We retrospectively reviewed the records of 100 patients with malignant disease and symptomatic pericardial effusion initially treated with pericardiocentesis at the National Cancer Institute of Mexico between 1985 and 2009. We analyzed predictive factors for recurrence of pericardial effusion by bi- and multivariate analyses. The group comprised 74 women and 26 men. Twenty patients had developed malignant pericardial effusion at the time of primary cancer diagnosis. Recurrence rate after pericardiocentesis was 33%. Progression-free survival for pericardial effusion at one year was 59% (range, 47-71%). Median overall survival (OS) after pericardiocentesis was 40.3+/-7.9 weeks (95% Confidence interval, 24.9-55.7). The sole factor that correlated with increased progression-free survival for pericardial effusion was the presence of bloody pericardial effusion. For OS, multivariate analysis yielded that female gender and presence of pericardial effusion at time of primary malignancy diagnosis were associated with higher life expectancy. Initial pericardiocentesis can provide successful management of patients with a control rate of 67%. In spite of the high effectiveness of the primary management of pericardial effusion with pericardiocentesis in oncologic patients, it might be considered for initial treatment, especially those with poor prognosis, leaving pericardial window as a secondary strategy for recurrence.
The epithelial ovarian cancer (EOC) has been underdiagnosed because it does not have a specific clinical presentation, and the signs and symptoms are similar to the irritable bowel syndrome and pelvic inflammatory disease. EOC is less common than breast and cervical cancer, but it is more lethal. On the whole, EOC has an early dissemination to peritoneal cavity, which delays a timely diagnosis and increases the rate of advanced diagnosed disease. The diagnosis usually surprises the women and the primary care physician. Therefore, it is necessary to count on prevention and early diagnosis programs. EOC has 80% response to surgical treatment, but nearly 70% of the patients may relapse in five years. El estado de choque es la expresión clínica de insuficiencia circulatoria, que se traduce en el inadecuado aporte de oxígeno a nivel tisular, siendo una condición común en pacientes críticamente enfermos. El propósito del uso de vasopresores es restaurar la perfusión tisular mediante el incremento de la presión arterial media (PAM), mejorando con esto el flujo sanguíneo y el aporte de oxígeno a los tejidos periféricos. En general, en pacientes con choque circulatorio el mantener una PAM de 65-75 mmHg puede ser suficiente para asegurar una buena presión de perfusión tisular, sin embargo, en pacientes con antecedente de hipertensión arterial crónica se recomienda mantener la PAM entre 75 y 85 mmHg, y en el manejo de la hipertensión endocraneana se puede requerir una PAM más alta que mantenga una adecuada presión de perfusión cerebral. El tratamiento con agentes vasopresores se debe individualizar en cada paciente de acuerdo con su patología. El propósito de esta revisión es describir el mecanismo de acción, dosificación, ventajas, desventajas, indicaciones y contraindicaciones específicas para el uso de cada uno de los vasopresores, enfocado en el manejo de patologías que ponen en riesgo la vida, con la finalidad de mejorar el pronóstico de los pacientes críticamente enfermos.
