Fundamento: Las enfermedades cardiovasculares constituyen la principal causa de mortalidad en España. El objetivo del estudio es estudiar la asociación entre la agrupación (clustering) de factores de riesgo cardiovascular y el riesgo de padecer eventos cardiovasculares mayores: cardiopatía isquémica, enfermedad cerebrovascular y arteriopatía periférica de extremidades inferiores. Método: Se realizó un estudio descriptivo transversal, realizado en un centro de salud urbano. Se estudiaron 2.248 historias clínicas de personas con edad igual o superior a 15 años, seleccionadas mediante muestreo aleatorio simple. Los datos se obtuvieron por revisión de las historias clínicas, estimando la odds ratio (OR) para padecer algún evento cardiovascular (n = 224), cardiopatía isquémica (n = 123), enfermedad cerebrovascular (n = 84) y arteriopatía periférica (n = 55) respecto al número de factores de riesgo cardiovascular. Se incluyeron los factores de riesgo cardiovascular tabaquismo, hipertensión arterial, hipercolesterolemia, hipertrigliceridemia, diabetes y obesidad. La OR fue ajustada por edad y sexo. Introducción. Ante las dificultades que implica el tratamiento efectivo de la obesidad en el ámbito de la atención primaria de salud, es importante evaluar alternativas terapéuticas, incluyendo las no financiadas. Objetivos: El objetivo principal del estudio fue evaluar la efectividad del tratamiento y de una unidad funcional de obesidad en práctica clínica real de atención primaria. Asimismo se valoraron los principales parámetros metabólicos relacionados con las comorbilidades asociadas (diabetes tipo 2, hipertensión y dislipemia) y las necesidades de tratamiento farmacológico. Métodos: Se incluyeron pacientes obesos que optaron ser tratados con un método comercializado, después de recibir informados sobre todas las posibilidades terapéuticas (consejo dietético, fármacos, cirugía). El método consistió en un programa multidisciplinar que combina una dieta inicialmente cetogénica, ejercicio físico y soporte emocional. Resultados: El programa fue aceptado por el 87,5% (84/96) de los pacientes a los que se les ofreció, de los cuales el 69% completaron el periodo de dieta cetogénica (paso 3) y el 38,1% alcanzaron la etapa de mantenimiento (Paso 6). La media de pérdida de peso al final del paso 3 fue de 21,40 (7,61) kg y de 24,13 (8,13) kg al llegar al paso 6 (ambos p<0,0001 vs peso inicial). Se registró una mejora significativa de las cifras de tensión arterial, triglicéridos, GOT, GPT y GGT y se redujo la prescripción de fármacos antihipertensivos en 15/18 pacientes y de estatinas en 4/13 pacientes. El tratamiento fue seguro y bien tolerado. Conclusiones: La implementación del programa multidisciplinar para perder peso ha mostrado ser una alternativa viable en la consulta de Atención Primaria para el tratamiento de pacientes con obesidad, mostrando eficacia en condiciones reales a pesar de que esta opción terapéutica no está financiada por el sistema público de salud.
