A atenção farmacêutica é realizada por profissionais farmacêuticos, sendo realizada para avaliar resultados clínicos-laboratoriais e utilizar dos mesmos para melhor conduta junto ao usuário, intervindo na detecção, prevenção e solução de problemas relacionados à farmacoterapêutica. Tal intervenção tem destaque em sujeitos polimedicados, os quais utilizam diversos medicamentos de forma crônica e simultânea para doenças e sintomas distintos, tais pacientes são principalmente idosos, em decorrência da idade avançada e começam a apresentar mudanças fisiológicas. Desta forma, na atual pesquisa objetivou-se compreender a atuação farmacêutica junto a pacientes hipertensos e/ou diabéticos para verificar possíveis melhoras após a intervenção do farmacêutico. Trata-se de uma integrativa de literatura, de natureza qualitativa e exploratória, cuja busca de artigos foi realizada no ano de 2022, entre os meses de maio e junho, filtrando-se os que foram publicados entre os anos de 2017 a 2022, utilizando as bases de dados, Pubmed, ScienceDirect, e BIREME, com o uso dos Descritores em Ciência da Saúde (DeCS): “Diabetes” and “Hypertension” and “aged” and “Pharmaceutical Services”. A intervenção farmacêutica tem influência positiva diante aos cuidados geriátricos, visto que é a maioria da sociedade e está exposta a diversos tipos de medicamentos. E como os serviços farmacêuticos podem diminuir a frequência de Reação adversa a medicamento (RAM). O farmacêutico clínico pode e deve ser primordial nas intervenções da terapia medicamentosa e ao aconselhamento adequado ao tratamento. Este estudo contribui cientificamente, pois evidenciou positividade da intervenção farmacêutica no auxílio a pacientes hipertensos e/ou diabéticos, verificando melhoras após a intervenção do farmacêutico.
