RESUMO Este artigo tem por objetivo contribuir para a reflexão no tocante aos impactos da pandemia Covid-19 na população negra, tendo como marco disparador a necessidade premente de analisar as assimetrias que essa emergência sanitária global produz, particularmente em contextos de desigualdade social, como é o caso do Brasil, em que a população em situação de vulnerabilidade social pode ser representada majoritariamente pela população negra, em seus diferentes grupos específicos, tipificados por gênero, por restrições de acesso a educação, proteção social, moradia adequada, serviços de saneamento básico, internet, bem como por ocupação/desocupação, por espaço geográfico, por privação de liberdade, ainda que paradoxalmente, quantitativamente equivalha a maioria da população brasileira, que acumula os piores indicadores.
Resumo Saúde prisional é, em sua essência, saúde pública. A pandemia de COVID-19 representa uma grande ameaça para o mundo e tem demonstrado que prevenir a escalada da doença em prisões faz parte do combate ao novo coronavírus na sociedade em geral. Sabe-se, até o momento, que a mais efetiva medida de contenção ao avanço da doença é o isolamento social. No entanto, em instituições penais, muitas vezes superlotadas, tal medida torna-se de difícil implementação e, quando acontece, leva a população privada de liberdade a um superisolamento, tendo consequências em sua saúde mental. Além disso, indivíduos presos sofrem com ambientes sem ventilação, falta de materiais de higiene pessoal, condições sanitárias básicas precárias e dificuldade de acesso a serviços de saúde. O presente artigo objetiva ser uma revisão narrativa sobre os efeitos da pandemia em presídios e como governos e sociedade civil têm se organizado a fim de reduzir as consequências sobre esses locais. A publicação foi dividida em três seções: na primeira, há uma revisão da literatura em saúde sobre a temática; na segunda, é tratado o modo como diferentes países estão lidando com a situação carcerária no contexto da pandemia; na terceira e última parte, é abordado o modo como o Sistema Penal brasileiro tem reagido à nova doença.
Prisional health is, in its essence, public health. The COVID-19 pandemic poses a great threat to the world and has shown that preventing the disease escalation in prisons integrates the novel corona virus clash in society in general. Up to this moment, the most effective known measure to curb the disease spread is social isolation. Nevertheless, in penal institutions, often overcrowded, social isolation becomes difficult to carry out and, when it happens, it takes the enclosed population to overisolation, with consequences to their mental health. Besides, prisoners suffer with clogged up environment, lack of materials for personal hygiene, poor basic sanitary conditions and difficulties in accessing health services. In order to reduce the disease spread, several countries are taking measures such as definite or temporary release of prisoners and visiting restrictions. This paper deals with a narrative review on the pandemic effects in prisons and how government and civil society have organized themselves in order to reduce the disease consequences at those places. The text has been divided into three sections: the first with literature review on the current health theme; the second discusses how different countries have been dealing with the prison situation in the pandemic context, and, the last part focuses on how the Brazilian Penal System has reacted to the new disease.
RESUMO Este estudo objetivou descrever a experiência do Brasil e dos Estados Unidos da América (EUA) em relação aos dados de morbimortalidade por Covid-19, segundo a raça/cor/etnia. Para isso, procurou-se descrever os fatores envolvidos no tratamento e divulgação dos dados de morbimortalidade por essa patologia nos dois países. Foram analisados boletins epidemiológicos divulgados pelo Ministério da Saúde, resultados parciais da Pesquisa Nacional por Amostra de Domicílio (PNAD) Covid-19 realizada pelo Instituto Brasileiro de Geografia e Estatística (IBGE) no Brasil e o estado da arte em saúde sobre os impactos da pandemia nos EUA, sob a perspectiva de raça/cor/etnia. Apesar da baixa qualidade da informação em saúde referente à morbimortalidade da população negra por Covid-19, os resultados desvelam iniquidades raciais em saúde para essa doença, ratificando o racismo estrutural/institucional em ambos os países. Como contribuição, enfatiza-se a necessidade de qualificar os dados sobre raça/cor/etnia, relacionando-os com idade, local de moradia, tipo de residência, acesso a saneamento básico, ocupação, entre outros determinantes sociais que, sabidamente, impactam no modo de adoecer e morrer pela Covid-19, a fim de viabilizar estratégias e políticas públicas verdadeiramente promotoras da equidade.
A COVID-19 é uma doença causada pelo SARS CoV-2, de variado espectro clínico, com alta transmissibilidade por via respiratória. No momento, a prevenção mostra-se essencial para enfrentamento dessa pandemia, assim como são necessários o distanciamento e regras de higiene. O novo coronavírus imprimiu uma realidade que demandou rápida adaptação do processo de trabalho dos profissionais nas unidades de saúde no que diz respeito aos cuidados primários em saúde. A telemedicina e suas modalidades vêm se destacando como meio de garantia da continuidade da assistência à saúde da comunidade, bem como para orientação e monitoramento de sintomáticos respiratórios, ratificando o papel da Atenção Primária e seus principais atributos, com destaque para o primeiro contato, a longitudinalidade e a integralidade. Este relato apresenta uma forma de organização da unidade docente-assistencial da Escola Bahiana de Medicina e Saúde Pública, em Salvador, Bahia, tendo o atendimento remoto como um dos instrumentos utilizados, revelando um novo caminho para a continuidade do cuidado da população, bem como um momento de aprendizado para os residentes do Programa de Residência em Medicina de Família e Comunidade, para a preceptoria e usuários do serviço durante a pandemia e que poderá ser incorporada no pós-pandemia por COVID - 19.
