BackgroundPlacental soluble fms-like tyrosine kinase-1 (sFlt-1) which is an antagonist of vascular endothelial growth factor and placental growth factor (PIGF), is considered as one of etiology factors cause endothelial damage in preeclampsia due to increase of sFlt-1 level that change vascular endothelial integrity. This study aims to analyze the difference of sFlt-1 and PlGF concentration in severe preeclampsia and normal pregnancy, and the correlation between both in occurrence of severe preeclampsia.MethodThis is case control study involving 18 subjects with severe preeclampsia and 19 subjects with normal pregnancy as controls who met inclusion and exclusion criteria. Concentration of sFlt-1 and PlGF are measured with ELISA. Statistical analysis is performed with Chi square test, Fisher’s exact test, T test, Mann–Whitney test, and Spearman’s rank correlation test.ResultsThis study results in no significant difference in characteristics of gestational age, and parity in both study groups. Median concentration of sFlt-1 in severe preeclampsia is higher (20,524.75 pg/mL) compared with normal pregnancy (6820.4 pg/mL). Concentration of PlGF is lower in severe preeclampsia (47 pg/mL) compared with normal pregnancy (337 pg/mL). sFlt-1 concentration is higher in severe preeclampsia compared to normal pregnancy. PlGF concentration is lower in severe preeclampsia compared to normal pregnancy. Ratio of sFlt-1 and PlGF concentration is significantly correlated in both severe preeclampsia and normal pregnancy.ConclusionsThere is a significant negative correlation between the concentration of sFLt-1 and PlGF in normal pregnancy.
Objective
Spontaneous (without instrumentation) depressed skull fracture cases are rare in newborns and, in most cases, are caused by trauma during delivery. The diagnosis of a skull fracture can sometimes be difficult and the correct management of these fractures in newborns is still uncertain.
Case presentation
Two cases of spontaneous (without instrumentation) depressed skull fracture in newborns are reported and were related to birth trauma. Each cases were managed differently according to its clinical presentation.
Conclusion
Spontaneous depressed skull fracture of the newborn presents as a challenge in neurosurgical management. Although widely accepted that conservative management is a treatment of choice, but few cases with intractable seizures are also indication for surgical intervention. Our case series is a good educational lesson that can also be used by pediatrician and obstetrician for better management and handling of birth head trauma.
Saat ini di negara berkembang, jutaan wanita usia subur (WUS) dengan HIV positif tidak menggunakan kontrasepsi untuk menunda atau mengakhiri kehamilan padahal pencegahan kehamilan yang tidak diinginkan pada WUS dengan HIV positif sangat penting untuk meningkatkan kualitas hidup ibu dan anak, dan mencegah penularan HIV dari ibu ke anak. Tujuan penelitian ini adalah untuk menganalisis faktor-faktor apa saja yang berpengaruh terhadap unmet need pada WUS dengan status HIV positif. Metode penelitian yang digunakan adalah cross sectional dengan pengambilan sampel secara consecutive sampling. Penelitian dilakukan di Klinik Mawar Kota Bandung pada bulan Maret-Juni 2015. Sampel berjumlah 130 orang WUS dengan HIV positif yang datang ke Klinik Mawar. Data melalui wawancara dengan menggunakan kuesioner terstruktur yang berisi data karakteristik responden, dukungan suami, pengetahuan tentang HIV dan KB. Data dianalisis dengan uji chi kuadrat dan regresi logistik ganda untuk menganalisis faktor-faktor yang berpengaruh terhadap unmet need. Hasil dinyatakan dalam odds ratio dengan interval kepercayaan 95%. Hasil penelitian menunjukkan bahwa keinginan mempunyai anak dengan POR= 2,67 (IK 95%:1,034−6,891), dukungan suami dengan POR= 7,803 (IK 95%: 2,037−29,884) berpengaruh terhadap unmet need dan status HIV suami dengan POR= 0,168 (IK 95%:0,064−0,44) berpengaruh lebih rendah untuk terjadi unmet need. Dukungan suami merupakan faktor yang paling berpengaruh terhadap unmet need dimana WUS dengan HIV positif yang tidak mendapat dukungan suami 7,8 kali lebih besar terjadi unmet need dibandingkan WUS yang mendapat dukungan suami. Simpulan penelitian ini adalah keinginan mempunyai anak, dukungan suami, dan status HIV suami berpengaruh terhadap unmet need pada WUS dengan HIV positif. Peran suami merupakan faktor penting pada unmet need sehingga untuk menurunkan angka unmet need pada WUS dengan HIV positif perlu disarankan untuk dilakukan konseling KB berpasangan (partner counseling).
Defisiensi zat besi merupakan penyebab utama anemia di negara berkembang. Kadar hemoglobin darah digunakan sebagai penanda anemia defisiensi besi. Indonesia kaya bahan makanan tinggi zat besi namun belum diketahui kemanfaatannya seperti daun salam (Syzygium polyanthum (Wight) Walp). Tujuan penelitian ialah mengetahui peningkatan kadar hemoglobin pada tikus model anemia defisiensi besi sesudah pemberian ekstrak daun salam. Penelitian eksperimental laboratorium dengan rancang acak lengkap dilakukan di Laboratorium Farmakologi dan Terapi Fakultas Kedokteran Unpad periode 4 Mei-19 Juni 2018. Sebanyak 20 ekor tikus Wistar betina berumur 7 minggu diinduksi menggunakan alumunium sulfat 67,5 mg/kg BB secara intramuskuler selama 7 hari, dan dibagi menjadi 5 kelompok perlakuan, yaitu K1 (tanpa perlakuan) K2 (tablet tambah darah 5,4 mg), P1 (ekstrak daun salam 2,2 mg), P2 (ekstrak daun salam 4,4 mg), P3 (ekstrak daun salam 6,6 mg). Kadar hemoglobin diukur menggunakan auto hematology analyzer sysmex. Uji LSD menunjukkan rerata kadar hemoglobin setelah perlakuan pada kelompok perlakuan berbeda signifikan dibanding dengan kelompok kontrol negatif (p<0,05) dan tidak berbeda signifikan dibanding dengan kelompok kontrol positif (p>0,05). Analisis uji median menunjukkan peningkatan kadar hemoglobin berbeda signifikan pada K2, P1, P2, dan P3 (p<0,05). Disimpulkan, ekstrak daun salam meningkatkan kadar hemoglobin pada tikus model anemia defisiensi besi.
