Rare neuromuscular diseases (NDs) are a group of inherited or acquired neurological pathologies affecting the muscles and the nervous system. Their low prevalence and high geographical dispersion can cause isolation and difficulties in social interaction between affected equals. New technologies, such as videoconferencing, offer a complementary option for improving the health of this population. The purpose of this study was to assess the effectiveness of a teleassistance program at improving health-related quality of life (HRQoL) through social interaction in adults with NDs. The sample consisted of 45 participants affected by rare NDs. Twenty-four participants were assigned to the experimental group (EG), which participated in the videoconferencing sessions, and 21 to the control group. Three questionnaires were administered: WHO-DAS II, Sickness Impact Profile, and SF-36 Health Survey. Effectiveness was assessed by a pre-post design. An online psychosocial program was applied over three-month period. Data revealed an improvement of the EG in psychosocial variables, e.g. "Getting along with people" (z = -2.289, r = -.47, p ≤ .05) or "Psychosocial Domain" (z = -2.404, r = -.49, p ≤ .05), and in physical variables, e.g. "Life activities" (z = -2.844, r = -.58, p ≤ .05). Social interaction appeared as a relevant factor at improving HRQoL levels. High levels of satisfaction about the teleassistance program were reported.
En 1985 hemos llevado a cabo la primera tentativa para conocer la magnitud de la enfermedad tuberculosa en nuestro medio (1), pero no fue hasta 1993 y 1996, (2,3) cuando logramos obtener unos índices epidemioló-gicos más cercanos a la realidad, aún constatando que nos quedaban sin constatar los casos de la medicina priv a d a .Con el objetivo de conocer la situación actual hemos recogido y revisado los datos de los años 1995, 1996,1997 y 98, que serán objeto de esta publicación. RESUMENObjetivo: Conocer los parámetros epidemiológicos entre 1995 y 1998 de la enfermedad tuberculosa (TB) en el Área Sanitaria de Santiago de Compostela.Método: Búsqueda activa y exhaustiva de los casos que reunían todos o cada uno siguientes criterios: a) hallazgos microbiológicos y/o patológicos diagnósticos de TB, en cualquier espécimen, b) pacientes menores de 35 años con historia clínica reciente compatible con TB con test de Mantoux con una induración >5 mm tras 48-72 horas de la inyección intradérmica de 2 TU PPD y adenosina deaminasa (ADA) en líqui-do pleural >47 IU/mL.Resultados: Hemos estudiado 1.150 pacientes (685 hombres y 465 mujeres), con edad media (X) de 38,9 (19,8 SD) años, con extremos de 3 meses y 88 años. El número de casos fue de 307 en los años 1995 y 1996, de 302 en 1997 y de 213 en 1998. La incidencia de la enfermedad por 100.000 habitantes (X/100.000H) fue de 78,3 en 1995 y 1996, de 79,8 en 1997 y de 61,9 en 1998, con una incidencia de formas contagiosas de 38,3 (X100.000H) en 1995, de 38,9 en 1996, de 37,2 en 1997 y de 21,9 en 1998. La tasa de meningitis tuberculosa fue de 1,8 (X100.000H) en 1995 y de 1,3 en 1996, 1,05 en 1997 y 0,8 en 1998, destacando que en tres últimos años no se declararon casos en niños menores de 5 años. En las formas de asociación con VIH la cifra fue de 4,2 % en 1995, 3,3% en 1996, 5,4% en 1997 y de 3,2% en 1998. Estratificada la muestra por edades hemos objetivado que más del 50% de los casos en ambos sexos se encontraba entre los 15 y 40 años con otro aumento después de los 65 años. La proporción hombre/mujer (H/M) fue de 1,8 en 1995, de 1,4 en 1996, 1997 y 1998.Conclusiones: La incidencia de tuberculosis ha disminuido en el últi-mo año, aunque es demasiado pronto para saber cuál es la tendencia real. 307 during 1995 and 1996, 302 in 1997 and 213 in 1998. ResumenLa presente investigación ha tenido como objetivo primordial aclarar la etiología de un cuadro clínico caracterizado por edema de las piernas, disestesias con déficit motor en miembros inferiores que, en dos casos, incluyó una cardiopatía que llevó a insuficiencia cardiaca irreversible. Este brote ocurrió entre julio de 1991 y junio de 1993; afectó selectivamente infantes de marina de la Escuela Naval Almirante Padilla en Cartagena de Indias.La metodología se centró en un estudio de casos y controles en el cual se sometió a cada grupo a un cuidadoso interrogatorio, examen clínico, medición por el laboratorio de metales pesados en sangre y orina de 24 horas y estudio de niveles de tiamina en muestras de suero; se analizaron cuatro biopsias de nervio sural y en el estudio postmortem de los fallecidos se puso especial atención a las alteraciones de miocardio. En muestras ambientales y biológicas, se analizó mercurio, plomo, arsénico y talio.Los resultados totalizan un grupo de 22 infantes de marina con un cuadro muy uniforme sobre todo por las manifestaciones de una neuropatía periférica nutricional, lo cual sumado a la cardiopatía fatal en dos pacientes, cuyo estudio tanto en microscopia de luz como electrónica, demostró cambios en todo compatibles con lo observado en beriberi, permiten que se considere que el problema de fondo reune las características del beriberi húmedo.Después de concluir que el problema tenía como fondo un origen nutricional basado, en primer lugar, en una falla de tiamina, se hicieron las recomendaciones y ajustes dietéticos indicados para estos casos con una respuesta inmediata y enteramente satisfactoria. SummaryThe main goal of this paper has been to understand the etiology of a clinical picture characterized by an edema of the legs and dysesthesia with motor dysfunction of the legs. In two cases the clinical picture ended up in a cardiopathy that lead to an irreversible cardiac failure.
