Psoriasis is a chronic, persistent skin disease characterized by characteristic lesions in the form of well-defined erythematous patches covered by thick white, shiny scales resembling wax droplets. Psoriasis has a psychological, social, and emotional impact. Psychological stress has also been shown to influence the disease and course of psoriasis. Therapy in psoriasis patients with psychological stress can be given pharmacologically and psychologically. Patients with moderate to severe psoriasis using the Goeckerman regimen for psoriasis showed significant improvements in anxiety and depression scores. Psychological therapies such as relaxation therapy and meditation are able to control the emotions that trigger stress and reduce the appearance and severity of psoriasis. This literature review aims to describe the impact of psychological stress on psoriasis patients so as to increase awareness to detect psychological stress and provide appropriate treatment.
Background: Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae. This disease has a high transmission ability and can cause physical disability and have an impact on the social life of the sufferer because of the negative stigma about leprosy. This study aims to describe the examinations performed in the diagnosis and treatment of patients with multibacillary leprosy, borderline lepromatous type. Case presentation: A 43-year-old woman with a chief complaint of redness on the arm since 1 year ago. The patches are not itchy, painless, stiff, and numb. Since 3 months ago, the patient complained of red patches increasing and spreading to the trunk and legs. Dermatological examination found in the facial region et truncus anterior et posterior et superior, and inferior extremities bilateral showing multiple erythematous plaques, well-demarcated with scales in several parts. Sensory function examination revealed a decrease in lesions on the face, superior and inferior extremities, and anterior and posterior trunks. Negative AFB examination and biopsy results support the diagnosis of borderline lepromatous type (BL) multibacillary leprosy (MB). Conclusion: Clinical findings in the form of hypopigmented lesions that feel numb is a cardinal signs of leprosy. Asymmetrical distribution of lesions with a number of more than 5 lesions accompanied by impaired nerve function in the form of decreased sensibility is a characteristic of MB leprosy. Histopathological features support the diagnosis of MB type BL leprosy even though the results of the skin slit smear examination were negative.
Background: Urticaria is a skin disorder due to the release of mast cell mediators, which is characterized by raised skin (urtica) accompanied by itching. Skin prick test (SPT) is a rapid, sensitive test for detecting IgE-mediated allergic disease. This study aimed to determine the profile of SPT in urticaria patients. Methods: A descriptive observational study involving 67 research subjects retrospectively using medical record data for the period January 2017-June 2022. Univariate data analysis was carried out in this study with the help of SPSS version 25 software. Results: A total of 67 urticarial patients with positive results on the SPT investigation found that there were more women (71.64%) than men (28.36%), with the most age group being 16-25 years old with 16 patients, followed by 13 patients aged 56-65 years, 12 patients aged 46-55 years, 10 patients aged 36-45 years, 9 years of 26-35 years, 5 patients aged ≥ 65 years and the last age group, namely ≤ 15 years of age in 2 patients. Distribution of urticaria with the most locations in the generalized region, with as many as 20 patients (29.85%), followed by the extremity region with as many as 18 patients (26.87%), the facial region with as many as 17 patients (25.37%) and the least location in the trunk region as many as 12 patients (17.91%). Allergen tests that caused the most were house dust in 21 patients (11.86%), crabs in 18 patients (10.17%), and egg yolk in 11 patients (6.21%). Conclusion: Skin prick test profile at the dermatology and venereology polyclinic, Dr. Moewardi General Hospital, Surakarta, Indonesia, for the period January 2017-June 2022, where the results obtained were that there were more women than men, the largest age group was 16-25 years, the most common distribution of urticaria locations was found in the generalized region and the most common allergens caused by house dust.
Latar belakang: Moluskum kontagiosum merupakan penyakit pada kulit yang disebabkan oleh infeksi poxvirus. Penularan penyakit ini dapat terjadi melalui kontak kulit dan membran mukosa secara langsung, termasuk melalui hubungan seksual, maupun melalui benda yang terkontaminasi. Prevalensi moluskum kontagiosum yang ditransmisikan secara seksual meningkat dalam beberapa tahun terakhir.Penyakit ini menimbulkan kecemasan dan rasa tidak nyaman bagi penderita. Diagnosis moluskum kontagiosum didasarkan pada hasil anamnesis, pemeriksaan fisik, pemeriksaan dermoskopi, dan pemeriksaan histopatologi. Penegakan diagnosis dan tata laksana yang tepat dapat membantu menurunkan risiko kekambuhan pada masa yang akan datang. Kasus:Seorang perempuan 26 tahun dengan keluhan utama timbul bintil-bintil berwarna putih mengkilat pada paha dan bokong yang tidak terasa gatal ataupun nyeri sejak 3 bulan yang lalu. Awalnya muncul 4 buah bintil di bagian bokong saja, namun sejak 1 bulan terakhir bintil bertambah banyak dan muncul juga di bagian paha. Pasien pernah berusaha memencet bintil tersebut dan keluar padatan berwarna putih seperti nasi. Pasien memiliki riwayat berhubungan seksual tanpa kondom sebelumnya. Dari hasil pemeriksaan dermatologis didapatkan pada regio femoralis dextra et sinistra serta gluteus tampak papul multipel berwarna putih mengkilat seperti mutiara, berukuran 0,1-0,5 cm, permukaan halus, berbatas tegas, berbentuk seperti kubah dengan cekungan di tengahnya (delle), dengan sebaran diskret. Pada pemeriksaan dermoskopi tampak gambaran poli lobular berwarna putih hingga kuning disertai crown vessel. Pemeriksaan histopatologi menunjukkan gambaran badan inklusi eosinofil intrasitoplasma (molluscum bodies).Diskusi: Adanya temuan klinis berupa papul multipel dengan sebaran diskret, berwarna seperti mutiara berbentuk kubah dengan lekukan di tengahnya (delle) merupakan tanda khas pada kasus moluskum kontagiosum. Hasil pemeriksaan dermoskopi serta gambaran histopatologi mendukung diagnosis ini. Tata laksana dilakukan dengan tindakan kuretase pada lesi serta pemberian terapi cimetidine oral 2x800 mg selama 2 bulan. Tidak terjadi kekambuhan dalam periode follow-up selama 5 bulan.
