ipofisari con particolare riguardo ai microprolattinomi. Particolare attenzione viene dedicata all'uso del Gd-DTPA, alia scelta del dosaggio piu opportuno da usare, ai tempi tra iniezione ed esecuzione delle scansioni. Dall'analisi delle caratteristiche anatomofunzionali ed istologiche emergono dati suggestivi per l'interpretazione dei variabili comportamenti esibiti dai microprolattinomi della nostra casistica, anche dopo terapia medica a base di Bromocriptina. In tale chiave inoltre vengono ricercate le ragioni che rendono i tumori prolattinosecernenti meglio indagabili con le tecniche di Imaging. Infine, viene proposto un protocollo di indagine mostratosi utile nella diagnostica delle neoplasie della regione sellare.
