A presença de inomogeneidades de tecido produz alterações na distribuição da dose absorvida, cuja magnitude depende das propriedades físicas desses tecidos e da qualidade da radiação. A avaliação incorreta da distribuição de dose pode afetar o controle local do tumor ou aumentar as probabilidades de complicações dos tecidos normais. O trabalho teve como objetivo investigar a influência dos efeitos de inomogeneidade de vértebras torácicas sobre a dose absorvida pelo tecido mole circundante. Os valores preditos pelo sistema de planejamento de tratamento (TPS) foram comparados às medidas experimentais com filmes radiocrômicos EBT-2 posicionados em um objeto simulador constituído somente de água e inseridos axialmente em um phantom simplificado de coluna vertebral e água. Observou-se significativa alteração do padrão de distribuição de dose, aumento da dose absorvida na interface osso-tecido mole e doses pontuais altas adjacentes ao osso em comparação com os resultados obtidos para os filmes em meio homogêneo e TPS. As medidas experimentais na água concordaram com o TPS em 1,0% no que se refere à dose modal enquanto que a maior diferença encontrada para o meio contendo as vértebras foi de 4,6%, no entanto, ambos os valores se encontram dentro da incerteza experimental.
A radioterapia constitui a modalidade clínica mais utilizada para tratar mestástases cerebrais, tipo mais comum de tumores secundários de mama, pulmão e melanomas. Os protocolos frequentemente utilizam doses diárias elevadas e, dependendo da técnica de irradiação empregada há elevada probabilidade de complicações em tecidos saudáveis. Com o propósito de minimizar os efeitos adversos, é importante a interpretação dosimétrica do planejamento radioterápico tridimensional por meio de imagens tomográficas ou, no caso de simulações 2D, pelo emprego de técnicas que otimizam a distribuição de dose aumentando a homogeneidade. O estudo objetivou comparar os planejamento 2D e conformacional 3D para a irradiação total de cérebro e verificar a evolução desses planejamentos através das técnica “field-in-field”. A metodologia consistiu em simular o planejamento bidimensional, reproduzi-lo em um conjunto de imagens tomográficas e compará-lo com o plano conformacional, tanto para técnica de dois campos quanto para a de quatro campos “field-in-field”. Os resultados mostraram que não há diferenças significativas entre planejamentos 2D e 3D para irradiação de cérebro total, e que a “técnica field in field” melhora sensivelmente a distribuição de dose no volume cerebral comparada à técnica de dois campos. Concluiu-se que a custo efetividade do tratamento é ótima para os 4 campos descritos e que a tomografia de planejamento não é essencial para o caso clinico abordado.
