RESUMO: O HPV é a doença anal sexualmente transmissível mais diagnosticada em pacientes HIV positivos. Neste estudo investigamos a taxa de recidiva após tratamento cirúrgico do HPV em pacientes HIV positivos. Foi realizado um estudo retrospecA transmissão é comumente por relação sexual, e é estimado que mais de 50% das pessoas sexualmente ativas tenham sido infectadas por HPV em algum momento de suas vidas.5 O acometimento anal é comum e predomina em grupos com comportamento de risco para DST, antecedentes de tratamento de HPV genital ou anal prévio e portadores de prurido anal idiopático.1,6-9 O HPV também pode ser transmitido por via oral, manual ou vestimentas, pela proximidade com os genitais. 6,10,11 A maioria das infecções por HPV são assintomáticas, sendo que apenas 10% dos pacientes desenvolverão lesões verrucosas ou displasias.3,12,13 Em pacientes HIV positivos, as manifestações anogenitais são mais agressivas e com maior número de recidivas. 3,10,[12][13][14] O grau de imunidade dos pacientes, o vírus HIV e a agressividade viral estão relacionados a maiores taxas de prevalência, severidade, persistência e recidiva no tratamento do HPV. 15,16 O objetivo desse estudo é avaliar a recidiva de lesões associadas ao HPV anal em pacientes HIV positivos após tratamento cirúrgico.
INTRODUÇÃOMesmo diante dos avanços na compreensão da etiopatogenia da doença de Crohn (DC) e de novas opções terapêuticas para o manejo clínico, a grande maioria dos doentes (50-70%) necessita de operação intestinal na evolução da doença 1 , e um número variá-vel destes (33 a 82%) terá a experiência da recidiva no seguimento pós-operatório 2, 3 .Desde a publicação de Crohn et al, em 1932, muitos pesquisadores têm identificado fatores clínicos preditores de recidiva no pós-operatório, porém com resultados conflitantes quanto a sua importância na determinação da recidiva 4, 5 .Diante do exposto, o objetivo deste trabalho foi relatar a experiência do serviço com os fatores de risco que influenciam na recidiva da doença de Crohn no pós-operatório. MATERIAIS E MÉTODOSO estudo retrospectivo descritivo foi realizado no Serviço de Coloproctologia do Hospital Heliópolis, São Paulo -SP, de dezembro de 1998 a dezembro de 2006, em pacientes com diagnóstico de doença de Crohn, submetidos a tratamento cirúrgico.Os dados foram coletados através de revisão rigorosa de prontuários médicos. As variáveis avaliadas foram idade, sexo, raça, tabagismo, tempo de diagnósti-co, padrão anátomo-clínico, localização da doença, motivo da primeira operação, recidiva no pós-operatório, número de procedimentos cirúrgicos e o tipo anastomose. Com relação ao motivo da primeira cirurgia, os pacientes foram divididos em dois grupos: o primeiro grupo com diagnóstico de perfuração livre, perfuração bloqueada com abscesso e perfuração crônica com fistula interna ou externa e o segundo grupo, sem perfuração.
INTRODUÇÃOO linfangioma é uma neoplasia bastante incomum no períneo, havendo alguns casos relatados no espaço retrorretal. Trata-se de uma malformação vascular de baixo fluxo do sistema linfático, podendo ser localizado ou difuso, microcístico, macrocístico (também chamado de higroma cístico) ou misto. 1A literatura limita-se a poucos relatos de casos, e somente estudos maiores têm conseguido proporcionar uma estimativa da incidência global desses tumores na pelve. 2O objetivo deste trabalho é relatar um caso de linfangioma perineal e o tratamento cirúrgico.
RESUMO: O enema opaco, apesar de atualmente apresentar indicações restritas, continua útil na propedêutica radiológica do cólon. A perfuração colorretal é a mais grave complicação do enema opaco, ocorrendo em 0,02 a 0,23% dos exames realizados, com taxa de mortalidade de até 50%. É relatado caso de paciente masculino, 40 anos, há dois meses apresentou dor anal intensa e sangramento durante a realização de enema opaco para investigação etiológica de constipação, com melhora espontânea após um mês. Ao exame proctológico constatou-se infiltração da margem anal, submucosa do canal anal e reto extraperitoneal por sulfato de bário, sem locais de perfuração. O enema opaco trazido pelo paciente e a radiografia simples de pelve após dois meses do exame mostravam perfuração retal incompleta por sulfato de bário. Optado por observação clínica e intervenção cirúrgica se houver complicação. Atualmente o paciente segue em acompanhamento ambulatorial, permanecendo assintomático há 15 meses. INTRODUÇÃOO enema opaco, apesar de atualmente apresentar indicações restritas, continua sendo muito útil na propedêutica radiológica do cólon. (1,2) O estudo radiológico contrastado do cólon é um exame invasivo de fácil realização, podendo raramente levar a complicações catastróficas tais como a perfuração retal ou colônica, megacólon tóxico, septicemia, embolia venosa por bário e enchimento gastrintestinal retrógrado associado a vômitos e aspiração em crianças. (3)(4)(5)(6) A perfuração colorretal é a mais grave complicação do enema opaco, ocorrendo em 0,02 a 0,23% dos exames realizados, com taxa de mortalidade de até 50%. (2,(4)(5)(6)(7)(8) O objetivo deste trabalho é relatar um caso de perfuração incompleta do reto após enema opaco, ocasionando a infiltração do bário na região anorretal. RELATO DO CASOC.J.P.A., masculino, 40 anos, há dois meses apresentou dor anal intensa e sangramento durante a realização de enema opaco para investigação diagnóstica de constipação intestinal. Referiu também grande dificuldade para realização do exame. Evoluiu com prurido e manutenção da dor em região perianal, com melhora espontânea dos sintomas há um mês.No exame proctológico, à inspeção, apresentava na margem anal nodulação na posição lateral direita e placa mediana anterior, ambas de cor esbranquiçada, consistência fibroelástica, medindo aproximadamente 1,5 cm de diâmetro cada (figura 1);
