Background Extremely drug-resistant gram-negative bacilli (XDR-GNB) increasingly cause healthcare-associated infections (HAIs) in intensive care units (ICUs). Methods A matched case-control (1:2) study was conducted from February 2007 to January 2010 in 16 ICUs. Case and control subjects had HAIs caused by GNB susceptible to ≤1 antibiotic versus ≥2 antibiotics, respectively. Logistic and Cox proportional hazards regression assessed risk factors for HAIs and predictors of mortality, respectively. Results Overall, 103 case and 195 control subjects were enrolled. An immunocompromised state (OR=1.55, p=0.047) and exposure to amikacin (OR=13.81, p<0.001), levofloxacin (OR=2.05, p=0.005), or trimethoprim-sulfamethoxazole (OR=3.42, p=0.009) were factors associated with XDR-GNB HAIs. Multiple factors in both case and control subjects significantly predicted increased mortality at different time intervals after HAI diagnosis. At 7 days, liver disease (Hazard Ratio [HZ]=5.52), immunocompromised state (HR=3.41), and bloodstream infection (HR=2.55) predicted mortality; at 15 days, age (HR=1.02 per year increase), liver disease (HR=3.34), and immunocompromised state (HR 2.03) predicted mortality; and at 30 days, age (HR=1.02 per one year increase), liver disease (HR=3.34), immunocompromised state (HR=2.03), and hospitalization in a medical ICU (HR=1.85) predicted mortality. Conclusions HAIs caused by XDR-GNB were associated with potentially modifiable factors. Age, liver disease, and immunocompromised state, but not XDR-GNB HAIs, were associated with mortality.
A 7 year experiment (2008–2014) evaluated cattle grazing intensity (sward height) effects on herbage mass, forage quality, and greenhouse gas emissions in continuously stocked pastures containing the tropical ‘Marandu’ palisade grass (Brachiaria brizantha (Hochst. ex A. Rich) Stapf cv. Marandu). The experiment consisted of three sward height treatments (15, 25, and 35 cm) and six replicates. There were four periods each year during the rearing phase. Significant effects were found for herbage mass, proportions of leaf and stem, crude protein, neutral detergent fiber, acid detergent fiber, lignin, animal performance, enteric methane (CH4), and greenhouse gas (GHG) emissions from soils. When the canopy height increased from 15 to 35 cm, the herbage mass rose from 5.23 to 9.86 kg t ha−1, leaf percentage decreased, and stem percentage increased. Crude protein content averaged 14.2%, and neutral detergent fiber averaged 58%. Average daily gain averaged 0.67, 0.81, and 0.90 kg −1 head−1, while live weight gain ha–1 was 649, 530, and 439 kg for the 15, 25, and 35 cm treatments, respectively. The weather variables explained the GHG emissions, interannual herbage mass, and structure variations.
O manejo do pastejo e a suplementação estratégica permitem maximizar a produção de carne bovina em pastagens de forma sustentável. A intensidade de pastejo influencia diretamente o crescimento individual, taxas de aparecimento e mortalidade de perfilhos, a determinar o acúmulo de forragem e a estrutura do dossel. Dessa forma, nas águas, é função do manejo do pastejo adequar a frequência e intensidade de desfolhação para que o animal possa colher forragem com idade fisiológica e valor nutritivo adequados. A idade e tamanho dos perfilhos determinam a proporção de tecidos de suporte lignificados que reduzem a digestibilidade da forragem. No período seco, o manejo do pasto e a estratégia de diferimento, ao final do período das águas, são determinantes na obtenção de forragem de melhor valor nutritivo. Assim, o manejo das pastagens visa, primeiramente, à produção de forragens com altos teores de fibra potencialmente digestíveis. A partir de então, a caracterização da quantidade e qualidade da forragem são primordiais à adequação dos nutrientes fornecidos, via suplementos, para otimizar a utilização dos recursos forrageiros basais. A suplementação da dieta dos animais em pastejo, com concentrado, permite aumentar o desempenho dos animais, o que reduz a idade de abate e melhora a qualidade da carcaça e da carne obtida, além dos benefícios na preparação dos animais terminados em confinamento. Portanto, o manejo do pastejo e a suplementação da dieta dos animais permitem aumento de produtividade e maior qualidade dos produtos.
RESUMO -Dois experimentos foram conduzidos com o objetivo de avaliar os efeitos da frequência de suplementação com duas fontes proteicas sobre o desempenho produtivo e econômico e o consumo e a digestibilidade dos nutrientes em bovinos em pastejo sob suplementação durante o período seco do ano. Foram utilizados 20 animais anelorados, não-castrados com 10 meses de idade e peso médio inicial de 208,43 kg, distribuídos em quatro piquetes de capim-marandu (Brachiaria brizantha, cv. Marandu), cada um com 1,6 ha. Ambos os experimentos foram estruturados em arranjo fatorial 2 × 2, com dois suplementos (com farelo de soja e com farelo de algodão de alta energia) e duas frequências de suplementação (diariamente ou três vezes por semana). Não houve interação fontes proteicas × frequências de suplementação para nenhuma das características de desempenho produtivo e econômico. As fontes proteicas não influenciaram o ganho médio diário, mas a frequência de fornecimento afetou o desempenho, que foi melhor nos animais sob suplementação três vezes/semana (0,670 kg/dia). O maior retorno econômico foi obtido com a oferta do suplemento com farelo de algodão três vezes por semana. A frequência de suplementação afetou o consumo de nutrientes, enquanto a fonte proteica do suplemento teve efeito somente sobre o consumo de extrato etéreo, que foi maior nos animais que consumiram o suplemento com farelo de algodão. O suplemento com farelo de algodão alta energia proporcionou maior digestibilidade da proteína bruta da dieta. O fornecimento de suplemento três vezes por semana, além de reduzir os custos da suplementação, possibilita desempenho superior ao obtido com a suplementação diária.A utilização do farelo de algodão de alta energia em suplementos múltiplos em substituição ao farelo de soja é satisfatória, principalmente do ponto de vista econômico.Palavras-chave: frequência de suplementação, ganho de peso, pastagem, suplemento Supplementation frequency and proteins sources for growing of steers in pasture during the dry season: productive and economical performance ABSTRACT -Two experiments were carried out with the objective of evaluating the effects of the frequency of supplementation with two proteins sources on productive and economical performance (experiment 1) and the nutrients digestibility of steers in supplemented pasture during the dry season over a year. The objective of this study was to evaluate the performance of beef heifers on Stapf. cv. Marandu grass pastures that were managed at three herbage allowances under continuous grazing with a variable stocking rate and were provided with mineral mix supplement or supplements with energy and protein. Seventy-two test heifers and 48 put-and-take heifers were used in a randomised design in a 3 × 2 factorial arrangement [three herbage allowances (1.5, 2.0, and 2.5 kg of green dry matter/kg of bodyweight, and two supplementation levels (mineral mix or 0.3% bodyweight of a mineral–protein–energy supplement)], with two replicates per treatment (paddocks), to maintain the pre-established forage allowance. No significant difference was observed in the gain per hectare as a function of the herbage allowance, but 2.5 kg green dry matter/kg bodyweight provided the greatest average daily gain of bodyweight (0.605 kg/animal/day). The energy/protein supplementation provided the greatest average daily gain compared with animals fed only the mineral mix. The supplements in the diet of beef heifers fed at 0.3% of bodyweight increased bodyweight gains without compromising the gain per hectare at different herbage allowances.
